State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Professions Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Asotin County
In June 2008 the Nursing Assistant Program revoked the credential of certified nursing assistant Jeanie L. Binggeli (NC10074573). Binggeli was convicted of first degree attempted theft, third degree assault, and fourth degree assault.
In June 2008 the Medical Commission indefinitely suspended the license of physician Mahlon Hiestand (MD00039133). Hiestand surrendered his license to practice medicine in Idaho to the Idaho State Board of Medicine for prescribing excessive or inappropriate amounts of controlled substances.
Benton County
In June 2008 the Nursing Commission suspended the license of licensed practical nurse Barbara A. Krogness (LP00051771). Krogness diverted drugs from patients and called a pharmacy representing herself as a physician’s assistant to get a Hydrocodone prescription.
Clallam County
In July 2008 the Nursing Commission indefinitely suspended the license of licensed practical nurse Jacalyn Simpson (LP00035048). Simpson failed to comply with a previous order.
Clark County
In July 2008 the Pharmacy Board revoked the credential of pharmacy technician Mandy Hilker (V00048747). Hilker diverted drugs from her place of employment and her Oregon license was placed on probation for illegally obtaining drugs.
Grays Harbor County
In June 2008 the Nursing Assistant Program indefinitely suspended the credential of Alicia A. Vasconcelles (NC10039518). Vasconcelles failed to comply with previous terms and conditions set against her credential.
King County
In June 2008 the Unlicensed Practice Program amended an order against Barbara Brewitt and Biomed Comm, Inc. She and her company must cease and desist from drug manufacturing, sale of drugs as a shopkeeper, and sale of drugs as a wholesaler, without the required licenses, and must pay a fine.
In July 2008 the Psychology Board indefinitely suspended the license of Philip A. Hirsch (PY00001117). Hirsch failed to comply with a previous order.
In June 2008 the Nursing Commission withdrew charges against licensed practical nurse Melissa M. Neyman (Somerville) (LP00056942).
In July 2008 the Licensed Mental Health Counselor Program dismissed charges against John E. Shephard (LH00006940).
Kitsap County
In June 2008 Radiologic Technologist Program revoked the credential of Theodore R. Alasagas (RT00003674). Alasagas falsified his American Registry of Radiologic Technologists certification and misrepresented himself as being nationally certified.
Lewis County
In June 2008 the Nursing Assistant Program indefinitely suspended the credential of Kathryn M. Coyne (NC10060684). Coyne failed to comply with a previous order.
Okanogan County
In June 2008 the Unlicensed Practice Program ordered Denise S. Taber to immediately cease and desist from engaging in any and all conduct constituting practice of nursing assistant in the state of Washington unless she obtains the appropriate license.
In July 2008 the Nursing Assistant Program dismissed charges against nursing assistant Kayla E. Taber (NA00182960).
In July 2008 the Medical Commission indefinitely suspended the license of physician assistant Elizabeth G. Russell (PA10003518). Russell was involved in an automobile accident and cited for driving under the influence of alcohol.
Snohomish County
In July 2008 the Nursing Commission indefinitely suspended the license of registered nurse Darren M. Slater (RN00122213). Slater surrendered his license to practice as a nurse in Arizona to the Arizona State Board of Nursing for chart discrepancies and drug diversion.
In June 2008 the Massage Program indefinitely suspended the credential of Wesley J. Washington (MA00017061). Washington failed to comply with terms set against his credential.
Spokane County
In July 2008 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Angie A. Fox (NC10059117). Fox abandoned a patient that required constant care.
In July 2008 the Medical Commission placed the license of physician Terrance L. Hauck (MD00038530) on probation for two years. Hauck provided negligent care to a patient. He must comply with terms and conditions set against his license.
In July 2008 Barbara G. Peterson (CP00003669, RC00015250) agreed to surrender her credentials to the Registered Counselor and Chemical Dependency Professional Programs. Peterson cannot practice as a registered counselor or chemical dependency professional. She failed to keep adequate client records.
Thurston County
In June 2008 the Unlicensed Practice Program ordered Dan Bryant to cease and desist from the unlawful practice of dentistry in the state of Washington. He owned Olympic Dental Center from 2001 to 2007 and is not licensed to practice dentistry in the state of Washington.
In May 2008 the Secretary of Health withdrew the statement of charges against nursing assistant Darcy Lockard (NC10041100).
In July 2008 the Nursing Assistant Program suspended the credential of Anthony M. Taylor (NA00171170). Taylor was convicted of third degree assault and domestic violence court order violation.
Walla Walla County
In June 2008 the Secretary of Health withdrew charges against Amber R. Rodriguez (NA00200254).
Whatcom County
In July 2008 the Nursing Commission indefinitely suspended the license of registered nurse Jeanette Anne Hunt (RN00061408). Hunt failed to comply with terms set against her license.
Yakima County
In July 2008 the Registered Counselor Program indefinitely suspended the credential of Brenda G. Duffey (RC00056857). Duffey failed to comply with a previous order.
In July 2008 the Chemical Dependency Professional Program suspended the credential of Karen I. Skager (CP00004937). Skager engaged in a sexual and romantic relationship with a patient.
Out of State
Idaho: In June 2008 the Registered Counselor Program indefinitely suspended the credential of Patricia J. Dill (RC00052370). Dill’s license to practice as a licensed social worker in Idaho was suspended by the Idaho Board of Social Work Examiners.
Oregon: In July 2008 the Medical Commission indefinitely suspended the license of Thomas Lee (MD00043402). Lee’s license to practice medicine in Oregon was indefinitely suspended based on information that he was arrested, booked, and indicted on multiple criminal felony charges of unlawful sexual penetration and sexual abuse in regard to two children.
