State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In November 2008 the Chiropractic Commission terminated probation against the license of chiropractor David W. Chan (CH00001534).

Clark County

In November 2008 the Nursing Commission charged licensed practical nurse Patricia A. Viles (LP00055508). She allegedly administered insulin to a patient without confirming the proper dosage. The patient died later that evening from insulin administration.

Cowlitz County

In November 2008 the Registered Counselor Program granted the application of Brad Lance Jensen (RC60023340) and placed his registration on probation for two years. He was convicted of driving under the influence on more than one occasion. He must comply with terms and conditions set against his registration.

Douglas County

In November 2008 the Board of Pharmacy granted the application of pharmacist Kenyon N. Lyle, Jr. (PH60018753) and placed his license on probation for five years. Restrictions were placed against his license to practice as a pharmacy intern. He must comply with terms and conditions set against his license.

Franklin County

In November 2008 the Nursing Commission placed the license of licensed practical nurse Helen Henrikson (LP00044172) on probation for two years. She incorrectly hooked up a patients feeding tube, and failed to administer the correct dosage of medication at the correct time for two other patients. She must comply with terms and conditions set against her license.

King County

In November 2008 the Medical Commission charged physician Peter Balousek (MD00035044) with unprofessional conduct. He allegedly provided negligent care to a patient.

In November 2008 the Nursing Commission reinstated the license of registered nurse Melissa L. Beech (RN00106401).

In November 2008 the Nursing Commission amended the statement of charges against licensed practical nurse Miriam V. Bernales (LP00048674). She allegedly provided substandard care to a patient in the adult family home she operated and was convicted of criminal mistreatment.

In November 2008 the Dental Commission corrected the statement of charges against Mark Caggiano (DE00006697). Allegations include performing unnecessary treatments, failing to have an interpreter present and failing to adequately chart examinations.

In November 2008 the Chemical Dependency and Licensed Mental Health Programs charged Geoffrey J. L. Chipps (CH00002870, LH00007078) with unprofessional conduct. He allegedly had a sexual relationship with a patient and made inappropriate sexual advances to other clients.

In November 2008 the Registered Counselor Program charged Kelly A. Cockrell (RC00054753) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

In October 2008 the Board of Pharmacy reinstated the licenses of pharmacy assistant and intern Richard P. Cole (VB00052991, IR00064870) and placed them on probation for five years. He must comply with terms and conditions set against his licenses.

In October 2008 the Nursing Commission terminated the probation against registered nurse Thomas D. Herwick (RN00114587).

In October 2008 the Board of Pharmacy terminated probation against pharmacist Michael S. Jones (PH00010993).

In November 2008 the Nursing Commission charged licensed practical nurse James Konzek (LP00045307) with unprofessional conduct. Allegations include failing to document administration of medications, diverting controlled substances for his own use, and testing positive for drugs during a urinalysis. He was convicted of possession of oxycodone.

In November 2008 the Nursing Assistant Program charged Carla S. Linwood (NC10091538) with unprofessional conduct. She allegedly left a patient alone which placed the patient at risk of harm.

In November 2008 the Nursing Assistant Program granted the application of Jackson M. Muriuki (NA60008337) and placed his license on probation for two years. He was convicted of vehicular assault. He must comply with terms and conditions against his license.

In November 2008 the Registered Counselor Program charged Edrene Renee Patterson (RC00027351) with unprofessional conduct. She allegedly failed to check on a suicidal patient every five minutes as ordered but initialed a log stating she had checked the patient. The patient committed suicide.

In November 2008 the Licensed Mental Health Counselor Program charged Gary R. Sarozek (LH00007235) with unprofessional conduct. He allegedly sent a client’s domestic violence report to a third party without the client’s consent.

In November 2008 the Massage Program denied the application of Christopher J. Tafoya (MA00024768). He had sexual contact with a patient while working in California. He can’t reapply for seven years.

Lewis County

In November 2008 the Emergency Medical Services and Trauma System Program charged emergency medical technician Bridgette N. Watson (ES01172547) with unprofessional conduct. She allegedly had sex with two minors and was convicted of two counts of rape of a child.

Mason County

In November 2008 the Emergency Medical Services and Trauma System Program reprimanded paramedic Barbara J. Bodin (ES00102715). She continued to work as a paramedic for three years with an expired credential.

Pierce County

In October 2008 the Nursing Assistant Program denied the reactivation application of Tisha E. Renner (NA00144474). She failed to cooperate with a department investigation regarding her application.

Snohomish County

In November 2008 the Registered Counselor Program charged Susan M. Riley (RC00047415) with unprofessional conduct. She allegedly failed to check on a suicidal patient every five minutes as ordered but initialed a log stating she had checked the patient. The patient committed suicide.

In November 2008 the Emergency Medical Services and Trauma System Program charged paramedic Paul W. Sheward (ES01169300) with unprofessional conduct. He allegedly gave his employer falsified certificates of completion for continuing education and experience.

Spokane County

In November 2008 the Nursing Assistant Program charged Cheryl L. Bengry (NC10054574) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

Whatcom County

In October 2008 the Nursing Commission reinstated the license of licensed practical nurse Kathi L. Downing (LP00042025).

Out of State

Arizona: In November 2008 the Medical Commission amended the statement of charges against physician Keith Levitt (MD00036391). He was convicted of acquiring a controlled substance by misrepresentation, deception, and subterfuge.

New Mexico: In November 2008 the Board of Pharmacy charged pharmacist Paul A. Schentrup (PH00018372) with unprofessional conduct. His New Mexico license was suspended and placed on probation.

Oregon: In October 2008 the Nursing Commission modified an earlier order and placed the license of registered nurse Norman E. Ryerson (RN00159090) on probation until his Oregon license has been fully reinstated. He must comply with terms and conditions set against his license.
