OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Asotin County
In October 2008 the Nursing Commission suspended the license of registered nurse Sam Olson (RN00056815). The suspension is stayed and Olson must comply with terms and conditions.
Clark County
In November 2008 the Nursing Assistant Program suspended the credential of Angela M. Uruo (NA00199800). Uruo failed to comply with a previous order.
Columbia County
In November 2008 the Nursing Commission suspended the license of registered nurse Mary J. Fordyce (RN00174914). Fordyce’s license to practice nursing in Oregon was revoked.
Cowlitz County
In November 2008 the Registered Counselor Program suspended the credential of Shannon R. Shinn (RC00051935). Shinn failed to comply with previous terms set against her credential.
Grays Harbor County
In November 2008 the Nursing Assistant Program indefinitely suspended the credential of Debra Marie Miller (NAR.NA.00004543). Miller yelled at, insulted, and made disparaging remarks about a patient. She also yelled at another patient and discussed a patient’s medical condition in front of other patients without proper consent.
King County
In November 2008 the Nursing Commission indefinitely suspended the license of registered nurse Christine D. Banman (RN00139211). Banman tampered with medications at the surgery center where she worked and used the drugs herself. She also failed to comply with previous terms set against her license. She’s also known as Christine D. Hartsock.
In November 2008 the Secretary of Health withdrew charges against registered counselor Rebecca L. Gaughan (RC00047605).
In November 2008 the Massage Program suspended the credential of Sonja M. Gleason (MA00022370). Gleason failed to comply with a previous order.
Kitsap County
In November 2008 the Nursing Commission indefinitely suspended the license of registered nurse Robin Karol Rough (RN00099645). Rough failed to comply with previous terms set against her credential.
Lewis County
In November 2008 Pharmacy Board summarily suspended the license of pharmacist Alvin Fitzgerald (PH00021040). Fitzgerald’s license to practice pharmacy was suspended in Montana.
Lincoln County
In November 2008 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Terry L. Garvin (NC10080705). Garvin was convicted of three counts of first degree theft and five counts of health care false statement.
Mason County
In November 2008 the Nursing Assistant Program suspended the credential of Rebecca J. Elam (NC10102470). Elam failed to comply with a previous order. She’s also known as Rebecca J. Fillmore.
In November 2008 the Pharmacy Board revoked the credential of pharmacy technician Rebecca G. Turk (VA00040637). Turk failed to comply with a previous order and diverted drugs from her former employer.
Okanogan County
In November 2008 the Nursing Commission indefinitely suspended the license of licensed practical nurse Robyn Henderson (LP00054419). Henderson was convicted of obtaining a controlled substance by forged or altered prescription, reckless driving, and hit and run of an attended vehicle.
Pierce County
In November 2008 the Nursing Commission suspended the license of licensed practical nurse Roberta M. Manford (LP00013636). Manford, while working at a long term care facility, made several expenditures for her own use totaling more than $2600 using a resident’s personal checking account.
In November 2008 the Nursing Commission suspended the license of registered nurse Karen J. Manners (RN00074457). Manners failed to comply with a previous order.
In November 2008 the Nursing Assistant Program suspended the credential of certified nursing assistant Michaela M. Sistrunk (NC10091744). Sistrunk was convicted of third degree assault.
Skagit County
In November 2008 the Nursing Assistant Program revoked the credential of certified nursing assistant Kevin F. White (NC10086081). White approached a vulnerable patient and offered to show his penis, asked for oral sex, and tried to place his penis in the patient’s mouth. He was also convicted of third degree malicious mischief and third degree theft.
Snohomish County
In November 2008 the Naturopathy Program accepted the license surrender of Elizabeth Mc Donald (NT00000659). Mc Donald can’t practice as a doctor of naturopathic medicine in Washington. For several years she didn’t follow through with additional evaluation or referral for a patient who later died of cancer.
Spokane County
In November 2008 the Nursing Assistant Program indefinitely suspended the credential of Cathy J. Chapman (NA00201382). Chapman was convicted of third degree assault and second degree malicious mischief.
In November 2008 the Licensed Social Worker Program suspended the credential of David P. Stanzak (LW00006436). Stanzak made inappropriate sexual comments to two patients and placed the patients at risk by failing to assess and address their goals, needs and vulnerabilities.
Thurston County
In October 2008 the Nursing Commission suspended the license of Karen R. Courtney (RN00168282). Courtney failed to comply with terms and conditions set against her license.
Walla Walla County
In October 2008 the Nursing Commission indefinitely suspended the license of registered nurse Nichole E. Nichols (RN00144363). Nichols failed to comply with a previous order.
Whatcom County
In November 2008 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Brian J. Chaput (NC10085101). Chaput used heroin.
Whitman County
In November 2008 the Nursing Commission suspended the license of Scot L. Askham (RN.RN.00163755). Askham was convicted of child pornography distribution.
California: In November 2008 Nursing Commission summarily suspended the license of registered nurse Laurie J. Brown (RN00165616). Brown’s license to practice nursing in California was revoked.
California: In November 2008 the Nursing Commission summarily suspended the license of registered nurse Kittra J. O’Conner (RN00122054). O’Conner surrendered her license and is prohibited from practicing nursing in Arizona.
North Dakota: In November 2008 the Nursing Commission summarily suspended the license of registered nurse Shawn E. Jacob (RN00122949). Jacob’s license to practice nursing in North Dakota was suspended.
