State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In October 2008 the Nursing Commission placed the license of licensed practical nurse Karen L. Andrews (LP00048993) on probation for six months. She failed to change a patient’s dressings despite the order to do so daily, charted she had changed the patient’s dressing when she had not, and signed she had done a skin assessment on the patient when she had not. She must comply with terms and conditions set against her license.

Clark County

In October 2008 the Veterinary Board of Governors denied the application of veterinary technician Michelle M. Lacey (AT60014514). She was convicted of conspiracy to commit manufacture of a controlled substance – marijuana.

In October 2008 the Nursing Commission granted the application of registered nurse Glenn E. Yauney (RN00175915) and placed terms and conditions against his license. He must comply with terms and conditions set against his license.

Grant County

In October 2008 the Medical Commission charged physician Irene K. Kimura (MD00032544) with unprofessional conduct. She allegedly provided negligent treatment to a patient.

Grays Harbor County

In October 2008 the Board of Pharmacy charged pharmacist Terry R. James (PH00010124) with unprofessional conduct. He allegedly billed an insurance company for prescriptions that were never purchased or picked up, and re-filled prescriptions for himself that stated no refills.

In September 2008 the Nursing Assistant Program denied the application of Tammy A. Ontiveros (NA00201239). She was convicted of violating the uniform controlled substances act – possession of methamphetamine.

Island County

In October 2008 the Board of Pharmacy charged pharmacy technician Kathleen D. Newman (VA00051808) with unprofessional conduct. She allegedly tested positive for cocaine during a workplace urinalysis and failed to respond to department inquiries regarding the test.

King County

In October 2008 the Board of Pharmacy charged pharmacy technician Christopher M. Day (VA00047800) with unprofessional conduct. He allegedly diverted controlled substances from his place of employment for delivery or sale to others, and was convicted of acquiring a controlled substance by forgery and subterfuge.

In October 2008 the Massage Therapy Program granted the application of Angel D. Lindblad (MA60021997) and placed her license on probation until November 2009. She was convicted of dealing in counterfeit obligations or securities. She must comply with terms and conditions set against her license.

In October 2008 the Medical Commission terminated the agreed order against physician Sjardo S. L. Steneker (MD00032063). Dr. Steneker’s license is no longer restricted.

In October 2008 the Dental Commission reinstated the license of dentist Joseph Thurston (DE00004460) and placed restrictions against his license. He must comply with terms, conditions, and restrictions set against his license.

Mason County

In September 2008 the Registered Counselor Program denied the application of Stephen E. Mace (RC60001623). He was convicted of theft, burglary, and assault.

Pierce County

In October 2008 the Registered Counselor Program granted the application of Vickie L. Maggitt (RC60026767) and placed terms, conditions, and restrictions against her registration. She was convicted of assault and theft.

In October 2008 the Nursing Commission entered into an agreed order with registered nurse Ederico B. Mariano (RN00156875). He dispensed but failed to document administration of medication, failed to document wastage of medication, and dispensed a medication more frequently than the physician’s order for a number of patients. He must comply with terms and conditions against his license.

In October 2008 the Nursing Commission charged licensed practical nurse Denise Martin (LP00046339) with unprofessional conduct. She tested positive for a controlled substance.

In October 2008 the Massage Therapy Program reinstated the license of Leslie J. Wintermute (MA00019033). Her license was suspended in September for failure to comply with a child support order.

Skagit County

In September 2008 the Health Care Assistant Program denied the application of Alison E. Lowe (HC00155876). She was convicted of reckless driving and theft on two occasions. Her license to practice as a nursing assistant in Washington was revoked.

Spokane County

In October 2008 the Nursing Assistant Program granted the application of Heather L. Connor (NC60016631) and placed restrictions against her certification. Terms and conditions were set against her registration to practice as a nursing assistant.

In September 2008 the Health Care Assistant Program denied the application of Brent E. Countryman (HC00155448). He administered flu shots without supervision and without a valid credential to administer the shots.

In October 2008 the Board of Pharmacy charged the pharmacy firm Northwest Health Systems, Inc (CF00058494) with unprofessional conduct. The firm allegedly continued to store, repackage, and distribute samples to patients without the board’s approval of required documentation.

Stevens County

In October 2008 the Nursing Assistant Program denied the application of Pillarr Harmon (NA00193017). She was convicted of theft and failed to appear at her pre-hearing conference. She can’t reapply until 2011.

Whatcom County

In October 2008 the Nursing Commission charged registered nurse Michelle Frank (RN00120509) with unprofessional conduct. She allegedly pre-poured and pre-charted administering medications to patients.

Yakima County

In September 2008 the Registered Counselor Program denied the application of Cammy Dawn Briggs (RC00060503). Her certification to practice as a nursing assistant was indefinitely suspended.

In October 2008 the Veterinary Board of Governors denied the application of veterinary technician David M. Marsh (AT00002296). He has a deferred prosecution for driving under the influence, and hit and run unattended.

Out of State

New York: In September 2008 the Medical Commission denied the application of physician Vincent H. Decaprio (MD60050467). He conducts online consultations and prescription monitoring through his current employer. He prescribes medication to patients via the Internet without seeing or examining the patients.

Ohio: In October 2008 the Medical Commission reinstated the license of physician Ann V. Govier (MD00018662). Dr. Govier’s license to practice is now unrestricted.

Oregon: In October 2008 the Dental Commission reinstated the license of dentist Jamshid Faghih (DE00008602).

Oregon: In October 2008 the Office of Emergency Medical Services and Trauma System denied the application of paramedic Joshua A. Keyes (ES60010872). His license to practice in Oregon is on probation for use of controlled substances.

Texas: In October 2008 the Medical Commission charged physician David L. Burger (MD00039295) with unprofessional conduct. He entered into an agreed order with the Texas Medical Board restricting his license to practice medicine in Texas.
