OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In December 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Crystal M. Gonzales (NA00195255). She was convicted of theft and failed to respond to department inquiries regarding the conviction. She cannot reapply for five years.
Chelan County
In December 2007 the Nursing Commission charged licensed practical nurse Rhonda J. Couch (LP00052344) with unprofessional conduct. She was convicted of forgery and theft.
Clark County
In December 2007 the Chemical Dependency Professional Program modified the order against chemical dependency professional Robert B. Collins (CP00004146). Collins’ suspension was lifted and his license is placed on probation for three years. He must comply with the terms and conditions set against his license.
In December 2007 the Registered Counselor Program charged Nahid F. Razzaghy (RC00032818) with unprofessional conduct. She allegedly failed to consider a client’s legal driving record while conducting an assessment related to a driving under the influence citation.
Grant County
In December 2007 the Nursing Assistant Program denied the application of certified nursing assistant applicant Danielle A. Ackerlund (NC10099104). She was convicted of passing bad checks and forgery. She failed to respond to department inquiries regarding the convictions. Ackerlund cannot reapply for five years.
Grays Harbor County
In December 2007 the Registered Counselor Program charged registered counselor applicant Ryan D. Donahue (RC00057899) with unprofessional conduct. His license to practice as a temporary licensed psychologist in Kansas was restricted and he allegedly failed to disclose the action on his application.
King County
In December 2007 the Board of Pharmacy placed the license of pharmacist Wayne Engstrom (PH00007938) on probation for one year. While he was the responsible pharmacy manager of a pharmacy, he failed to properly supervise his pharmacy technicians and other employees in regard to the requirement to counsel the patient or patient’s agent on new prescriptions. He must comply with the terms and conditions set against his license.
In December 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Destinie M. Erskine (NA00191933). She was convicted of theft and unlawful issuance of bank checks. She failed to respond to department inquiries regarding the convictions. She cannot reapply for five years.
In December 2007 the Registered Counselor Program charged registered counselor applicant Gina R. Martinez (RC00059152) with unprofessional conduct. Martinez was convicted of delivery of cocaine, theft on three different occasions, burglary and violation of pseudoephedrine restrictions. She is also known as Gina R. Swenson.
In December 2007 the Dental Commission modified the agreed order against dentist Wonzel M. Mobley (DE00004032). He must comply with the terms and conditions set against his license.
In December 2007 the Marriage and Family Therapist Program reinstated the license of marriage and family therapist C. Brian Wilcox (LF00000841).
Okanogan County
In December 2007 the Nursing Assistant Program charged nursing assistant applicant Amanda M. Axtell (NA00195192) with unprofessional conduct. She was convicted of criminal trespass, delivery of a controlled substance – marijuana and assault. A previous application was denied and there are terms and conditions upon her reapplication.
Pierce County
In December 2007 the Nursing Assistant Program denied the application of nursing assistant applicant Gardeh Y. Blanton (NA00195449). He was convicted of weapons apparently capable of producing bodily harm – unlawful carrying or handling and one count of malicious mischief. He failed to disclose the information on his application and failed to respond to department inquiries regarding the convictions. Blanton cannot reapply for 10 years.
In December 2007 the Nursing Assistant Program denied the application of certified nursing assistant applicant Jacqueline M. Issac (NC10055775). She was convicted of malicious mischief and failed to respond to department inquiries regarding the conviction. She cannot reapply for five years.
In December 2007 the Dental Commission charged Debra Hopkins (DE00006126) with unprofessional conduct. She allegedly made 11 substandard restorations which resulted in the patient not being able to bite correctly.
In December 2007 the Nursing Assistant Program denied the application of certified nursing assistant applicant Sheila F. Kendrick (NC10036019). She was convicted of domestic violence – criminal assault on two different occasions and shoplifting. She failed to disclose the information on her application and respond to department inquiries regarding the convictions. Kendrick cannot reapply for five years. She is also known as Sheila F. White.
In December 2007 the Chiropractic Commission placed the license of Burl R. Pettibon (CH00000551) on probation for two years. Pettibon performed chiropractic adjustments while his license was expired. He must comply with the terms and conditions set against his license.
Snohomish County
In December 2007 the Registered Counselor Program charged Jeff P. Fosdick (RC00057286) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his license.
In December 2007 the Chiropractic Commission placed the licensed of Marleen L. Meinzer (CH00003432) on probation for two years. Meinzer failed to adequately document treatment by failing to specify patient’s complaints, indicate changes in the patient’s care, indicate progress and include a code legend. She must comply with the terms and conditions set against her license. She is also known as Marleen L. Brozovich.
In December 2007 the Nursing Commission reinstated the licenses of registered nurse and advanced registered nurse practitioner Karen Nichols-Hoppe (RN00049096 and AP30001487).
Spokane County
In December 2007 the X-Ray Technician Program entered into an agreed order with x-ray technician applicant Justin T. Arbury (XT00007082). He had a deferred prosecution for driving under the influence and convicted of assault. He failed to disclose the criminal activity on his application. Arbury must comply with the terms and conditions set against his license.
Out of State
Idaho: In December 2007 the Board of Osteopathic Medicine and Surgery charged osteopath Carlos W. Bond (OP00001666) with unprofessional conduct. His Idaho license is restricted and he pled guilty to obtaining a controlled substance by misrepresentation.
Oregon: In December 2007 the Registered Counselor and Chemical Dependency Professional Programs charged registered counselor and chemical dependency professional Sally A. Lambert (RC00008445 and CP00000869) with unprofessional conduct. She allegedly failed to consider a client’s legal driving record while conducting an assessment related to a driving under the influence citation.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.###
