State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In November 2007 the Nursing Assistant Program granted the application of applicant Victoria A. Cuello (NA00191993). She was convicted of driving under the influence on two different occasions and hit and run of an unattended vehicle. Cuello must comply with the terms and conditions set against her license.

Chelan County

In November 2007 the Nursing Commission reinstated the license of registered nurse Ernest B. Mauk, Jr. (RN00096067) and placed his license on probation. He must comply with terms and conditions set against his license.

Clallam County

In December 2007 the Nursing Assistant Program charged Raymond Grissett (NC10092817) with unprofessional conduct. He allegedly performed a procedure outside his scope of practice as a nursing assistant and failed to respond to department inquiries regarding the incident.

Clark County

In December 2007 the Medical Commission charged physician Rod I. Belkin (MD00025808) with unprofessional conduct. Allegations include writing a prescription for a person who was not his patient, writing prescriptions without examining or diagnosing the patient and failing to chart prescriptions given to the patient.

In December 2007 the Nursing Assistant Program charged applicant Vadim V. Sashchenko (NC10098996). He was convicted of driving under the influence on two different occasions, driving while his license suspended on two different occasions and criminal trespass. He allegedly failed to respond to department inquiries regarding the convictions.

Cowlitz County

In November 2007 the Registered Counselor Program denied the application of registered counselor applicant Erik M. Richardson (RC00057648). Richardson was convicted of theft on three different occasions. He cannot reapply for three years.

King County

In December 2007 the Nursing Assistant Program placed the license of Kevin P. Bernadt (NA00182206) on probation for two years. He applied a topical treatment to a patient without completing the nurse delegation required to administer medication. He must comply with the terms and conditions set against his license.

In November 2007 the Medical Commission suspended the license of physician Janis Bridge (MD00035256). The suspension is stayed and she must comply with terms and conditions. She was convicted of assault, driving under the influence, attempting to elude and driving while her license was suspended/revoked.

In November 2007 the Licensed Marriage and Family Therapist Program reinstated the license of licensed marriage and family therapist Vickie A. Tomlinson (LF00001157).

Kitsap County

In December 2007 the Nursing Commission charged registered nurse Mary Elizabeth Mullen (RN00122217). Allegations include allowing a 10 year old to wheel a patient to their bed and failing to properly administer medication to a patient.

In November 2007 the Nursing Assistant Program denied the application of applicant Nicholas C. Oliver (NA00195453). He was convicted of possession of marijuana with intent to manufacture or deliver. He cannot reapply for five years.

Lewis County

In November 2007 the Nursing Assistant Program entered into an agreed order with Kathryn M. Coyne (NC10060684). She admitted to using drugs and controlled substances on several occasions. She must comply with the terms and conditions set against her license.

Pierce County

In November 2007 the Registered Counselor Program reinstated the license of registered counselor Walter L. Harris (RC00050217).

In December 2007 the Nursing Assistant Program charged Centell T. Jackson (NA00150556) with unprofessional conduct. Jackson was convicted of aiming or discharging firearms and assault.

In December 2007 the Nursing Commission charged registered nurse Mary A. Johnson (RN00128013) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

In November 2007 the Medical Commission placed the license of physician assistant Kristie M. Mattern (PA10003450) on probation for an infinite period of time. She failed to comply with previous terms and conditions set against her license and failed to cooperate with department investigations regarding her adult family home license. She must comply with the terms and conditions set against her license.

Spokane County

In December 2007 the Medical Commission placed the license of John P. Moulton (MD00015837) on probation for five years. He crossed professional boundaries by entering into a romantic relationship with a patient. He must comply with the terms set against his license.

In December 2007 the Nursing Assistant Program charged Tina L. Voorhees (NC10036604) with unprofessional conduct. She allegedly roughly repositioned a patient and spoke harshly to him.

In November 2007 the Nursing Commission placed the license of licensed practical nurse Kesne Winton (LP00045250) on probation for 18 months. He failed to administer medications to patients and falsely documented administration of the medications. Winton must comply with the terms and conditions set against his license.

Thurston County

In December 2007 the Nursing Assistant Program charged Romeo V. Aquino (NC10098059) with unprofessional conduct. He was convicted of assault – domestic violence.

Out of State

Arizona:  In December 2007 the Nursing Commission charged registered nurse and advance registered nurse practitioner Penny L. Fairman (RN00145459 and AP30006209) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

Colorado:  In December 2007 the Nursing Assistant Program charged Melinda M. Bob (NC10062058) with unprofessional conduct. She was convicted of assault – domestic violence and allegedly failed to respond to department inquiries regarding the conviction.

New York:  In December 2007 the Medical Commission denied the application of physician applicant Lloyd G. Bayme (MD00047874). He surrendered his license to practice as a physician in New York after numerous allegations of unprofessional conduct. He was convicted of conspiracy and criminal sale of a prescription and was denied a medical license in Delaware and North Dakota. He cannot reapply until he provides proof of an unrestricted credential in the state of New York.

Oregon:  In November 2007 the Nursing Commission granted the application of registered nurse applicant Darrah Buitron (RN00169134). She failed to disclose on her application that her license to practice as a registered nurse in Texas was suspended. She must comply with the terms and conditions set against her license.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
