State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clark County

In February 2008 the Nursing Assistant Program charged Jessica L. Davidson (NA00194610) with unprofessional conduct. She allegedly verbally abused aged and vulnerable patients and failed to assist them when needed. She allegedly failed to submit a written statement to the department regarding the allegations.

Franklin County

In January 2008 the Board of Pharmacy placed the license of pharmacist Jesus L. Alvarez (PH00020548) on probation for three years. He failed to record an updated prescription and refilled the outdated prescription.

Grays Harbor County

In January 2008 the Board of Pharmacy released the license of Jack H. Jones (PH00007554) from all restrictions.

King County

In February 2008 the Registered Counselor Program charged registered counselor applicant James E. Huff (RC00018816) with unprofessional conduct. He was convicted of possession of heroin on more than one occasion and possession of cocaine on more than one occasion.

In January 2008 the Registered Counselor Program denied the application of registered counselor applicant Gina R. Martinez (RC00059152). She was convicted of delivery of cocaine, theft on more than one occasion, burglary, and violation of pseudoephedrine restrictions. Martinez cannot reapply for two years. She is also known as Gina R. Swenson.

In February 2008 the Nursing Assistant Program charged certified nursing assistant Afemuya Mesfin (NC10002622) with unprofessional conduct. She allegedly pulled the hair of an 86-year old patient and jerked her head back.

In February 2008 the Nursing Assistant Program charged nursing assistant applicant Tiana M. Payran (NA00116596) with unprofessional conduct. She was convicted of attempted theft.

In January 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Jennifer L. Pesta (NA00191999). She was convicted of attempted theft. She cannot reapply until August 2008.

In February 2008 the Nursing Assistant Program charged nursing assistant applicant Florante L. Sobreo (NA00196013) with unprofessional conduct. She was convicted of attempted theft and allegedly failed to disclose the conviction on her application and failed to respond to department inquiries regarding the conviction.

In January 2008 the Dispensing Optician Program reinstated the license of dispensing optician apprentice Pascal Tran (DT10005610).

In February 2008 the Health Care Assistant Program charged health care assistant applicant Julio M. Vernaza (HC60003262) with unprofessional conduct. He was convicted of attempted theft.

In January 2008 the Registered Counselor Program charged Ruby E. Vaughan (RC00055987) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

Kitsap County

In January 2008 the Chiropractic Commission imposed monitoring conditions on chiropractor Carl R. Botefuhr (CH00002461). He entered into an inappropriate personal relationship with a patient. He must comply with terms and conditions set against his license.

In January 2008 the Registered Counselor Program charged registered counselor applicant Cameron Y. Fisher (RC00058679) with unprofessional conduct. Fisher was convicted of forgery.

In January 2008 the Board of Pharmacy reinstated the license of pharmacy technician Danielle T. Nguyen (VA00050330).

Pierce County

In January 2008 the Board of Pharmacy granted the termination of probation against pharmacist David J. Filler (PH00015516).

In February 2008 the Radiological Technologist Program reinstated the license of radiological technologist Jerrie D. Guillermo (RT00005303).

In January 2008 the Board of Pharmacy denied the application of pharmacy assistant applicant Stephanie O. Kennedy (VB60002009). She was convicted of false public assistant verification and concealed the conviction on her application. She cannot reapply for 10 years.

In February 2008 the Nursing Assistant Program charged nursing assistant applicant Kila J. Linton (NA00199469) with unprofessional conduct. She was convicted of shoplifting.

In January 2008 the Board of Pharmacy granted the application of pharmacy assistant applicant Alvin M. Uhl (VB00072550) and placed his license on probation for two years. He was convicted of theft and failed to disclose the conviction on his application. Uhl must comply with terms and conditions set against his license.

Snohomish County

In January 2008 the Board of Pharmacy denied the application of pharmacy assistant applicant David H. Smith (VB60002000). Smith cannot perform the job of a pharmacy assistant with reasonable skill and safety. He cannot reapply for 10 years.

Spokane County

In February 2008 the Board of Pharmacy charged pharmacy technician Desirae A. Gutierrez (VA00060721) with unprofessional conduct. She allegedly tested positive for controlled substances during a urinalysis.

Thurston County

In February 2008 the Nursing Commission charged licensed practical nurse Kenneth D. Saucke (LP00027619) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his license.

Whatcom County

In January 2008 the Board of Pharmacy reinstated the license of pharmacist Jatinder S. Bhatia (PH00041178).

Yakima County

In January 2008 the Health Care Assistant Program charged health care assistant applicant Elizabeth C. Palacios (HC00153859) with unprofessional conduct. She was convicted of theft and allegedly failed to disclose the conviction on the application.

In February 2008 the Health Care Assistant Program charged Tamera J. Williamson (HC00148677) with unprofessional conduct. She allegedly ordered Vicodin on more than one occasion under different doctors’ names without authorization from the doctors. She allegedly ordered the drugs for her own use.

Out of State

Idaho: In February 2008 the Nursing Commission lifted the summary suspension of registered nurse Avis Sabrina G. Kinzer (RN00124319) and imposed terms and conditions on her license. Her license to practice in the state of Idaho was revoked because of failure to comply with a previous order.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
