State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for 57 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In March 2008 the Dental Commission charged Gregory A. Johnson (DE00007170) with unprofessional conduct. He allegedly failed to diagnose decay on a patient’s teeth, failed to use anesthetic prior to treating the patient, and failed to produce and maintain sufficient chart notes.

Clallam County

In March 2008 the Registered Counselor Program charged Eddie Dost (RC00052988) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his credential.

Clark County

In March 2008 the Nursing Commission charged registered nurse Jennifer A. Green (RN00111529) with unprofessional conduct. She was convicted of criminal mistreatment.

In March 2008 the Massage Program charged David R. Harris (MA00022698) with unprofessional conduct. He was fined by the State of Oregon Board of Massage Therapists for violation of Oregon law.

In March 2008 the Nursing Assistant Program charged Clayton E. Lane (NC10062707) with unprofessional conduct. He allegedly touched a patient in an inappropriate manner. He also allegedly failed to respond to department inquiries regarding the allegation.

Grays Harbor County

In March 2008 the Registered Counselor Program charged registered counselor applicant Pheobe M. Olivera (RC00059530) with unprofessional conduct. Olivera was convicted of distribution of methamphetamine and admitted guilt to using and possessing methamphetamine. She is also known as Phoebe Marie Muro.

Island County

In February 2008 the Medical Commission placed the license of physician Bradford S. Weeks (MD00030856) on probation for five years. He prescribed Xyrem, a medication that can have adverse and serious effects, to patients without following the procedure to ensure patient safety. He also prescribed medication without examining patients. Weeks must comply with terms and conditions set against his license.

King County

In March 2008 the Board of Pharmacy terminated the probation against the license of pharmacist Richard S. Jacobs (PH00016230).

In March 2008 the Health Care Assistant Program granted the application of health care assistant applicant Julie A. Jewell (HC00151976) and placed her license on probation for two years. She was convicted of harassment. She must comply with terms and conditions set against her license.

In March 2008 the Board of Pharmacy charged pharmacy assistant applicant Moises A. Reyes (VB00068907) with unprofessional conduct. He was convicted of physical control of vehicle under the influence.

In March 2008 the Massage Program charged massage therapist applicant Shi Shi (MASS.MA.60003807) with unprofessional conduct. He was convicted of malicious mischief. He is also known as Corbin Shi and Corben Shi.

In March 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Florante L. Sobreo (NA00196013). She was convicted of attempted theft. Sobreo failed to disclose the conviction on her application or respond to department inquiries regarding the conviction. She cannot reapply for five years.

In March 2008 the Dental Commission amended the statement of charges against Lloyd M. Tucker (DE00007759). Allegations include failing to obtain patient consent, failing to assess and document findings of a patient’s pre-operative CT scan and radiographs, and failing to adequately manage two surgical implants. He also allegedly failed to comply with the department request for patient records.

In March 2008 the Dental Commission charged Ralph K. Zech (DE00005207) with unprofessional conduct. He allegedly left gauze in the surgical site after surgery and failed to submit a written explanation of the event.

Kitsap County

In March 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Audrey M. Daugherty (NA00199786). She was convicted of identity theft and concealed the conviction on her application. She cannot reapply for five years.

In March 2008 the Board of Pharmacy placed the license of pharmacist Renee C. Roadruck (PH00009961) on probation for five years. She incorrectly filled a patient’s prescription dispensing twice the amount prescribed. She took steps to address the error. She must comply with terms and conditions set against her license.

Pierce County

In March 2008 the Nursing Commission terminated probationary terms against licensed practical nurse Shana Finley (LP00047404).

In March 2008 the Nursing Commission charged registered nurse Rose Hoover (RN00151136) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

In March 2008 the Nursing Commission charged licensed practical nurse Teresa J. Miles (LP00045536) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

In March 2008 the Nursing Assistant Program reinstated the credential of nursing assistant Emilio M. Montances (NA00111088).

Skagit County

In March 2008 the Nursing Commission charged registered nurse Teresa M. Eilers (RN00120953) with unprofessional conduct. Allegations include failing to assess and intervene for several hours after a nursing assistant reported a patient in poor condition and failing to ensure the patient’s physician was notified.

Spokane County

In March 2008 the Nursing Assistant Program charged nursing assistant applicant Shameika C. Turner (NA00193036) with unprofessional conduct. She was convicted of criminal trespass, obstructing a law enforcement officer, resisting arrest, and theft on more than one occasion. She allegedly failed to respond to department inquiries regarding her convictions.

Thurston County

In March 2008 the Nursing Assistant Program charged certified nursing assistant Marsha V. Edwards (NC10043137) with unprofessional conduct. She accepted a loan of $1,000 from a patient.

In March 2008 the Nursing Assistant Program charged nursing assistant applicant Lori R. Kenyon (NA00201384) with unprofessional conduct. Her license to practice as a pharmacy technician in Washington was revoked based on diversion of controlled substances for her own use.

Walla Walla County

In March 2008 the Nursing Assistant Program charged nursing assistant applicant Christine M. Rutherford (NA00200406) with unprofessional conduct. She was convicted of theft on two different occasions and allegedly failed to disclose the convictions on her application as required.

Out of State

Montana: In March 2008 the Registered Counselor and Chemical Dependency Professional Programs terminated conditions against the licenses of William J. Lind (RC00031410 and CP00000809).

Oregon: In March 2008 the Board of Pharmacy granted the reactivation application of pharmacist Sokunnarith Saing (PH00039435) and placed his license on probation for two years. He was convicted of two counts of harassment with sexual or intimate touching. He must comply with terms and conditions set against his license.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
