State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Professions Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Cowlitz County

In January 2008 the Nursing Assistant Program suspended the credential of Sara L. Stone (NC10081946). Stone’s health care assistant credential was suspended based on conviction of driving under the influence.

Clallam County

In January 2008 the Nursing Assistant Program suspended the credential of Aimee L. Chaffin (NA00131146). Chaffin failed to comply with previous conditions set against her credential.

Clark County

In January 2008 the Board of Pharmacy withdrew the statement of charges against pharmacist Shelton Louie (PH00015946).

In January 2008 the Registered Counselor Program suspended the credential of Nahid Razzaghy (RC00032818). Razzaghy failed to consider a client’s legal driving record in the DUI assessment being done on the client.

King County

In January 2008 the Nursing Commission suspended the credential of registered nurse Fawaz B. Al-Mohamad (RN00111181). Al-Mohamad was convicted of third degree attempted assault.

In January 2008 the Respiratory Care Practitioner Program revoked the credential of Kathy Benson (LR00000973). Benson is unable to practice because of her substance abuse.

In January 2008 the Nursing Assistant Program suspended the credential of Angela Culpepper (NC10056800). Culpepper was convicted of second degree assault, possession of stolen property and controlled substance violation.

In January 2008 Michelle Sawyer entered into an agreed order to permanently cease and desist from engaging in any and all conduct constituting the practice of massage in Washington. She gave massages but does not have a massage therapy license.

In January 2008 the Medical Commission dismissed charges against Sjardo S. Steneker (MD00032063). Steneker is exonerated from all accusations.

In January 2008 the Nursing Assistant Program revoked the credential of Angela J. Watson (NC10011779). While working as a home caregiver for vulnerable adults, Watson borrowed over $3000 from two patients.

Kitsap County

In January 2008 the Nursing Assistant Program suspended the credential of Adrian B. Hernando (NA00169082). Hernando was convicted of third degree assault.

In January 2008 the Nursing Assistant Program suspended the credential of Ashley L. Hyde (NC10075785). Hyde admitted to using her patient’s debit card without authorization, totaling over $2000 in charges.

Pierce County

In January 2008 the Nursing Assistant Program suspended the credential of Lyndell F. James (NC10041285). James was convicted of third degree theft.

In December 2007 the Secretary of Health withdrew the statement of charges against chemical dependency professional O.C.K. Leontine (CP00006093).

In January 2008 the Nursing Assistant Program suspended the credential of Joseph Mackin (NC10077854). Mackin failed a workplace drug test.

Snohomish County

In January 2008 the Board of Osteopathic Medicine and Surgery withdrew the statement of charges against Melissa S. Chinn (OP00001921).

In February 2008 the Registered Counselor Program withdrew the statement of charges against Jeff P. Fosdick (RC00057286).

In January 2008 the Secretary of Health withdrew the statement of charges against Sandra J. Swanson (HC00125331).

Spokane County

In January 2008 the Nursing Assistant Program revoked the credential of Karl D. Anderson (NA00179854). Anderson was convicted of child rape.

In January 2008 the Nursing Assistant Program revoked the credential of Elizabeth A. Bentancourt (NC10021390). Bentancourt stole money from a patient’s locked security box.

In January 2008 the Health Care Assistant Program suspended the credential of Michael D. Easley, Jr. (HC00146254). Easley failed to comply with previous conditions set against his credential.

In January 2008 the Nursing Commission suspended the credential of Patricia A. Doyle (LP00017953). Doyle failed to comply with previous terms set against her license.

In January 2008 the Radiologic Technologist and Health Care Assistant Programs revoked the credentials of Randy L. Hughes (RT00000628 and HC00128444). Hughes was convicted of second degree child molestation.

In January 2008 the Nursing Commission indefinitely suspended the license of registered nurse Colleen J. Wagner (RN00046949). Wagner failed to comply with previous conditions set against her license.

Thurston County

In February 2008 the Board of Pharmacy revoked the credential of pharmacy technician Mary A. Takatsui (VA00048429). Takatsui tried to fill three forged prescriptions for herself.

In January 2008 the Nursing Assistant Program suspended the credential of Susie R. Towne (NC10075645). Towne failed to comply with previous terms set against her credential.

In January 2008 the Nursing Assistant Program suspended the credential of Rachael J. Wilbur (NC10094344). Wilbur provided substandard care to three patients.

Whatcom County

In January 2008 the Registered Counselor Program indefinitely suspended the credential of David Ditullio (RC00054675). Ditullio failed to comply with a previous order.

In January 2008 the Midwifery Program revoked the credential of Leslie Gesner (MW00000219). Gesner practiced below the standard of care resulting in the death of a newborn.

In January 2008 the Nursing Assistant Program revoked the credential of Tracy A. Muder (NC10078566). Muder was convicted of driving under the influence, first degree negligent driving, operating a vehicle without ignition interlock, disorderly conduct and shoplifting.

Yakima County

In January 2008 the Nursing Commission suspended the license of Kimberly K. Martinez (LP00054708). Martinez altered information on her CPR card and provided it to her prospective employer.

Out of State

Missouri: In January 2008 David S. Debyle entered into an agreed order with the Medical Commission to voluntarily surrender his license to practice as a physician in the state of Washington. His Missouri license is on probation.

Montana: In January 2008 the Registered Counselor Program indefinitely suspended the credential of Martine Welty (RC00028653). Welty failed to comply with a previous order.

Oregon: In January 2008 the Nursing Assistant Program revoked the credential of Laura K. Hubbert (NA00169495). Hubbert’s Oregon nursing assistant credential was revoked.
