State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In December 2007 the Medical Commission reinstated the license of physician Janet S. Arnold (MD00025273).

Chelan County

In January 2008 the Nursing Assistant Program charged certified nursing assistant applicant Laurie J. White (NC10060248) with unprofessional conduct. Her license to practice as a nursing assistant in Oregon was indefinitely suspended for misuse of heroin.

Cowlitz County

In January 2008 the Registered Counselor Program modified an earlier order and placed registered counselor Eli A. Bouchea (RC00020663) on stayed suspension. She must comply with the terms and conditions set against her license.

Douglas County

In January 2008 the Board of Pharmacy charged pharmacy intern Sara M. Cunningham (IR00056885) with unprofessional conduct. She allegedly diverted controlled substances for her own use and failed to respond to department inquiries regarding the accusations.

Franklin County

In January 2008 the Chiropractic Commission charged chiropractor applicant Bradley L. Akridge (CH00001829) with unprofessional conduct. He was convicted of possession of cocaine on two different occasions and theft. He also allegedly failed to maintain patient records as required.

Island County

In January 2008 the Chiropractic Commission lifted the suspension against the license of chiropractor Bruce Linehan (CH00002852). He must meet requirements to renew his license.

King County

In January 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Coffee Blue (NA00195457). She was convicted of attempted theft, attempted forgery and forgery. She failed to disclose the convictions on her application and failed to respond to department inquiries regarding the convictions. Blue cannot reapply for 10 years.

In January 2008 the Registered Counselor Program charged registered counselor applicant Kimbal C. Brown, Sr. (RC00058732) with unprofessional conduct. He was convicted of assault – domestic violence and assault.

In January 2008 the Nursing Assistant Program reinstated the license of certified nursing assistant Linda K. Ferrel (NC10023835).

In January 2008 the Nursing Assistant and Massage Therapy Programs reinstated the licenses of certified nursing assistant and massage therapist Heather D. Francis (NC10033500 and MA00017901). She is also known as Heather Robinson.

In January 2008 the Massage Program charged Brian C. Jewett (MA00021909) with unprofessional conduct. He allegedly forged prescriptions and filled the medications. He was convicted of solicitation to possess oxycontin.

In January 2008 the Nursing Commission charged licensed practical nurse Melissa M. Somerville (LP00056942) with unprofessional conduct. Allegations include administering intravenous medications without proper supervision and injecting a patient with intravenous medication left in a syringe from another patient.

Kitsap County

In January 2008 the Registered Counselor Program charged registered counselor applicant Jamie R. Wright (RC00058256) with unprofessional conduct. Wright was convicted of possession of stolen property and possession of methamphetamine. She is also known as Jamie Rayola Wilson.

Lewis County

In January 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Brianna L. Bishop (NA00196955). She pled guilty to possession of cocaine and possession of drug paraphernalia. She also failed to respond to department inquiries regarding the criminal activities. Bishop cannot reapply for seven years.

In January 2008 the Licensed Mental Health Counselor Program amended the statement of charges against licensed mental health counselor Helgi S. Douay (LH00007168). She allegedly treated a married couple, terminated counseling with one of the patients without explanation and without referring the patients to another counselor, continued to counsel the other patient, and later began a friendship with one of the patients.

In January 2008 the Nursing Assistant Program charged Anthony M. Taylor (NA00171170) with unprofessional conduct. He was convicted of violating a domestic violence court order and assault – domestic violence.

Pierce County

In January 2008 the Nursing Assistant Program reinstated the license of Mary L. Osborn (NC10054047).

In January 2008 the Board of Pharmacy charged pharmacy assistant applicant Alvin M. Uhl (VB00072550) with unprofessional conduct. He was convicted of theft and allegedly failed to disclose the conviction on his application.

In January 2008 the Medical Commission reinstated the license of physician Nelson J. Williams (MD00026207).

Snohomish County

In January 2008 the Nursing Commission charged licensed practical nurse Herco C. Ordona (LP00049311) with unprofessional conduct. Allegations include documenting dispensing medications to patients when the medications were not given, dispensing medications that had been discontinued by the patient’s physicians, dispensing a medication contrary to doctor’s orders and dispensing medications but failing to ensure the medications had actually been ingested.

Spokane County

In December 2007 the Board of Physical Therapy stayed the suspension of physical therapist Daniel Roberts (PT00002482). He must comply with terms and conditions set against his license.

In January 2008 the Board of Pharmacy charged pharmacist Michael F. White (PH00018978) with unprofessional conduct. He allegedly failed a urinalysis and took hydrocodone pills from another person’s filled prescription.

Out of State

California: In January 2008 the Surgical Technologist Program denied the application of surgical technologist applicant Lucio R. Bonilla (ST00003701). He pled guilty to battery and making a criminal threat in California. Bonilla failed to respond to department inquiries regarding his criminal activities. He cannot reapply for five years.

Idaho: In January 2008 the Nursing Assistant Program placed the license of certified nursing assistant Tina L. Voorhees (NC10036604) on probation for one year. She repositioned a patient and spoke harshly to him causing the patient to become afraid and tearful. She must comply with the terms and conditions set against her license.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
