OLYMPIA ľ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In March 2008 the Nursing Assistant Program charged nursing assistant applicant Natasha N. Bob-Bozarth (NA00195127) with unprofessional conduct. She was convicted of possession of methamphetamine.
In March 2008 the Registered Counselor Program charged registered counselor applicant Tamra M. Walker (RC00059263) with unprofessional conduct. She was convicted of two counts of assault and protection order violation – domestic violence.
In March 2008 the Veterinary Board of Governors placed veterinarian Jim Farrish (VT00001397) on stayed suspension. When treating a cat he failed to perform diagnostic testing and performed little examination before diagnosis. Also, during an inspection of the facility, a number of deficiencies were found. He must comply with terms and conditions set against his license.
In April 2008 the Nursing Commission summarily suspended the license of registered nurse Ellen L. Shark (RN00146098). Shark voluntarily surrendered her license to the California Board of Nursing for failing to comply with a previous order.
In March 2008 the Veterinary Board of Governors charged veterinarian Virginia M. Johnson (VT00002875) with unprofessional conduct. She allegedly failed to confine two puppies imported from India, certified the puppies did not have rabies when it had not been confirmed, and allowed one of the puppies to be shipped to another state before state and federal confinement requirements had been met. One of the puppies died of rabies and one was euthanized because it was bit by the puppy with rabies.

She allegedly failed to confine or quarantine two puppies she imported from India, and allowed one of the puppies to be shipped to another state before state and federal confinement requirements had been met. One of the puppies died of rabies and one was euthanized because it was bit by the puppy with rabies.
In March 2008 the Nursing Assistant Program charged nursing assistant applicant Angel Shena Berry (NA00198768) with unprofessional conduct. She allegedly failed to cooperate with a department investigation.
In March 2008 the Nursing Commission charged registered nurse Richard C. Boggs (RN00163168) with unprofessional conduct. He was convicted of assault – domestic violence and interfering with report of domestic violence.
In March 2008 the Registered Counselor Program charged registered counselor applicant Victor C. Conquest (RC00058216) with unprofessional conduct. He was convicted of residential burglary, burglary, and possession of stolen property on more than one occasion.
In March 2008 the Board of Optometry charged Jeffry A. Green (OD00001713) with unprofessional conduct. He allegedly failed to comply with previous terms set against his license.
In March 2008 the Registered Counselor Program denied the application of registered counselor applicant Richard S. Lott (RC00058416). He was convicted of possession of methamphetamine, taking a motor vehicle without permission, and a controlled substance violation. He cannot reapply for three years from his last conviction.
In March 2008 the Nursing Assistant Program charged Nikkii R. Richardson (NA00176737) with unprofessional conduct. She allegedly failed to take care of several patients during her night shift at the facility in which she was employed.
In March 2008 the Massage Program charged massage therapist applicant Chastity R. Herbert (MA00024480) with unprofessional conduct. She was convicted of possession of marijuana – over 40 grams.
In March 2008 the Medical Commission charged physician assistant Elizabeth G. Russell (PA10003518) with unprofessional conduct. She allegedly misused alcohol.
In March 2008 the Chiropractic Commission charged chiropractor Steve C. Baek (CH00034360) with unprofessional conduct. He allegedly practiced beyond his scope of practice by providing electrical stimulation and/or ultrasound to two patients.
In March 2008 the Health Care Assistant Program charged health care assistant applicant Shirelle J. Mercer (HC00155208) with unprofessional conduct. She was convicted of reckless driving and theft.
In February 2008 the Medical Commission withdrew the statement of charges against physician assistant Kerry Scarvie (PA10003805).
In March 2008 the Nursing Assistant Program charged Patricia A. Walton (NA00029223) with unprofessional conduct. She was convicted of unlawful possession of a controlled substance – hydrocodone and possession of a controlled substance – cocaine on more than one occasion. She allegedly failed to respond to the department regarding the convictions.
In March 2008 the Chiropractic Commission charged chiropractor Stephanie A. Greenall (CH00002657) with unprofessional conduct. She allegedly failed to adequately document the care and need for care provided to a patient. She also took x-rays that failed to contain adequate collimation.
In March 2008 the Board of Pharmacy charged pharmacy technician Carmen M. Olson (VA00053926) with unprofessional conduct. She allegedly stole controlled substances from her place of employment.
In March 2008 the Nursing Assistant Program charged certified nursing assistant Mariah O. Allen (NC10030537) with unprofessional conduct. She was convicted of possession of methamphetamine and allegedly failed to respond to the department regarding the conviction.
In March 2008 the Nursing Assistant Program charged nursing assistant applicant Amber R. Rodriguez (NA00200254) with unprofessional conduct. She was convicted of criminal trespass, theft, residential burglary, and assault. She allegedly failed to disclose the convictions on her application and failed to respond to department inquiries regarding the convictions.
In March 2008 the Dental Commission entered into an agreed order with Alfred M. Fairbanks (DE00004076). He failed to obtain consent from patients before extracting their teeth and billed for restorations that were not needed, not performed, or done differently than billed. He must comply with terms and conditions set against his license.
In March 2008 the Nursing Assistant Program charged nursing assistant applicant Erick D. Cox (NA00198856) with unprofessional conduct. He was convicted of theft and allegedly failed to disclose the conviction on his application.
In March 2008 the Surgical Technologist Program charged surgical technologist Basilisa Pacheco (ST00000927) with unprofessional conduct. She allegedly presented a forged prescription to a pharmacy, and left during her shift without telling a shift coordinator or supervisor.
Arizona: In March 2008 the Nursing Assistant Program denied the application of nursing assistant applicant Monique N. Marin (NA00197332). She was convicted of theft and cannot reapply for three years.
Oregon: In April 2008 the Medical Commission summarily suspended the license of Mark Gearhart (MD00019315). Gearhart is prohibited from practicing medicine in Oregon.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
