OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In November 2007 the Nursing Assistant Program charged nursing assistant applicant Crystal M. Gonzales (NA00195255) with unprofessional conduct. She was convicted of theft.
In November 2007 the Nursing Assistant Program charged certified nursing assistant Yolonda G. Pugh (NC10069979) with unprofessional conduct. She was convicted of theft.
In November 2007 the Nursing Assistant Program charged nursing assistant applicant Kellie L. Resser (NA00195101) with unprofessional conduct. She was convicted of identity theft.
In November 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Tammi J. Kindle (NA00055305). She was convicted of burglary and violation of the uniform controlled substance act – possession of methamphetamine and concealed the convictions on her application. She cannot reapply for seven years.
In November 2007 the Nursing Assistant Programs charged nursing assistant applicant Coffee Blue (NA00195457) with unprofessional conduct. She was convicted of attempted forgery and forgery and pled guilty to attempted theft. She allegedly failed to disclose the convictions on her application and failed to respond to department inquiries regarding the convictions.
In November 2007 the Nursing Assistant Program charged nursing assistant Jon C. Debey (NC10052345) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his license.
In November 2007 the Registered Counselor and Licensed Social Worker Programs charged registered counselor and licensed social worker applicant Lynn Devin (RC00025112 and LW00009352) with unprofessional conduct. She was convicted of conspiracy to commit money laundering and conspiracy to commit visa fraud. Devin allegedly failed to notify the department of the conviction as required by law.
In November 2007 the Nursing Commission charged licensed practical nurse Cynthia D. Halvorson (LP00046604) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In November 2007 the Nursing Assistant Program charged nursing assistant Valerie R. Pagel (NA00165459) with unprofessional conduct. She was convicted of negligent driving and assault.
In November 2007 the Chemical Dependency Professional and Licensed Mental Health Counselor Programs charged chemical dependency professional and licensed mental health counselor Carol Van Buren (CP00000124 and LH00010800) with unprofessional conduct. She allegedly submitted an altered verification of post-graduate experience and was unable to prove she had completed the required experience.
In November 2007 the Nursing Assistant Program charged nursing assistant Adrian B. Hernando (NA00169082) with unprofessional conduct. Hernando was convicted of assault.
In November 2007 the Nursing Assistant Program charged nursing assistant reactivation applicant Jennifer L. Leard (NA00131974) with unprofessional conduct. She was convicted of driving under the influence on two different occasions and failed to respond to department inquiries regarding the convictions.
In November 2007 the Nursing Assistant Program charged nursing assistant Benjamin S. Asaeli (NA00155150) with unprofessional conduct. He was convicted of two counts of murder, assault and possession of a stolen firearm.
In November 2007 the Nursing Assistant Program charged nursing assistant applicant Gardeh Y. Blanton (NA00195449) with unprofessional conduct. He was convicted of weapons apparently capable of producing bodily harm – unlawful carrying or handling and malicious mischief. Blanton allegedly failed to disclose the convictions on his application and failed to respond to department inquiries regarding the convictions.
In November 2007 the Nursing Assistant Program charged certified nursing assistant applicant Sheila F. Kendrick (NC10036019) with unprofessional conduct. She was convicted of shoplifting and domestic violence – criminal assault on two different occasions. Kendrick allegedly failed to disclose the convictions on her application. She is also known as Sheila F. White.
In November 2007 the Registered Counselor Program charged registered counselor Jamie S. Sweatman (RC00054295) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In November 2007 the Nursing Assistant Program denied the reactivation application of nursing assistant Erica C. Hurtado (NA00144748). She was convicted of unlawful issuance of bank check – single check over $250 and two counts of violation of anti-harassment order. She cannot reapply for five years.
In November 2007 the Nursing Assistant Program charged nursing assistant applicant Jennifer R. Ren (CA00048096) with unprofessional conduct. She was convicted of soliciting possession of a controlled substance, making a false or misleading statement to a public servant, no contact order violation and protection order violation. She allegedly failed to respond to department inquiries regarding the convictions.
In November 2007 the Chiropractic Commission charged chiropractor Michael A. Baker (CH00001832) with unprofessional conduct. He allegedly directed his unlicensed staff members to perform different procedures without a licensed chiropractor present and billed for treatment which was not actually given, potentially benefiting his clinic.
In November 2007 the Nursing Assistant Program charged nursing assistant applicant Tanya P. Myers (NA00195932) with unprofessional conduct. She was convicted of minor in possession and/or consumption and theft. She allegedly failed to disclose the convictions on her application.
In November 2007 the Nursing Assistant Program amended the statement of charges against nursing assistant applicant Laura A. Hooper (NA00194947). She was convicted of unlawful issuance of bank checks or drafts and malicious mischief. As a result, the program denied her application. Hooper cannot reapply for three years.
In November 2007 the Dental Commission entered into an agreed order with dentist Sherilyn V. Malloy (DE00006693). She practiced below the standard when restoring a patient’s tooth, failed to adequately document the care given to patients and administered conscious sedation without obtaining a conscious sedation permit. She must comply with terms and conditions set against her license.
In November 2007 the Nursing Assistant Program charged nursing assistant Kamillie L. Concepcion (NC10097575) with unprofessional conduct. She allegedly was sleeping at her place of employment during her shift and failed to respond to department inquiries regarding the incident.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
