State disciplines health care providers

OLYMPIA ľ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clallam County

In December 2007 the Nursing Assistant Program charged Aimee L. Chaffin (NA00131146) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

In December 2007 the Registered Counselor Program denied the application of registered counselor applicant Donna A. Reedy (RC00058305). She was convicted of theft. Reedy cannot reapply for three years.

Clark County

In December 2007 the Nursing Assistant Program charged certified nursing assistant applicant Tina M. Bacus (NC10038212) with unprofessional conduct. She was convicted of theft.

In December 2007 the Health Care Assistant Program charged Shirley E. Combs (HC00146247) with unprofessional conduct. She allegedly submitted a prescription refill without authorization and obtained the drugs for her own use.

In December 2007 the Dental Commission charged Wallace V. Teuscher (DE00002435) with unprofessional conduct. Allegations include failing to review medical history, medication lists and allergies of patients, failing to record testing, diagnosis and justification for extracting teeth, failing to provide appropriate post-operative instructions and follow-up appointments and failing to date the entry of notes entered in the treatment charts.

Cowlitz County

In December 2007 the Registered Counselor Program charged Marty L. Paquette (RC00054417) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his license.

In December 2007 the Nursing Assistant Program charged certified nursing assistant Sara L. Stone (NC10081946) with unprofessional conduct. Her license to practice as a health care assistant was suspended for failure to comply with a previous order and conviction of driving under the influence.

Franklin County

In December 2007 the Board of Physical Therapy charged Melinda Irvine (PT00005994) with unprofessional conduct. She allegedly failed to report the several years of her business partner’s inappropriate conduct to the department even though staff notified her of the conduct.

King County

In December 2007 the Registered Counselor Program denied the application of registered counselor applicant Mark K. Bratsch (RC00056423). He was convicted of six counts of indecent liberties for sexually abusing a minor. He cannot reapply for 20 years.

In December 2007 the Nursing Commission reinstated the license of licensed practical nurse Jessie A. Hollins (LP00018654) and placed her license on probation for six months. She must comply with the terms and conditions set against her license.

In November 2007 the Medical Commission placed the license of physician Kinne D. McCabe (MD00017145) on probation for two years. He failed to be present at a study he was supposed to supervise, failed to sign consent forms before the study took place and failed to examine any of the patients in the study he agreed to supervise. McCabe must comply with terms and conditions set against his license.

Kitsap County

In December 2007 the Nursing Assistant Program charged nursing assistant applicant Chaundra Murphy-Gould (NA00196571) with unprofessional conduct. She was convicted of theft and allegedly failed to disclose the conviction on her application. She also allegedly failed to respond to department inquiries regarding the conviction.

In December 2007 the Nursing Assistant Program charged nursing assistant applicant Christopher Randall (NA00197484) with unprofessional conduct. He was convicted of assault and allegedly failed to disclose the convictions on his application.

Mason County

In December 2007 the Nursing Assistant Program charged nursing assistant applicant Amy J. Bowles (NC10009180) with unprofessional conduct. She was convicted of forgery and allegedly failed to respond to department inquiries regarding the conviction.

Pierce County

In December 2007 the Nursing Assistant Program charged nursing assistant applicant Jarret A. Abeel (NA00197504) with unprofessional conduct. He pled guilty to assault and possession of stolen property. He allegedly failed to disclose his convictions on his application and failed to respond to department inquiries regarding the convictions.

In December 2007 the Chiropractic Commission placed the license of Danny D. Baldwin (CH00003681) on probation for two years. He failed to comply with a previous agreed order and submitted billings not supported by patient records. Baldwin must comply with terms and conditions set against his license.

In December 2007 the Nursing Commission amended the statement of charges against licensed practical nurse Maria E. Lengele (LP00055419). She allegedly failed to ensure a physician was timely notified of a patient’s critical lab results and failed to document the lab results on the patient’s charts.

In December 2007 the Nursing Commission charged registered nurse Barbara S. Oxborrow (RN00123482) with unprofessional conduct. She allegedly withdrew medications from the dispensing machine after she documented giving the medications to the patients and on more than on occasion failed to document the administration of the medication.

Spokane County

In December 2007 the Radiologic Technologist and Health Care Assistant Programs charged radiological technologist and health care assistant Randy L. Hughes (RT00000628 and HC00128444) with unprofessional conduct. He was convicted of child molestation and allegedly failed to report his conviction to the department.

In December 2007 the Massage Program denied the reactivation application of massage therapist Nadine Miller (MA00008066). Miller was convicted of three counts of forgery, unlawful possession of a controlled substance – marijuana, illegal drug conduct and unlawful possession of a controlled substance – heroin. She cannot reapply for 10 years.

Thurston County

In December 2007 the Health Care Assistant Program modified the agreed order against Julie R. Hinchcliff (HC00141491). She must comply with terms and conditions set against her license.

Whatcom County

In December 2007 the Nursing Assistant Program charged Tracy A. Muder (NC10078566) with unprofessional conduct. She was convicted of driving under the influence, negligent driving, operation of a vehicle without ignition interlock, disorderly conduct and shoplifting.

In December 2007 the X-Ray Technician Program charged x-ray technician Jeni L. Wolford (XT00005315) with unprofessional conduct. Her license to practice as a licensed practical nurse was suspended and her application to practice as a registered nurse was denied based on the criminal conviction of attempted forgery.

Out of State

Nebraska: In December 2007 the Medical Commission charged physician Ziad L. Zawaideh (MD00023457) with unprofessional conduct. He entered into a non-disciplinary Assurance of Compliance with the state of Nebraska and agreed not to provide obstetrical care in the future to any patients.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
