State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In November 2007 the Nursing Commission charged licensed practical nurse Laurie L. Bohnstengel (LP00046293) with unprofessional conduct. She was convicted of attempting to obtain controlled substances through fraud.

Chelan County

In November 2007 the Psychology Board reinstated the license of psychologist Glen A. Frese (PY00000990).

In November 2007 the Medical Commission charged physician Kenneth M. Jones (MD00028268) with unprofessional conduct. He allegedly aided and abetted unlicensed practice of medicine by allowing unlicensed persons to evaluate clients, provide injections, use his name to obtain and use equipment and use his DEA number to obtain medications. Jones was the medical director for a Seattle area clinic.

King County

In November 2007 the Nursing Commission charged registered nurse Fawaz B. Al-Mohamad (RN00111181) with unprofessional conduct. He was convicted of attempted assault – domestic violence.

In November 2007 the Dental Commission entered into an agreed order with dentist Robert Buda (DE00007787). He practiced with an expired license for approximately five months. He must comply with the terms and conditions set against his license.

In November 2007 the Registered Counselor Program granted the application of registered counselor applicant Sarahlynne S. Cieslikowski (RC00058161) and placed her license on probation for three years. She was convicted of assault of a child. Cieslikowski must comply with the terms and conditions set against her license. She is also known as Sarah L. Cieslikowski.

In November 2007 the Registered Counselor and Chemical Dependency Professional Programs reinstated the credentials of registered counselor and chemical dependency professional James H. Goff (RC00014404 and CP00003696).

In November 2007 the Registered Counselor Program amended the statement of charges against registered counselor Florence Greff (RC00003333). She allegedly misrepresented herself as a psychologist on more than one occasion and she also failed to provide adequate disclosure statements to patients.

In November 2007 the Nursing Commission denied the application of registered nurse applicant Leah R. McCray (CA00066551). Her licenses to practice as a nursing assistant and a health care assistant were revoked. McCray failed to respond to the statement of charges.

In November 2007 the Registered Counselor Program reinstated the credential of registered counselor Brandy L. Oto (RC00049768).

Kitsap County

In November 2007 the Registered Counselor Program granted the application of registered counselor applicant Dawn Wilson (RC00058553) and placed her credential on probation for three years. Wilson was convicted of assault. She must comply with terms and conditions set against her license.

Mason County

In November 2007 the Medical Commission charged physician Donald R. Miller (MD00026759) with unprofessional conduct. Miller allegedly failed to respond to department inquiries regarding a complaint filed against his license.

Pierce County

In November 2007 the Nursing Assistant Program charged certified nursing assistant Angie A. Fox (NC10059117) with unprofessional conduct. While working as a home caregiver, she allegedly abandoned a patient.

In November 2007 the Dental Commission entered into an agreed order with dentist Abbie Hage (DE00006890). She practiced below the standard of care by failing to identify, diagnose and treat the decay of a tooth on a patient resulting in the tooth having to be extracted. She must comply with terms and conditions set against her license.

In November 2007 the Nursing Commission charged licensed practical nurse Julie A. Luxton (LP00046946) with unprofessional conduct. She allegedly made errors documenting medicine administration and failed to comply with previous terms and conditions set against her license.

In November 2007 the Orthotics and Prosthetics Program placed the licensed of orthotist and prosthetist Justin Maughan (OI00000326 and PS00000183) on probation for one year. While under his supervision, another individual practiced prosthetics without a credential and orthotics with an expired credential. Maughan must comply with terms and conditions set against his license.

Skagit County

In November 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Valencia Reed (NA00190316). She pled guilty to assault and concealed the conviction on her application. Reed cannot reapply for five years.

In November 2007 the Nursing Assistant Program denied the application for nursing assistant Ashlee R. Spangler (NA00193496). Spangler was charged with two counts of burglary and one count of trafficking in stolen property. She failed to disclose the charges on her application. She cannot reapply until she has complied with the court and her criminal charges are dismissed.

Snohomish County

In November 2007 the Health Care Assistant Program denied the application of health care assistant applicant Adela G. Hussen (HC00153657). She was convicted of theft and concealed the conviction on her application. Hussen was also denied a credential to practice as a nursing assistant. She cannot reapply for five years.

Spokane County

In November 2007 the Medical Commission charged physician William T. Roth (MD00035230) with unprofessional conduct. He allegedly entered into a sexual relationship with a patient.

In November 2007 the Nursing Commission charged registered nurse Jeanne M. Tritle (RN00077010) with unprofessional conduct. She allegedly delegated the care of her patient to her family members who were not licensed to practice.

Thurston County

In November 2007 the Nursing Commission charged licensed practical nurse Dara L. Rodriguez (LP00050159) with unprofessional conduct. Rodriguez allegedly failed to comply with previous terms and conditions set against her license.

Yakima County

In November 2007 the Nursing Commission charged licensed practical nurse Kimberly K. Martinez (LP00054708) with unprofessional conduct. She allegedly submitted an altered CPR card to a prospective employer.

Out of State

Virginia: In November 2007 the Nursing Commission charged licensed practical nurse Matthew J. McFarland (LP00057281) with unprofessional conduct. He voluntarily relinquished his license to practice as a licensed practical nurse in Florida. McFarland also allegedly diverted drugs from a patient for his own personal use.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
