OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In August 2008 the Nursing Assistant Program indefinitely suspended the credential of Sabrie L. Tramble (NA00197331). Tramble failed to comply with terms set against her credential.
In August 2008 the Medical Commission modified a prior restriction on the license of physician Patrick K. Chau (MD00030053). Chau must prescribe the lowest dose that’s medically effective when prescribing stimulants, benzodiazepines, and thyroid medication.
In August 2008 the Chemical Dependency Professional Program suspended the credential of Laurence L. Logan (CP00000094) for at least 7 years. Logan was convicted of two counts of communication with a minor for immoral purposes.
In August 2008 the Nursing Commission summarily suspended the license of registered nurse Joyce E. Pivetti (RN00160002). Pivetti surrendered her license to practice as registered nurse in Oregon to the Oregon State Board of Nursing following a first degree theft conviction.
In August 2008 the Psychology Board set practice limitations on the license of Dennis R. Allen (PY00002454). Allen cannot accept any parenting evaluation appointments until he completes training and supervision requirements.
In August 2008 the Osteopathic Medicine and Surgery Board restricted the license of Dale E. Alsager (OP00001485). Alsager provided care that fell below the standard to several patients. He cannot prescribe Schedule II and Schedule III controlled substances until he completes additional training.
In August 2008 the Nursing Assistant Program denied the application of Josif Cocos (NC10045567). Cocos was convicted of reckless driving, disorderly conduct, fourth degree assault, hit and run attended, and aiming or discharging firearms. He cannot reapply for 7 years.
In August 2008 the Nursing Assistant Program suspended the credential to Elizabeth M. Gichuki (NC10084369) for at least 3 years. Gichuki left a developmentally disabled adult patient alone in a vehicle. The patient got out and wandered into strangers’ apartments. Gichuki was also convicted of reckless endangerment.
In August 2008 the Registered Counselor Program suspended the credential of Wayne G. McDade, Jr. (RC00035698). McDade was convicted of first degree criminal trespass and second degree criminal impersonation and did not report the convictions to the department.
In August 2008 the Nursing Assistant Program indefinitely suspended the credential of Stella M. Murphy (NA00125545). Murphy was convicted of unlawful use of drug paraphernalia and lied to the department during investigation about her arrest and conviction.
In August 2008 the Chiropractic Commission revoked the license of Mark J. Svetcos (CH00034350). Svetcos engaged in sexual contact with several patients.
In September 2008 the Nursing Assistant Program permanently revoked the license of Joseph Wakarugi Thurura (NA00188252). Thurura had sexual contact with a developmentally disabled, blind resident of the long term care facility where he worked, and impregnated her. He has been criminally charged with second degree rape.
In August 2008 the Nursing Assistant Program indefinitely suspended the credential of Rheycel S. Orante (NA00184273). Orante failed to comply with a previous order.
In August 2008 the Nursing Assistant Program indefinitely suspended the credential of Linda F. Washington (NA00019334). Washington was convicted of telephone harassment - domestic violence.
In August 2008 the Registered Counselor Program withdrew the statement of charges against Clarence F. Farmer (RC00050383, CA00005381).
In August 2008 the Nursing Assistant Program permanently revoked the credential of Angela M. Ferguson (NC10047286). Ferguson was convicted of first degree murder.
In August 2008 the Nursing Assistant Program suspended the credential of Kaliana Renkel (NC10086282). Renkel tested positive for methamphetamine and marijuana during a random urine test at her place of employment. She also failed to respond to department inquiries about the incident.
In August 2008 the Denturist Board indefinitely suspended the license of Kevin C. Kowalski (DN00000254). Kowalski provided a substandard set of dentures to a patient.
In August 2008 the Unlicensed Practice Program ordered Min Jeong Kim to permanently cease and desist from engaging any and all conduct constituting the practice of unlicensed massage in the state of Washington.
In August 2008 the Osteopathic Board suspended the license of osteopathic physician assistant Steven M. Featherkile (OA10000199). Featherkile failed to examine and treat a diabetic patient’s feet. He also practiced outside his scope of practice by prescribing controlled substances.
In August 2008 the Medical Commission revoked the credential of Jitesh Chawla (MD00044182). The commission found that Chawla engaged in sexual misconduct with several patients and provided negligent care to a patient.
In August 2008 the Nursing Commission withdrew the statement of charges against Kenneth D. Saucke (LP00027619).
In August 2008 the Nursing Commission suspended the license of Stefanie R. Ambuehl (LP00058765). Her nursing assistant credential was revoked after she took personal bank checks from a patient, wrote four checks to herself, forged the patient’s signature, and cashed the checks totaling more than $2700. She is also known as Stepfanie Millspaugh.
In August 2008 the Registered Counselor Program suspended the credential of Frank G. Laliberte (RC00053944). Laliberte failed a workplace urinalysis; he tested positive for barbiturates, phenobarbital, benzodiazepines, and oxazepam.
In August 2008 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Cammy D. Briggs (NC10022359). Briggs failed to comply with terms set against her credential.
Arizona: In August 2008 the Chiropractic Commission suspended the credential of Matthew J. Reilly (CH00034693) for at least 5 years. Reilly engaged in sexual misconduct with a patient.
Florida: In July 2008 the Registered Counselor, Chemical Dependency Professional, and the Massage Programs modified an order against Jeffrey C. Padgett (RC00033640, CP00003648, MA000171720). His credentials will remain suspended for at least two years.
Montana: In August 2008 the Medical Commission summarily suspended the license of physician Paul W. Anderson (MD00033126). The commission based the suspension on the fact that Anderson surrendered his license to practice medicine in Nebraska.
Oregon: In August 2008 the Nursing Commission summarily suspended the license of Laura E. Cassily (RN00143704). Cassily’s license to practice as registered nurse in Oregon was suspended by the Oregon Board of Nursing for driving under the influence and reckless burning.
