OLYMPIA ľ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Chelan County
In January 2008 the Health Care Assistant Program charged health care assistant Nathan W. Rouse (HC00149801) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his license.
King County
In January 2008 the Dental Commission entered into an agreed order with dentist Ashok A. Lalli (DE00005520). He kissed a patient and touched the chest of another patient. He must comply with the terms and conditions set against his license.
In January 2008 the Medical Commission modified an order against physician Leonico G. Panlasigui (MD00021383). He must comply with the terms and conditions set against his license.
In January 2008 the Dental Commission reinstated the license of dentist David G. Thomas (DE00006772).
In January 2008 the License Social Worker Program modified an order against licensed social worker Ruth Van De Carr (LW000004938) and placed her license on probation for six months. She must comply with the terms and conditions set against her license.
Kitsap County
In January 2008 the Massage Program charged Elaine L. Hernandez (MA00013169) with unprofessional conduct. She was convicted of assault with sexual motivation.
In January 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Chaundra Murphy-Gould (NA00196571). She was convicted of theft and failed to respond to department inquiries regarding the conviction. She cannot reapply for five years.
In January 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Christopher P. Randall (NA00197484). Randall was convicted of assault on two different occasions and denied the convictions on his application. He cannot reapply for five years.
Lewis County
In January 2008 the Surgical Technologist Program charged surgical technologist applicant Wendy R. Justus (ST00003784) with unprofessional conduct. She was convicted of assault, malicious mischief and assault – domestic violence on more than one occasion. She is also known as Wendy Phillips, Wendy Whitlow and Joyce R. Coleman.
Mason County
In January 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Amy J. Bowles (NC10009180). Bowles was convicted of forgery and failed to respond to department inquiries regarding the conviction. She cannot reapply for five years.
In January 2008 the Nursing Assistant Program granted the application of nursing assistant applicant Rebecca J. Elam (NA00139412) and entered into an agreed order with her. She was convicted of possession of methamphetamine and theft. Elam must comply with the terms and conditions set against her license. She is also known as Rebecca J. Fillmore.
In January 2008 the Medical Commission reinstated the license of physician assistant Michael S. Myers (PA10003073).
Okanogan County
In January 2008 the Nursing Assistant Program charged certified nursing assistant Rachel A. Bainard (NC10088972) with unprofessional conduct. She allegedly diverted a patient’s pain medication for her own use. She allegedly failed to respond to department inquiries regarding the convictions.
Pend Oreille County
In January 2008 the Nursing Assistant and Health Care Assistant Programs charged nursing assistant and health care assistant Debby N. Holter (NA00174934, HC00152799 and CA00010830) with unprofessional conduct. She was found guilty of obtaining or attempting to obtain a controlled substance by fraud. This crime was allegedly committed while she was working at a health care facility. She is also known as Deborah N. Henderson.
Pierce County
In January 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Jarret A. Abeel (NA00197504). He pled guilty to assault and possession of stolen property and failed to respond to department inquiries regarding the criminal activities. He cannot reapply for five years.
Skagit County
In January 2008 the Nursing Assistant Program charged certified nursing assistant Tammy K. Vega (NC10076977) with unprofessional conduct. She allegedly attended her shift with methamphetamine in her system.
Spokane County
In January 2008 the Medical Commission denied the application of physician applicant Gabriel A. Campbell (MC00019824). He was convicted of driving under the influence, reckless driving, theft and negligent driving and failed to disclose the convictions on his application. Also he did not complete two years postgraduate medical training in a program acceptable to the commission.
Whatcom County
In January 2008 the Nursing Assistant Program reinstated the license of certified nursing assistant Jana M. Travenshek (NC10095744).
Out of State
Alaska: In January 2008 the Nursing Commission charged registered nurse Curtis Meacham (RN00074469) with unprofessional conduct. He allegedly failed to adequately assess, report, document and/or intervene when a patient showed signs of severe respiratory distress with declining oxygen saturation and vital signs.
Arizona: In January 2008 the Medical Commission reprimanded physician James L. Sameshima (MD00026001). He was issued a letter of reprimand from the Arizona Medical Board because he failed to verify a medication list and signed a form resulting in the administration of the wrong medications to a patient. He must comply with the terms and conditions set against his license.
Kansas: In January 2008 the Nursing Commission charged registered nurse applicant Janette C. Delorme-Denton (RN00132230) with unprofessional conduct. Her license to practice as a registered nurse in Oregon was suspended. She allegedly failed to respond to department inquiries regarding the Oregon order.
New Mexico: In January 2008 the Board of Nursing Home Administrators charged nursing home administrator Kerry L. Hansen (NH00001434) with unprofessional conduct. He allegedly took money from his place of employment or the patients. He failed to ensure the facility policies were followed in regards to the patient’s personal monies.
Oregon: In January 2008 the Nursing Assistant Program charged nursing assistant applicant Guadalupe Romero (NA00198386) with unprofessional conduct. Romero was convicted of assault and malicious mischief and allegedly failed to disclose the convictions on her application. She is also known as Maria G. Romero Flores.
Texas: In January 2008 the Medical Commission placed the license of physician Robert Lonian (MD00036163) on probation and he must comply with the terms and conditions set against his license. His license to practice in Texas is subject to supervision and other terms and conditions.
Texas: In January 2008 the Medical Commission permanently denied the application of physician applicant Aliyah Qureshi (MC00016036). She does not meet the requirements for a credential to practice as a physician and surgeon.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
