OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In September 2008 the Nursing Assistant Program denied the application of Brian S. Radford (NA60018934). He was convicted of theft. He failed to disclose the conviction on his application and failed to respond to department inquiries regarding the conviction.
In September 2008 the Registered Counselor Program charged Raeann M. VanTrojen (RC00054887) with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her license. She is also known as Raeann M. Bishop.
In June 2008 the Board of Osteopathic Medicine and Surgery charged Jack J. Calabria (OP00000789) with unprofessional conduct. Allegations include failing to keep adequate records of both medical history and prescriptions given to a patient, failing to refer the patient to a pain specialist for her symptoms, and failing to reflect reasons for giving the patient increased doses of opioid pain medications.
In September 2008 the Licensed Marriage and Family Therapist Program placed the license of Brian Desroches (LF00001459) on probation for three years. He identified himself as a psychologist and provided patient information to a journalist without the patient’s consent. He must comply with terms and conditions set against his license.
In September 2008 the Registered Counselor Program granted the application of registered counselor Chanel Renee Flemings (RC60001396) and placed her registration on probation for three years. She was convicted of conspiracy to deliver cocaine and possession of cocaine. She must comply with terms and conditions set against her license.
In September 2008 the Examining Board of Psychology charged Kim E. Kendall (PY00001379) with unprofessional conduct. Allegations include failing to inform the father of the patient about treatment recommendations and failing to disclose records upon the father’s request.
In September 2008 the Registered Counselor Program granted the application of David R. Patten (RC60028189) but required him to comply with terms and conditions. He was convicted of two counts of residential burglary, assault – domestic violence and attempted forgery.
In September 2008 the Board of Pharmacy denied the application of pharmacy assistant Laurie A. Sillence (VB00071693). She was convicted of custodial interference and failed to disclose the conviction on her application.
In September 2008 the Veterinary Board of Governors denied the application of animal technician Jessica K. Strickland (AT60002808). She was convicted of trafficking in stolen property.
In October 2008 the Board of Pharmacy charged pharmacy technician Terri Woods (VA00019889) with unprofessional conduct. She allegedly forged prescriptions and diverted drugs from her place of employment.
In September 2008 the Registered Counselor and Chemical Dependency Professional Programs placed the credentials of Lee H. Young (RC00031018, CP00001474) on probation for three years. He crossed professional boundaries by posting bail for a patient and visiting the patient at her home. He also presented himself as an addiction psychologist. He is not a licensed psychologist in Washington.
In September 2008 the Nursing Commission charged registered nurse Robin Karol Rough (RN00099645) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In September 2008 the Massage Therapist Program granted the application of Christine R. Cruse (MA60012594) and placed her license on probation for two years. Cruse was convicted of assault. She must comply with terms and conditions set against her license.
In September 2008 the Nursing Assistant Program granted the application of Bobbi R. Calley (NA00196056) and placed her registration on probation for one year. She was convicted of destruction of property, soliciting prostitution, criminal trespass, and obstructing a public servant. She failed to disclose the convictions on her application. Calley must comply with terms and conditions set against her registration.
In September 2008 the Board of Pharmacy charged pharmacist Debbie A. Eastman (PH00019936) with unprofessional conduct. She allegedly filled and dispensed prescriptions for a family member when the prescriptions had not been paid for. She also documented un-purchased prescriptions for herself to fool the insurance company into believing she paid her deductible.
In September 2008 the Registered Counselor Program granted the application of Randall S. Johnston (RC00059982) but required him to comply with terms and conditions. He was convicted of unlawful possession of a controlled substance – methamphetamine and burglary.
In October 2008 the Nursing Commission charged licensed practical nurse Roberta M. Manford (LP00013636) with unprofessional conduct. She allegedly made numerous expenditures from a patient’s checking account for her own benefit.
In September 2008 the Nursing Commission charged licensed practical nurse Robin K. Tate (LP00039802) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In September 2008 the Naturopathy Program charged Elizabeth McDonald (NT00000659) with unprofessional conduct. She allegedly failed to follow up with further evaluation or referral for a patient when her Pap smear and ultrasound on her uterus came back abnormal. After a second ultrasound, she notified the patient it was normal when it was documented as abnormal. The patient later died from endometrial cancer.
In September 2008 the Massage Therapist Program placed the license of Kimberly L. Robinson-Searles (MA00017055) on probation for three years. She entered into a sexual relationship with a client. She must comply with terms and conditions set against her license.
In September 2008 the Nursing Assistant Program charged Shannon L. Cummins (NC10080504) with unprofessional conduct. She allegedly stole $80 from a patient and paid the patient back when confronted about the theft.
In September 2008 the Nursing Assistant Program reinstated the registration of Scott Davis (NA00185766).
In September 2008 the Nursing Assistant Program granted the application of Stephanie J. McGinnis (NA00151166) and placed her registration on probation for 18 months. McGinnis was convicted of theft. She must comply with terms and conditions set against her license.
In September 2008 the Nursing Assistant Program charged Dedra Marie Miller (NA00004543) with unprofessional conduct. She allegedly yelled at and insulted a patient, made disparaging remarks about her, and discussed a patient’s medical condition in front of other patients without consent.
In September 2008 the Nursing Assistant Program denied the application of Lindsey D. Brehm (NA60018651). She was convicted of possession of stolen property and malicious mischief. She failed to disclose the convictions on her application and failed to respond to department inquiries regarding the convictions.
In August 2008 the Nursing Commission denied the application of licensed practical nurse Teresa A. Lester-Salsman (LP60012278). She was convicted of five counts of theft.
In October 2008 the Nursing Commission charged registered nurse Debra J. Good (RN00158527) with unprofessional conduct. She was found guilty of driving under the influence.
In October 2008 the Board of Pharmacy charged pharmacist Donald Fortner (PH00013635) with unprofessional conduct. He allegedly diverted drugs from his place of employment and failed a workplace drug test.
Texas: In September 2008 the Health Care Assistant Program reinstated the license of Misty M. Riser (HC00135795).
