OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Professions Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s Web site. Click on "Provider Credential Search" on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Chelan County
In February 2008 the Secretary of Health withdrew charges against health care assistant Nathan W. Rouse (HC00149801).
Clallam County
In February 2008 the Nursing Assistant Program suspended the credential of Raymond T. Grissett (NC10092817). Grissett performed a procedure that was outside his scope of practice.
Clark County
In February 2008 the Nursing Assistant Program suspended the credential of Irina N. Gorlova (NA00091485). Gorlova stole $130 from a resident and was convicted of third degree theft.
Cowlitz County
In February 2008 the Registered Counselor Program indefinitely suspended the credential of Marty L. Paquette (RC00054417). Paquette failed to comply with previous terms set against her credential.
Grays Harbor County
In February 2008 the Board of Physical Therapy withdrew the statement of charges against physical therapist Sky R. Goudey (PT00002582).
Island County
In February 2008 the Nursing Assistant Program indefinitely suspended the credential of Amber A. McCabe (NA00190081). McCabe admitted to a department investigator that she used methamphetamine.
King County
In February 2008 the Registered Counselor Program indefinitely suspended the credential of Vickie T. Anderson (RC00056803). Anderson failed to comply with a previous order.
In February 2008 the Medical Commission indefinitely suspended the license of physician John R. Brinkley (MD00014634). Brinkley failed to respond to an investigator regarding a complaint made against him.
In February 2008 the Respiratory Care Practitioners Program suspended the credential of Erica N. Dimmer (LR00003506). Dimmer failed to comply with previous terms set against her credential.
In February 2008 the Health Care Assistant Program suspended the credential of Jayna Bried Fuentes (HC00147975). Fuentes failed to comply with previous terms against set against her credential. She is also known as Mary J. Bried Fuentes.
In February 2008 the Health Care Assistant Program indefinitely suspended the credential of Betty J. Harris-Morgan (HC00010770). Her nursing assistant certificate was suspended in March 2007 when Harris-Morgan failed to comply with previous terms set against her credential.
In February 2008 the Registered Counselor Program suspended the credential of Ruby E. Vaughan (RC00055987). Vaughan failed to comply with previous terms set against her credential.
Mason County
In February 2008 the Nursing Commission indefinitely suspended the license of licensed practical nurse Shelley A. Dixon (LP00018169). Dixon failed to comply with a previous order. She is also known as Shelley A. Dodge.
Pierce County
In February 2008 the Denturist Program suspended the credential of Traci B. Chisley (DN00000357). Chisley accepted money for services rendered but did not record the payments or give the money to her employer.
In February 2008 the Nursing Assistant Program indefinitely suspended the credential of Harold J. Harding (NA00148165). Harding’s health care assistant credential was suspended in October 2007 when he failed to comply with previous terms set against the credential.
In January 2008 Ronald G. Lentz (RC00033716) voluntarily surrendered his credential to practice as a registered counselor in the state of Washington. Lentz made sexual comments to female clients and represented himself as a doctor and a mental health counselor when he does not hold a license to practice either profession. Lentz also disclosed confidential information without client consent.
In January 2008 the Nursing Assistant Program suspended the credential of Lyndell F. James (NC10041285). James was convicted of third degree theft. She is also known as Lyndell F. Hines.
In January 2008 the Nursing Commission suspended the license of licensed practical nurse Julie A. Luxton (LP00046946). Luxton documented she provided medication to a patient, when she did not. She also failed to comply with previous terms and conditions set against her license.
In January 2008 the Nursing Commission indefinitely suspended the license of registered nurse Barbara S. Oxborrow (RN00123482). Oxborrow withdrew medications from the dispensing machine after she documented giving the medications to patients and on more than one occasion failed to document the administration of medications withdrawn from the machine.
Skagit County
In February 2008 the Nursing Assistant Program suspended the credential of Tammy K. Vega (NC10076977). Vega worked as a nursing assistant with methamphetamine in her system.
Snohomish County
In February 2008 the Nursing Assistant Program revoked the credential of Foday Conta (NC10082412). Conta had a sexual relationship with a vulnerable patient and continued the sexual relationship after the patient transferred to another hospital.
Spokane County
In January 2008 the Nursing Commission indefinitely suspended the license of licensed practical nurse Carol G. Johnson (LP00021874). Johnson placed a pillowcase over the face of an elderly dementia patient to prevent the patient from biting or spitting at her.
Stevens County
In February 2008 Nursing Assistant Program suspended the credential and the Health Care Assistant Program denied the application of Debby N. Holter (HC00152799 and NA00174934). Holter was convicted of obtaining or attempting to obtain a controlled substance by fraud.
Whatcom County
In January 2008 the Pharmacy Board suspended the license of pharmacist Thomas O’Leary (PH00009950). O’Leary diverted drugs from his place of employment for his own use.
In February 2008 Jan Templeton entered into an agreed order with the Unlicensed Practice Program to permanently cease and desist in any and all conduct constituting the practice of medicine and the unlicensed representation as a certified nutritionist in the state of Washington.
Yakima County
In February 2008 the Nursing Assistant Program suspended the credential of Zenaida S. Gatica (NC10050307). Gatica was convicted of third degree assault and obstructing a law enforcement officer.
Out of State
Arizona: In January 2008 the Nursing Commission suspended the licenses of registered nurse and advanced registered nurse practitioner Penny L. Fairman (RN00145459 and AP30006209). Fairman failed to comply with previous terms set against her licenses.
