State disciplines health care providers

OLYMPIA ľ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In December 2008 the Nursing Commission charged licensed practical nurse Gina L. Castle (LP00047596) with unprofessional conduct. She allegedly documented medication was administered to patients when the medications had not been used. She also allegedly tested positive for methamphetamines on two occasions.

Clark County

In January 2009 the Nursing Assistant Program reinstated the registration of Angela Uruo (NA00199800) and placed her registration on probation until November 2010. She must comply with terms and conditions set against her registration.

Cowlitz County

In January 2009 the Nursing Assistant Program charged certified nursing assistant Alan D. Beerbohm (NC10093881) with unprofessional conduct. He allegedly gave a patient liquid with a standard cup causing the patient to cough and choke. He allegedly continued to use the standard cup even after he was instructed to use a sip cup. Beerbohm was also convicted of driving under the influence on more than one occasion.

In January 2009 the Registered Counselor Program reinstated the registration of Katrina L. Cansler (RC00053072).

Grant County

In January 2009 the Nursing Commission charged registered nurse Latanya J. Lucas (RN00149289) with unprofessional conduct. She allegedly failed to comply with previous terms and conditions set against her license.

Jefferson County

In January 2009 the Registered Counselor Program granted the application of Clarence William Wetherald, Sr. (RC60032063) and placed his registration on probation for two years. He was convicted of delivery of a controlled substance, felony attempt to elude, identity theft on more than one occasion, and possession of a controlled substance on more than one occasion. He must comply with terms and conditions set against his registration.

King County

In January 2009 the Registered Counselor Program granted the application of Jim Ray Rainwater (RC60043115) and placed his registration on probation for two years. He was convicted of conspiracy to distribute narcotics. He must comply with terms and conditions set against his registration.

Mason County

In December 2008 the Nursing Assistant Program denied the application of Ashley Rose Weber (NA60044707). She was convicted of unlawful issuance of bank checks.

Pierce County

In December 2008 the Registered Counselor Program reinstated the registration of Robert C. Boord (RC00048199) and placed his registration on probation for three years. He must comply with terms and conditions set against his registration.

In January 2009 the Nursing Assistant Program charged Marilou P. Faust (NA00121619) with unprofessional conduct. She was convicted of misbranding drugs while held for sale with intent to mislead or defraud.

In December 2008 the Registered Counselor Program charged Denise Haavik (RC00055002) with unprofessional conduct. She was convicted of unlawful possession of controlled substance – methamphetamine.

In December 2008 the Board of Physical Therapy charged Nancy F. Wong (PT00002236) with unprofessional conduct. The physical therapy clinic she owned was convicted of two counts of health care fraud.

Snohomish County

In January 2009 the Registered Counselor Program granted the application of Diana M. Hansen (RC60011534) and placed terms and conditions against her registration. She was convicted of attempted theft, theft, and burglary. She must comply with terms and conditions.

In December 2008 the Nursing Assistant Program entered into an agreed order with certified nursing assistant Nasya K. Hilton (NC10088339). She bumped a wheelchair patient’s leg against a doorjamb, putting the patient at risk of harm. She must comply with terms and conditions set against her certification.

In December 2008 the Chemical Dependency Professional Program denied the application of Robert A. Stankaitis (CP00006447). His registration to practice as a registered counselor was revoked. He was convicted of criminal mischief over $50.

Spokane County

In December 2008 the Nursing Assistant Program denied the applications of certified nursing assistant Charity R. Holstein (NA60015942, NC10101006). She was convicted of assault – domestic violence and failed to respond to department inquiries regarding the conviction.

In December 2008 the Nursing Assistant Program reinstated the certification of certified nursing assistant Joshua C. Hooper (NC10065365). He must comply with terms and conditions set against his certification.

In December 2008 the Board of Pharmacy terminated the order of probation against pharmacist Gerald Mamishian (PH00009592).

In December 2008 the Nursing Assistant Program denied the application of Jennifer Lynn Pettit (NA60024320). She was convicted of theft.

In January 2009 the Registered Counselor Program charged Nova D. Zaro (RC00055119) with unprofessional conduct. Her license to practice as a pharmacy technician was indefinitely suspended. She is also known as Paula N. Miotke.

Thurston County

In December 2008 the Board of Pharmacy terminated the order of probation against pharmacy technician Penny Buell (VA00043182).

In January 2009 the Nursing Assistant Program placed the certification of certified nursing assistant Marsha V. Edwards (NC10043137) on probation for three years. She borrowed $1000 from a patient and failed to pay back as agreed. She must comply with terms and conditions.

In January 2009 the Registered Counselor Program granted the application of Walter Kenneth Reichel (RC60002409) and placed his registration on probation for two years. He was convicted of unlawful manufacturing – methamphetamine and malicious mischief. He must comply with terms and conditions set against his registration.

Whatcom County

In January 2009 the Registered Counselor and Chemical Dependency Professional Programs charged Paul R. MacBeth (RC00004492, CP00003069) with unprofessional conduct. He allegedly allowed a patient to live on his property and sold drugs to law enforcement during undercover operations.

Out of State

Michigan: In December 2008 the Medical Commission charged physician Christopher P. Kelly (ML20008044) with unprofessional conduct. He allegedly crossed patient-physician boundaries and entered into a personal relationship with a patient. He also allegedly disclosed personal and medical information about the patient without authorization or consent.

Oregon: In December 2008 the Chemical Dependency Professional Program granted the application of Julee A. Mutch (CP60042252) and placed her license on probation for two years. She was convicted of negligent homicide. She must comply with terms and conditions set against her license.

Utah: In November 2008 the Medical Commission denied the request for a temporary permit and physician and surgeon application of George B. Keenan (MD60017153, TD60017154). He was suspended from a previous place of employment and settled several malpractice claims.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
