OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on
the agency Web site. Click on “Provider Credential Search” on the left hand side
of the
Department of Health home page (www.doh.wa.gov). The site includes
information about a health care provider’s license status, the expiration and
renewal date of their credential, disciplinary actions and copies of legal
documents issued after July 1998. This information is also available by calling
360-236-4700. Consumers who think a health care provider acted unprofessionally
are also encouraged to call and report their complaint.

In October 2008 the Nursing Assistant Program denied the application of Isabel A. Montes (NC10075206). She was convicted of minor in possession by consumption and unlawful possession of a controlled substance – cocaine.
In November 2008 the Medical Commission amended the statement of charges against physician Kenneth M. Jones (MD00028268). He allegedly aided and abetted the unlicensed practice of medicine by allowing unlicensed persons to evaluate clients, provide injections, use his name to obtain and use equipment, and use his DEA number to obtain medications. Jones also mis-prescribed human growth hormone to two patients.
In October 2008 the Registered Counselor Program granted the application of Nona G. Deanda (RC00059512) and placed her registration on probation for five years. She was convicted of forgery and theft on more than one occasion. She must comply with terms and conditions set against her license.
In November 2008 the Nursing Commission charged licensed practical nurse Russell G. Tucker (LP00041862) with unprofessional conduct. He was convicted of indecent liberties and assault – domestic violence on more than one occasion.
In October 2008 the Board of Pharmacy placed the license of pharmacy intern Kenyon N. Lyle, Jr. (IR00066943) on probation for five years. He was convicted of driving under the influence and hit and run attended. He must comply with terms and conditions set against his license.
In October 2008 the Nursing Assistant Program denied the application of Marissa L. Morello (NA60002024). She was convicted of possession of stolen property, assault, and assault – domestic violence. She can’t reapply for five years.
In October 2008 the Board of Pharmacy reinstated the license of pharmacist Nanette O’Shay (PH00018892) and placed her license on probation for five years. O’Shay must comply with terms and conditions set against her license. She is also known as Nanette Hollingsworth and Nanette Cotter.
In October 2008 the Nursing Commission entered into an agreed order with registered nurse Linda L. Martin (RN00089136). She failed to comply with previous terms and conditions set against her license. She must comply with new terms and conditions set against her license.
In November 2008 the Registered Counselor Program stayed the suspension of Valerie Morford‘s credential (RC00049814). She must comply with terms and conditions set against her license.
In October 2008 the Nursing Commission terminated the compliance monitoring of registered nurse Margaret Ellen Muai (RC00066498).
In October 2008 the Nursing Assistant Program denied the application of Raksa Son (NC60029011). She was convicted of assault – domestic violation and two counts of protection order violation – domestic violence.
In October 2008 the Registered Counselor and Sex Offender Treatment Provider Programs charged Lang Taylor (RC00006893, FC00000022) with unprofessional conduct. He allegedly failed to provide timely and accurate quarterly progress reports for clients, and presented a quarterly report for a client that was identical to two previous quarterly reports for that client.
In November 2008 the Board of Pharmacy charged pharmacy technician Brandi Hendricks (VA00062622) with unprofessional conduct. She allegedly diverted drugs from her place of employment.
In October 2008 the Board of Pharmacy terminated the probation for pharmacist Tasha N. Wong (IR00053006, PH00071841).
In October 2008 the Nursing Commission reinstated the license of licensed practical nurse Patricia S. Scheuffele (LP00049847). She must comply with terms and conditions set against her license.
In November 2008 the Nursing Commission charged registered nurse Annette M. Knoll (RN00094378) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.
In November 2008 the Nursing Assistant Program released Julie A. Teske (NA00129510) from probation. She is also known as Julie A. Raymont.
In October 2008 the Chemical Dependency Program denied the application of Nova D. Zaro (CP60027736). Her license to practice as a pharmacy technician was indefinitely suspended.
In November 2008 the Nursing Assistant Program granted the reactivation application of Jennifer L. McKinney (NC10075495) and placed restrictions against her license. She was convicted of reckless endangerment and minor in possession/consumption of alcohol. McKinney must comply with terms and conditions set against her license.
In November 2008 the Registered Counselor Program granted the application of David J. Merscher (RC60023143) and placed his registration on probation for one year. He was convicted of unlawful manufacture of a controlled substance – marijuana. He must comply with terms and conditions set against his license.
In October 2008 the Nursing Commission entered into an agreed order with licensed practical nurse Andrea R. Wilson (LP00050785). She failed to assess a patient as ordered or inform other nursing staff that the assessment needed to be done. She must comply with terms and conditions set against her license.
In October 2008 the Nursing Commission modified the order against licensed practical nurse Kathleen Hardcastle-Aleck (LP00028920). Her license is reinstated but placed on probation for one year. She must comply with terms and conditions set against her license.
In October 2008 the Registered Counselor Program granted the application of Lisa M. Lafferty (RC00059809) and placed her registration on probation for three years. She was convicted of solicitation to posses a controlled substance – cocaine. She must comply with terms and conditions set against her license.
In October 2008 the Board of Pharmacy entered into an agreed order with pharmacist Donald Fortner (PH00013635). He diverted drugs from his place of employment and submitted a urinalysis that tested positive for a legend drug. He must comply with terms and conditions set against his license.
In October 2008 the Nursing Commission granted the license of licensed practical nurse Sara R. Kastholm (LP00059996) and placed restrictions against her license. While working as a nursing technician, she practiced beyond her scope by giving a patient a controlled substance. She also documented giving a patient medication while failing to administer the medication to the patient. She must comply with terms and conditions set against her license.
Alabama: In September the Medical Commission denied the application of physician Jeffery R. Long (MD00049390). He had sexual relationships with two patients in Alabama.
California: In October 2008 the Chiropractic Commission denied the reactivation application of David P. Young (CH00002902). His license to practice as a chiropractor in California was revoked.
