State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Professions Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In December 2007 the Nursing Commission indefinitely suspended the license of licensed practical nurse Laurie L. Bohnstengel (LP00046293). Bohnstengel was convicted of attempting to obtain controlled substances through fraud.

In January 2008 the Nursing Commission withdrew the statement of charges against registered nurse Debbie K. Knouf (RN00114725).

Clark County

In January 2008 the Registered Counselor Program indefinitely suspended the credential of Deborah K. Osborn (RC00033923). Osborn failed to comply with a previous order set against her credential.

In December 2007 the Dental Commission indefinitely suspended the license of Charles J. West (DE00007238). West provided dental treatment that was below the standard of care.

Jefferson County

In December 2007 the Secretary of Health withdrew the statement of charges against Raeann M. Van Trojen (RC00054887).

King County

In January 2008 the Massage Program revoked the credential of Charlie Y. Cheng (MA00004389). Cheng was convicted on two count of second degree rape.

In January 2008 the Nursing Assistant Program indefinitely suspended the credential of Jon C. Debey (NC10052345). Debey failed to comply with conditions set against his credential.

In December 2007 the Medical Commission indefinitely suspended the license of physician William N. Kay (MD00040609). Kay’s license to practice medicine in Oregon was revoked.

In December 2007 the Nursing Commission indefinitely suspended the license of licensed practical nurse Jon J. Lohr (LP00057834). Lohr’s license to practice in Colorado was suspended.

In December 2007 the Nursing Assistant Program revoked the credential of Valerie R. Pagel (NA00165459). Pagel was convicted of first degree negligent driving and third degree assault.

In December 2007 the Nursing Commission withdrew the statement of charges issued against Peggy A. Rutan (RN00064845).

In December 2007 the Nursing Assistant Program suspended the credential of Hibo Sahal (NC10090607). Sahal attempted to transfer a patient from a bed to a wheel chair. The patient fell to the floor and fractured his ankle.

Mason County

In December 2007 the Licensed Mental Health Counselor Program revoked the credential of Preston L. Carbary (LH00006880). Carbary was convicted on three counts of second degree rape.

In January 2008 the Board of Pharmacy indefinitely suspended the license of Albert C. Legault (PH00010560). Legault failed to comply with a previous order set against his license.

Pierce County

In January 2008 the Nursing Assistant Program indefinitely suspended the credential of Benjamin S. Asaeli (NA00155150). Asaeli was convicted of first degree murder, first degree assault, second degree murder and possession of a stolen firearm.

In December 2007 the Registered Counselor Program indefinitely suspended the credential of Johnny L. Evans (RC00045213). Evans failed to comply with terms set against his credential.

In January 2008 the Nursing Assistant Program indefinitely suspended the credential of Dominique Fleming (NC10084935). Fleming took a necklace from a patient without paying for it.

In January 2008 the Nursing Assistant Program suspended the credential of Centell T. Jackson (NA00150556). Jackson was convicted of aiming or discharging a firearms — dangerous weapons and third degree assault.

In January 2008 the Nursing Commission summarily suspended the license of registered nurse Heather M. Thompson (RN00157172). Thompson’s license to practice as a registered nurse was suspended in Arizona. She is also known as Heather Mary Peters and Heather Mary Stubbs.

Snohomish County

In December 2007 the Health Care Assistant Program revoked the credential of Devone M. Crane (HC00131825). Crane was convicted of assault and driving under the influence.

In December 2007 the Nursing Assistant Program denied the application of Jennifer R. Ren (CA00048096). Ren was convicted of soliciting possession of a controlled substance, making a false or misleading statement to a public servant, no contact order violation and a protection order violation.

Spokane County

In December 2007 the Nursing Assistant Program revoked the credential of Patricia J. Campbell (NA00004440). Campbell had sexual contact with a developmentally delayed adult patient. She also borrowed money from residents and did not repay the money.

In January 2008 the Nursing Commission indefinitely suspended the license of registered nurse Jeanne M. Tritle (RN00077010). Tritle delegated the care of a juvenile patient to her daughter who suffers from a developmentally disability.

Thurston County

In January 2008 the Nursing Commission indefinitely suspended the license of Dara L. Rodriguez (LP00050159). Rodriguez failed to comply with previous conditions set against her license.

In January 2008 the Licensed Mental Health Counselor Program withdrew the amended statement of charges against Gayle L. Zeller (LH00007534).

In January 2008 the Board of Pharmacy withdrew the statement of charges against pharmacist Linda B. Wixom (PH00015132).

Walla Walla County

In December 2007 the Medical Commission indefinitely suspended the license of David W. Shoemaker (MD00029733). Shoemaker’s Oregon medical license was indefinitely suspended for repeated acts of negligence.

Whatcom County

In December 2007 the registered counselor Program indefinitely suspended the credential of Albert E. White (RC00054826). White failed to comply with a previous order.

In December 2007 the Unlicensed Practice Program issued a cease and desist order against Kristin Morrow. She provided massage treatments to a client but does not hold a credential to practice massage in the state of Washington.

Out of State

California: In December 2007 the Board of Hearing and Speech revoked the license of Mir Jafarinejad (HA00000845). He provided substandard care to several patients, kept incomplete and inaccurate records and practiced beyond his authorized scope of practice.

California: In December 2007 the Nursing Commission suspended the license of licensed practical nurse Dennis S. Pratt (LP00055862). Pratt diverted controlled substances for his own use. He also directed nursing assistants to practice outside their scope of practice.

Idaho: In January 2008 the Nursing Commission summarily suspended the license of registered nurse Avis G. Kinzer (RN00124319). Kinzer’s license to practice as a registered nurse was revoked in Idaho.

Idaho: In January 2008 the Nursing Commission indefinitely suspended the license of registered nurse John D. Bartosh (RN00136194). Bartosh failed to comply with a previous order.

Illinois: In January 2008 the Medical Commission summarily suspended the license of physician Charles H. Dickerson (MD00039603). Dickerson’s license to practice medicine was revoked in California.

Virginia: In December 2007 the Nursing Commission indefinitely suspended the license of licensed practical nurse Matthew J. McFarland (LP00057281). McFarland voluntarily relinquished his Florida license based on allegations of substance abuse. While employed in Washington, he diverted oxycodone from a patient for his own use.
