OLYMPIA ľ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In February 2008 the Nursing Assistant Program charged nursing assistant applicant Stacy K. Erickson (NA00200058) with unprofessional conduct. She was convicted of petty larceny and theft.
In February 2008 the Registered Counselor Program charged Christopher A. Becker (RC00059148) with unprofessional conduct. He was convicted of assault on two different occasions.
In February 2008 the Registered Counselor Program granted a credential to registered counselor applicant Paul F. Jones (RC00058218) and placed it on probation for three years. Jones was convicted of crimes including unlawful possession of methamphetamine, unlawful imprisonment, and attempting to elude. He must comply with terms and conditions set against his license.
In February 2008 the Registered Counselor Program vacated a default order against Nahid F. Razzaghy (RC00032818). He has been charged with unprofessional conduct and the case will go to hearing.
In February 2008 the Health Care Assistant Program charged Joel D. Richardson (HC00135653) with unprofessional conduct. He allegedly accessed a patient’s personal and medical information and used that information to locate the patient’s address and enter illegally to obtain prescription drugs. He was convicted of residential burglary.
In February 2008 the Nursing Assistant Program charged certified nursing assistant Frank S. Moldenhauer (NC10063770) with unprofessional conduct. He allegedly took pain medications from patients for his own use. He was convicted of burglary.
In January 2008 the Nursing Commission reinstated the license of registered nurse Susan Akin (RN00081476) and placed her license on probation. She must comply with terms and conditions set against her license.
In February 2008 the Chiropractic Commission charged chiropractor Joseph C. Ayele (CH00003391) with unprofessional conduct. He allegedly failed to respond to department inquiries regarding a complaint submitted against his license and the chiropractic and massage facility he owns.
In February 2008 the X-Ray Technician Program charged x-ray technician applicant Adela G. Hussen (XT00006781) with unprofessional conduct. Her earlier applications for nursing assistant and health care assistant credentials were denied because she was convicted of theft.
In February 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Tiana M. Payran (NA00116596). She was convicted of attempted theft. She cannot reapply for five years.
In February 2008 the Chemical Dependency Professionals Program denied the application of chemical dependency professional applicant Kimberli S. Kibler (CP00003921). She was convicted of possession of cocaine on two different occasions. She cannot reapply for five years.
In February 2008 the Nursing Assistant Program denied the application of Kerri L. Zeall (NA00198164. She was convicted of assault and failed to disclose the conviction on her application.
In February 2008 the Chiropractic Commission reinstated the license of chiropractor Jeffrey B. Kahrs (CH00002112).
In January 2006 (correct) the Medical Commission terminated oversight of physician Klaus D. Mergener (MD00039380).
In February 2008 the Nursing Assistant Program charged nursing assistant applicant Raksa Son (NA000198237) with unprofessional conduct. Son was convicted of assault – domestic violence and two counts of protection order violation – domestic violence. She allegedly failed to respond to department inquiries regarding the convictions.
In February 2008 the Licensed Mental Health Counselor and Chemical Dependency Professional Programs charged Robert D. Baize (LH00005969 and CP00005724) with unprofessional conduct. He allegedly entered into an inappropriate relationship with a patient.
In January 2008 the Nursing Commission reinstated the license of licensed practical nurse Jacquelyne L. Cameron (LP00030487) and placed her license on probation. She must comply with terms and conditions set against her license.
In February 2008 the Nursing Assistant Program charged Kenneth C. Hargrove (NA00178241) with unprofessional conduct. He was convicted of three counts of child molestation.
In February 2008 the Nursing Assistant Program charged certified nursing assistant Linda Hing (NC10080674) with unprofessional conduct. She pled guilty to assault.
In February 2008 the Nursing Commission charged licensed practical nurse Teresa L. Page (LP00035469) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In February 2008 the Nursing Commission and the Nursing Assistant Program amended the statements of charges against Ethan Hansen (NC10084604, RN00170167 and LP00057938). He allegedly assaulted two children.
In February 2008 the Registered Counselor Program ended restrictions on the credential of James A. Lippold (RC00015144).
In February 2008 the Chiropractic Commission placed the license of chiropractor Willis S. Muncey (CH00002393) on probation for one year. He practiced below the standard of care by failing to adequately document treatment, diagnosis, and changes in the condition of the patient. He must comply with terms and conditions set against his license.
In February 2008 the Board of Pharmacy placed the license of pharmacist John R. White (PH00017312) on probation for 10 years. He must comply with terms and conditions set against his license.
In February 2008 the Nursing Assistant Program charged nursing assistant applicant Pillarr Harmon (NA00193017) with unprofessional conduct. She was convicted of theft.
In February 2008 the Board of Pharmacy charged pharmacist Magdalene Atem-Ossiander (PH00019524) with unprofessional conduct. She allegedly filled a prescription inaccurately and failed to comply with previous terms and conditions set against her license.
In February 2008 the Chiropractic Commission placed the license of chiropractor Marvin Kunikiyo (CH00002656) on probation for two years. He failed to keep proper records and failed to record a justification for number of x-rays taken. He must comply with terms and conditions set against his license.
In February 2008 the Nursing Assistant Program granted a credential to nursing assistant applicant Teresa J. Alatorre (CA00048265) and placed it on probation for one year. She was convicted of driving under the influence. She must comply with terms and conditions set against her license.
Texas: In February 2008 the Medical Commission charged physician William R. Sheldon (MD00027296) with unprofessional conduct. The Texas Medical Board put conditions on his practice.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
