State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Chelan County

In September 2008 the Nursing Commission reinstated the license of registered nurse Cheryl R. Jedlow (RN00076917).

Clark County

In September 2008 the Nursing Assistant Program denied the application of Lourin A. Holt (NA00201504). She has pending criminal charges for theft and bail jumping. She also failed to respond to department inquiries regarding the charges.

Cowlitz County

In October 2008 the Registered Counselor Program charged Shannon R. Shinn (RC00051935) with unprofessional conduct. He allegedly failed to comply with previous terms and conditions set against his registration.

Island County

In October 2008 the Medical Commission entered into an agreed order with physician Robert L. Lycksell (MD00020054) and placed his license on probation. He was convicted of the crime of reckless endangerment. He must comply with terms and conditions set against his license.

Jefferson County

In October 2008 the Board of Optometry entered into an agreed order with Terrence T. Clark (OD00001204). He provided treatment outside his scope of practice. He must comply with terms and conditions set against his license.

King County

In September 2008 the Nursing Assistant Program denied the application of Mary Jane E. Bigayan (NA60011360). She was convicted of assault – domestic violence, telephone harassment, and violation of a no contact order.

In October 2008 the Nursing Assistant and Health Care Assistant Programs charged Caldera W. Fries (NC10047803, HC00141666, HC00150777, HC00152116) with unprofessional conduct. She allegedly signed the names of two doctors on authorizations for medical release without the physicians’ permission or knowledge.

In October 2008 the Medical Commission entered into an agreed order with physician Howard G. Maron (MD00016708). He prescribed a growth hormone to a patient when the patient was within the normal growth range, failed to document the prescription, and failed to document a discussion or informed consent regarding the growth hormone treatments. He must comply with terms and conditions set against his license.

In September 2008 the Health Care Assistant Program denied the application of Farley S. Milan (HC00154434). He was convicted of assault with sexual motivation.

In October 2008 the Board of Pharmacy charged pharmacy assistant Aleksandr Milman (VB00063032) and his pharmacies A-Z Pharmacy in Kent, A-Z Pharmacy in Bellevue, and Lakeshore Pharmacy (CF00058237, CF00058238, CF00058547) with unprofessional conduct. Milman allegedly forged prescriptions to be reimbursed by the drug manufacturers, used stolen coupons, and created false records so he could bill Medicaid for prescriptions never delivered to patients. Milman was convicted of conspiracy to commit health care fraud and structuring currency transaction.

In October 2008 the Board of Pharmacy charged pharmacist Valentina Milman (PH00020193) with unprofessional conduct. She was convicted of conspiracy to commit health care fraud and conspiracy to launder money.

In October 2008 the Chiropractic Commission entered into an agreed order with chiropractor Greg Pfiffner (CH00002939). He billed under his provider number when the treatment was given by another chiropractor and used erroneous billing codes for treatments provided to patients. He will be monitored for at least three years and must comply with terms and conditions set against his license.

In October 2008 the Chiropractic Commission granted the application of chiropractic x-ray technician Fernanda E. Velderrain-Loreto (CX60012160) and placed her license on probation for two years. She was convicted of malicious mischief –domestic violence, assault, and four counts of bank embezzlement. She must comply with terms and conditions set against her license.

Kitsap County

In September 2008 the Nursing Assistant Program denied the application of Carlye R. Nelson (NA60019067). She was convicted of malicious mischief – domestic violence, assault, minor in possession of alcohol, and possession of marijuana. She failed to disclose the convictions on her application.

Mason County

In October 2008 the Nursing Assistant Program charged Michael Abrams (NC10100369) with unprofessional conduct. He allegedly failed to respond to department inquiries regarding a complaint filed against him.

In October 2008 the Office of Emergency Medical Services and Trauma System charged paramedic Barbara J. Bodin (ES00102715) with unprofessional conduct. She allegedly worked as a paramedic on an expired license for three years.

Okanogan County

In October 2008 the Nursing Assistant Program granted the application of Chelcy J. Bucholtz (NA60012028) and placed her registration on probation for two years. She was convicted of theft. Bucholtz must comply with terms and conditions set against her registration.

In October 2008 the Nursing Assistant Program charged Nucobey Nelson (NC10101202) with unprofessional conduct. He allegedly administered medications to a patient when he didn’t have nurse delegation to do so, and failed to respond to department inquiries regarding the allegations.

Pacific County

In October 2008 the Nursing Assistant Program charged Casey J. Riggs (NC10080169) with unprofessional conduct. She allegedly stole two money orders from a resident with a total value of $300 and entered into a diversion agreement for theft.

Pierce County

In October 2008 the Nursing Assistant Program granted the application of LaReina C. Larson (NA00200643) and placed her registration on probation for four years. She was convicted of driving while under the influence. She entered into a deferred disposition for obtaining or attempt to obtain a controlled substance by forgery or alteration of a prescription or any written order. She must comply with terms and conditions set against her registration.

In October 2008 the Nursing Assistant Program charged Cheri S. Toomey (NA00183740) with unprofessional conduct. She allegedly attempted to take household items from her place of employment without permission.

Skagit County

In September 2008 the Nursing Assistant Program denied the reinstatement application of Alison E. Lowe (NC10059957). Her certification to practice as a nursing assistant was revoked in 2002 for engaging in misconduct. Since then, she was convicted of reckless driving and theft on more than one occasion.

Snohomish County

In October 2008 the Board of Pharmacy charged pharmacy assistant Jennifer Funk (VB00067425) with unprofessional conduct. She allegedly stole several bottles of OxyContin from her place of employment. She entered into a pre-prosecution diversion agreement in criminal court for the thefts.

In October 2008 the Nursing Assistant Program charged Lorella A. Washington (NC1002523) with unprofessional conduct. She allegedly slapped a patient on the face.

Spokane County

In October 2008 the Registered Counselor Program granted the application of Kimberly R. Puntney (RC60020450) and imposed terms and conditions for at least three years. She was convicted of assault – domestic violence and possession of a controlled substance with intent to deliver – methamphetamine.

Whitman County

In October 2008 the Massage Therapy Program granted the application of Gail Wayne Tackett (MA60027133) and placed his license on probation for one year. He was convicted of theft. He must comply with terms and conditions set against his license.

Out of State

Idaho: In September 2008 the Nursing Commission reinstated the license of registered nurse Lorri L. Tretter (RN00167540). She is also known as Lorri L. Champion.

Nebraska: In October 2008 the Medical Commission entered into an agreed order with physician Ziad Zawaideh (MD00023457). His license was restricted in Nebraska. He must comply with terms and conditions set against his license.
