State disciplines health care providers

OLYMPIA ľ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Adams County

In November 2007 the Nursing Assistant Program charged certified nursing assistant Heather S. Smith (NC10073655) with unprofessional conduct. She was convicted of assault of a child.

Chelan County

In November 2007 the Registered Counselor Program charged registered counselor E. Stanley Foster (RC00054054) with unprofessional conduct. Allegations include touching a patient inappropriately during a session, giving inappropriate suggestions regarding a patient’s relationship and leaving a patient at a bus stop to find their own way home instead of driving the client home.

In November 2007 the X-Ray Technician and Health Care Assistant Programs reinstated the licenses of Roxy M. Richards (XT00001074 and HC00141519).  

Clallam County

In November 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Donna A. Reedy (NA00194605). She was found guilty of theft and was placed on probation for two years. She cannot reapply for three years.

Clark County

In November 2007 the Nursing Assistant Program charged nursing assistant Irina N. Gorlova (NA00091485) with unprofessional conduct. She was convicted of theft and allegedly took $130 from a resident at her place of employment.

Island County

In November 2007 the Nursing Assistant Program charged nursing assistant Amber A. McCabe (NA00190081) with unprofessional conduct. She allegedly admitted to using methamphetamines.

Jefferson County

In November 2007 the Nursing Commission reinstated the license of registered nurse Carol Anspach (RN00121232).

In November 2007 the Registered Counselor Program charged registered counselor Raeann M. Van Trojen (RC00054887) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

King County

In November 2007 the Nursing Commission charged registered nurse Pamela Jorgenson (RN00074576) with unprofessional conduct. She allegedly forged prescriptions to obtain narcotics for her own personal use.

In November 2007 the Medical Commission charged physician William H. Levy (MD00021325) with unprofessional conduct. He allegedly used inappropriate language during counseling sessions with a patient and the patient’s mother.

In November 2007 the Nursing Assistant Program charged nursing assistant Leah Presleigh (NC10071105) with unprofessional conduct. She was convicted of assault – domestic violence and allegedly failed to respond to department inquiries regarding the conviction.

In November 2007 the Registered Counselor Program amended the statement of charges against registered counselor Dean Taylor (RC00016289). He allegedly engaged in a sexual relationship with a client.

Kitsap County

In November 2007 the Nursing Assistant Program charged nursing assistant Christine J. Simonson (NC10031483) with unprofessional conduct. She allegedly stole money from patients and was convicted of attempted violation of the uniform controlled substance act – possession of oxycodone and acetaminophen. Simonson allegedly failed to respond to department inquiries during the investigation.

Pierce County

In November 2007 the Dental Commission entered into an agreed order with dentist Robert T. Erickson (DE00006354). He employed, supervised, directed and/or allowed a non-licensed practitioner to place intravenous lines and deliver medications to patients before and during oral surgery. He must comply with the terms and conditions set against his license.

In November 2007 the Nursing Assistant Program charged nursing assistant Wilfredo Ortiz (NA00121470) with unprofessional conduct. He allegedly punched a patient in the face and/or abdomen.

Skagit County

In November 2007 the Denturist Program charged denturist Kevin C. Kowalski (DN00000254) with unprofessional conduct. He allegedly provided a patient with a full set of dentures that did not meet the standard of care.

Snohomish County

In November 2007 the Dental Commission entered into an agreed order with dentist Michael A. Dire (DE00004072). He diverted drugs in order to have them administered to his race horses for financial gain. He must comply with the terms and conditions placed against his license.

In November 2007 the Chiropractic Commission charged chiropractor Shawn L. Gay (CH00003609) with unprofessional conduct. He allegedly failed to re-examine a patient for 56 more treatments after the initial examination.

In November 2007 the Board of Pharmacy charged pharmacy assistant applicant David H. Smith (CA00065706) with unprofessional conduct. He is allegedly unable to practice with reasonable skill and safety to the public.

Spokane County

In November 2007 the Nursing Assistant Program amended the statement of charges against nursing assistant Patricia J. Campbell (NA00004440). She allegedly borrowed money from patients without repaying them and had sexual contact with a patient.

In November 2007 the Nursing Commission reinstated the license of registered nurse John G. Parr (RN00107014).

Thurston County

In November 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Gregory S. Benner (NA00194946). He was convicted of driving under the influence, reckless burning and criminal trespass. He failed to respond to department inquiries regarding the convictions. Benner cannot reapply for seven years.

Whatcom County

In November 2007 the Unlicensed Practice Program issued a notice of intent to issue cease and desist order to Kristin Morrow. She allegedly provided massage treatments to a patient. She does not hold a license to practice as a massage therapist.

In November 2007 the Nursing Commission charged registered nurse Albert C. Snow (RN00114689) with unprofessional conduct. He allegedly failed to follow up on a patient’s medial alert status of low blood pressure, failed to follow up on STAT lab results and failed to assess and document the patient’s symptoms.

Yakima County

In November 2007 the Nursing Assistant Program charged certified nursing assistant Christina O. Pacheco (NC10068585) with unprofessional conduct. She was convicted of assault.

In November 2007 the Nursing Assistant Program charged nursing assistant Enrique Pola (NC10025918) with unprofessional conduct. He was convicted of driving with a suspended license, driving under the influence, hit and run and assault – domestic violence.

Out of State

Arizona: In November 2007 the Medical Commission charged physician James L. Sameshima (MD00026001) with unprofessional conduct. He was issued a letter of reprimand by the Arizona State Board for practicing below the standard of care.

California: In November 2007 the Surgical Technologist Program charged surgical technologist applicant Lucio R. Bonilla (ST00003701) with unprofessional conduct. He pled guilty to two charges in California. He also allegedly failed to respond to department inquiries regarding pleas.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
