OLYMPIA ľ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In August 2008 the Nursing Assistant Program denied the reactivation application of Rex W. Miller (NC10054779). He was convicted of reckless endangerment, driving under the influence and theft.
In September 2008 the Nursing Assistant Program denied the application of Mandi K. Murdoch (NA00170027). She was convicted of forgery and unlawful issuance of bank checks or drafts. She also failed to respond to department inquiries regarding the convictions. Murdoch cannot reapply for three years.
In August 2008 the Board of Pharmacy denied the application of pharmacy assistant applicant Allison M. Kuning (VB00067481). She was arrested for possession of a controlled substance – marijuana and use of drug paraphernalia. Kuning failed to respond to department inquiries regarding the arrest.
In August 2008 the Nursing Commission denied the application of licensed practical nurse applicant Travis M. Edmonds (LP00059015). His application to practice as a licensed practical nurse in Illinois was denied. He failed to disclose the denial on his application.
In August 2008 the Nursing Assistant Program denied the application of Byron E. McLemore (NA00192637). He was convicted of indecent liberties.
In September 2008 the Medical Commission amended the order against physician Behrouz Pirouzkar (MD00037233). He performed repeated tests and provided treatment to patients that were not necessary and not justified by the patients’ conditions and billed the patients’ insurance for the unnecessary tests and treatment. He must comply with terms and conditions set against his license.
In September 2008 the Massage Program granted the application of Patricia Rae Richerson (MA60016586) and placed her license on probation for 18 months. She was convicted of theft. Richerson must comply with terms and conditions set against her license.
In August 2008 the Nursing Assistant Program denied the application of Jennifer D. Keith (NA60001883). She was convicted of domestic violence – assault and failed to disclose the conviction on her application.
In August 2008 the Nursing Assistant Program denied the application of Krista L. Lessenden (NA60003347). She was convicted of defraud by check and unlawful issuance of bank checks. She also failed to disclose the convictions on her application or respond to department inquiries regarding the convictions.
In August 2008 the Registered Counselor Program denied the application of Terry L. Harrison (RC00060359). Harrison was convicted of assault.
In September 2008 the Nursing Commission granted the application of registered nurse applicant Alexis R. Johnson (RN60018076) and placed conditions on her license. She must comply with terms and conditions set against her license.
In August 2008 the Nursing Assistant Program denied the application of Maire E. Keller (NA60018429). Her license to practice as a pharmacy assistant was revoked. Keller failed to disclose the revocation on her application. She is also known as Mary Melious.
In August 2008 the Nursing Assistant Program denied the reactivation application of Sione Kupu (NC10086988). Kupu was convicted of possession of heroin and failed to respond to department inquiries regarding the conviction.
In August 2008 the Nursing Assistant Program denied the application of Kimberly S. Murphy (NA60012287). She was convicted of possession of a controlled substance – cocaine and possession of a controlled substance – hydrocodone.
In August 2008 the Nursing Assistant Program denied the application of Eugene J. Robinson (NA60002697). He was convicted of burglary, residential burglary and three counts of robbery. He failed to disclose the convictions on his application.
In August 2008 the Nursing Assistant Program denied the application of Anna M. Isquirisigza (NA00161478). She was convicted of domestic violence – reckless endangerment and attempted theft.
In September 2008 the Registered Counselor Program granted the application of Kerry G. Baziotis (RC60019341) and placed his registration on probation for two years. He was convicted of robbery on two different occasions. He must comply with terms and conditions set against his license.
In September 2008 the Nursing Assistant Program charged Cathy J. Chapman (NA00201382) with unprofessional conduct. She was convicted of assault – domestic violence and malicious mischief - domestic violence.
In August 2008 the Nursing Assistant Program denied the application of Robin E. Fellows (NA60002546). She was convicted of theft and failed to disclose the conviction on her application. She also failed to respond to department inquiries regarding the conviction.
In August 2008 the Nursing Assistant Program denied the application of Timothy J. Martinez (NA00202075). He was convicted of harassment – domestic violence and failed to disclose the conviction on his application.
In August 2008 the Nursing Assistant Program denied the application of Dara L. Peters (NA60002011). In lieu of disciplinary action, she surrendered her license to practice as a certified nursing assistant in Nevada. She failed to disclose the surrender on her application.
In August 2008 the Nursing Assistant Program denied the application of Kelly R. Warner (NA00198738). She was convicted of domestic violence – harassment and failed to disclose the conviction on her application.
In August 2008 the Nursing Assistant Program denied the application of Jessica E. Belieu (NA60015214). She was convicted of theft and trafficking in stolen property and failed to disclose the convictions on her application. She also failed to respond to department inquiries regarding the conviction.
In August 2008 the Nursing Assistant Program denied the application of Julie S. Bueno (NA60012905). She was convicted of false statement and failed to disclose the conviction on her application.
In August 2008 the Nursing Assistant Program denied the reactivation application of Chrystal Cheri Raigoza (NA00149400). She was convicted of harassment of another and malicious mischief.
California: In August 2008 the Nursing Commission denied the reactivation application of registered nurse John G. Montez (RN00147217). The California Board of Registered Nursing revoked his license to practice as a registered nurse in California.
Colorado: In August 2008 the Nursing Commission denied the application of registered nurse applicant Kimberly Marie Cripe (RN00171271). She failed to provide the department with requested information.
Georgia: In August 2008 the Nursing Commission denied the application of registered nurse applicant Joseph Dill (RN00176580). He failed to disclose he was a registered nurse in Florida under the name of Corey Dill and his license to practice as a registered nurse in Florida was suspended. Dill failed to respond to department inquiries regarding the suspension.
Mississippi: In August 2008 the Nursing Commission denied the application of registered nurse applicant Jacqualine S. Harris (RN00117868). She surrendered her license to practice as a registered nurse in Mississippi and the Mississippi Nursing Board denied reinstatement.
