OLYMPIA ľ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In September 2008 the Health Care Assistant Program denied the application of Dorinda M. Soderman (HC00154059). She was convicted of driving under the influence and assault.
In September 2008 the Nursing Assistant Program denied the application of Haley D. Windsheimer (NA60019022). She was charged with theft and possession of stolen property.
In October 2008 the Board of Pharmacy charged pharmacy assistant Judy Melgoza (VB00070306) with unprofessional conduct. She allegedly diverted drugs for her own use and stole more than $4,000 worth of drugs and merchandise from her place of employment.
In September 2008 the Massage Therapy Program denied the application of Dmitri V. Vilenski (MA00024567). He was convicted of malicious mischief on more than one occasion, burglary on more than one occasion, criminal trespass, and vehicle prowling.
In October 2008 the Chemical Dependency Professional Program granted the application of Krista R. Heppe (CP60002760) and placed restrictions and conditions on her chemical dependency professional license. In May 2006 restrictions were placed on her license to practice as a registered counselor.
In October 2008 the Nursing Commission charged registered nurse Christine D. Banman (RN00139211) with unprofessional conduct. Banman allegedly failed to comply with previous terms and conditions set against her license. She is also known as Christine D. Hartsock.
In September 2008 the Surgical Technologist Program denied the application of Niquara L. Beeks (ST00003610). She was convicted of harassment and two counts of possession of stolen property.
In October 2008 the Nursing Commission charged licensed practical nurse Miriam V. Bernales (LP00048674) with unprofessional conduct. She allegedly failed to notify the physician of changes in a patient’s condition.
In October 2008 the Registered Counselor Program granted the application of Meagan M. Bowser (RC60001991) and placed her license on probation for two years. She was convicted of unlawful possession of a controlled substance and forgery. She must comply with terms and conditions set against her license.
In October 2008 the Nursing Assistant Program granted the application of Pepper Rose Clark (NC10101158) and placed her certification on probation for two years. She was convicted of theft, minor in possession of alcohol, and obstructing a law enforcement officer. She must comply with terms and conditions set against her license.
In October 2008 the Registered Counselor Program charged Alexander J. Chun (RC00058835) with unprofessional conduct. A patient’s chart required she be visually checked every five minutes for her safety. He allegedly logged and initialed he had visually checked on the patient when the patient had committed suicide before his shift began.
In September 2008 the Nursing Assistant Program denied the application of Keisha M. Clemons (NC10063596). She was convicted of theft and failed to respond to department inquiries regarding the conviction.
In October 2008 the Acupuncture Program reinstated the license of JianJun Jiang (AC00000293).
In October 2008 the Health Care Assistant Program charged Farley Milan (HC00144341) with unprofessional conduct. He was convicted of assault with sexual motivation.
In October 2008 the Nursing Assistant Program reinstated the registration of Tracy May Brady (NA00076751).
In September 2008 the Nursing Assistant Program denied the reactivation application of Jennifer Nogales (NA00114322). She was convicted of assault – domestic violence and criminal assault.
In October 2008 the Registered Counselor Program charged Elizabeth M. Oreskovich (RC00013767) with unprofessional conduct. She allegedly rented business and residential space to patients, shared a business space with a patient, and bartered services with a patient.
In September 2008 the Nursing Assistant Program denied the application of Edith De laCerda (NA60017999). She has pending criminal charges for conspiracy to commit assault – deadly weapon and perjury.
In October 2008 the Registered Counselor Program charged Jeff P. Fosdick (RC00057286) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his license.
In October 2008 the Registered Counselor Program granted the application of Michael A. Arcondizzi (RC00060446) and placed his registration on probation for three years. He was convicted of two counts of assault. He must comply with terms and conditions set against his license.
In September 2008 the Nursing Assistant Program denied the applications of Charlotte A. Beavers (NA60009348, NC60014793). She was convicted of possession of a controlled substance.
In October 2008 the Nursing Assistant Program granted the application of Andrew T. Davie (NC10101755) and placed restrictions on his certification. His license to practice as a licensed practical nurse in Idaho was revoked. He must comply with terms and conditions set against his license.
In October 2008 the Nursing Commission terminated the probation previously imposed against the license of registered nurse Reginald M. Kaylor (RN00111025).
In October 2008 the Nursing Assistant Program reprimanded and imposed conditions on Stephanie C. Spicer (NA00179647). She failed to respond to investigators about an allegation that she took controlled substances from her place of employment. She must comply with terms and conditions set against her license.
In September 2008 the Nursing Assistant Program denied the application of David V. Herrera (NA00144444). He was convicted of unlawful possession of a controlled substance – marijuana with intent to deliver and unlawful possession of a controlled substance – hydrocodone.
In October 2008 the Hypnotherapy Program granted the application of Thomas J. Ward (HP10001650) and placed his license on probation for six months. Restrictions were placed on his license to practice as a nursing home administrator and he failed to disclose the restrictions on his hypnotherapy application. He must comply with terms and conditions set against his license.
In October 2008 the Nursing Assistant Program charged Brian J. Chaput (NC10085101) with unprofessional conduct. He allegedly used heroin.
In September 2008 the Nursing Assistant Program denied the application of Vladimir U. Dubrosvskiy (NA60015821). He was convicted of shoplifting on more than one occasion and failed to disclose the convictions on his application.
In October 2008 the Nursing Assistant Program charged Marjory L. Krueger (NC10096584) with unprofessional conduct. She allegedly exposed herself to a patient.
Florida: In October 2008 the Chiropractic Commission granted the application of chiropractor applicant Maurice Daoud (CH60024972) and placed his license on probation for five years. He was convicted of health care fraud and restrictions were placed against his license to practice in Florida. He must comply with terms and conditions set against his license.
Oregon: In October 2008 the Medical Commission charged physician assistant Theoul Thuch (PA10001757) with unprofessional conduct. While working as a counselor, he allegedly intentionally and falsely charted and billed for five 60-minute home visits with a patient that did not occur.
