OLYMPIA ¾ The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In June 2009 the Registered Counselor and Chemical Dependency Professional Programs terminated the order of probation against Sharon L. Barrier-Yacinich (RC00020789, CP00000040).
In June 2009 the Registered Counselor Program granted the application of Miguel Angel Mejia Mendoza (RC60084215) and placed his registration on probation for three years. He was convicted of theft, possession of stolen property, attempted theft, and unlawful possession of a controlled substance — methamphetamine. He must comply with terms and conditions set against his registration.
In June 2009 the Nursing Assistant Program charged Marcy L. Thatcher (NA00183940) with unprofessional conduct. She was convicted of assault.
King County
In June 2009 the Registered Counselor Program charged Christopher Bruce Evans (RC00040982) with unprofessional conduct. He allegedly had a sexual relationship with a client.
In June 2009 the Dental Commission released dental assistant Jessica L. Fisher (D160057453) from terms and conditions set against her registration.
In June 2009 the Health Care Assistant Program granted the application of Melvin Lee Harris, Jr. (HC60083732) and placed his license on probation for two years. He was convicted of reckless driving, and must comply with terms and conditions set against his license.
In May 2009 the Nursing Commission entered into an agreed order with licensed practical nurse James R. Jovick (LP00040424). He failed to comply with previous terms and conditions set against his license, and must now comply with new terms and conditions against his license.
In June 2009 the Health Care Assistant Program charged Lakisha M. Lollie (HC00156052) with unprofessional conduct. She allegedly called in and faxed prescriptions for controlled substances for herself using a co-workers name and physician’s name without their authorization.
In June 2009 the Radiological Technologist Program terminated the order of probation against A. Brian Stambuk (RT00007655).
In June 2009 the Denturist Program charged Herminia H. Subaran (DN00000011) with unprofessional conduct. An inspection in the clinic she owned, allegedly showed a number of deficiencies in sterilization and testing. She also allegedly failed to display her credential in a conspicuous place within her office.
Kitsap County
In June 2009 the Dental Commission released dentist Robert T. Erickson (DE00006354) from terms and conditions set against his license.
In June 2009 the Registered Counselor Program granted the application of Cameron Yvonne Fisher (RC00058679) and placed her registration on probation for two years. Her previous application was denied due to a conviction. She must comply with terms and conditions set against her registration.
In June 2009 the Chemical Dependency Professional Program terminated the order of probation against Stephanie R. McKendry (CP60026487).
In May 2009 the Licensed Marriage and Family Therapist Program charged Michelle G. Naden (LF00001540) with unprofessional conduct. Allegations include failing to get a disclosure statement from a patient, failing to get a written release from a patient to contact a family member and attorney, crossing professional boundaries, accepting personal gifts from a patient, failing to assess the patients needs and develop a plan to address her needs, and failing to arrange transfer care for the patient when Ms. Naden went on maternity leave.
Klickitat County
In June 2009 the Nursing Assistant Program reinstated the registration of Pamela K. Meyer (NA00053271).
Okanogan County
In June 2009 the Registered Counselor Program released Michael F. Boyd (RC00055527) from terms and conditions set against his registration.
Pierce County
In June 2009 the Board of Pharmacy granted the application of pharmacy assistant Jennifer E. Carr (VB60051191) and placed conditions against her license. She was convicted of robbery and assault. She must comply with terms and conditions set against her license.
In June 2009 the Nursing Assistant Program charged Tyrone A. Davis (NA00188662) with unprofessional conduct. He was convicted of murder, assault, and unlawful possession of a firearm. He is also known as Quincy V. Hawkins.
In June 2009 the X-Ray Technician Program released Donna L. Gorenson (XT00006299) from terms and conditions set against her registration. She is also known as Donna L. Zumwalt.
In June 2009 the Nursing Assistant Program charged certified nursing assistant Megan N. King (NC10093115) with unprofessional conduct. She allegedly took $30 from a patient to purchase hair products for the patient and didn’t purchase the products or return the money to the patient. She also allegedly failed to respond to department inquiries regarding the incident.
In June 2009 the Nursing Assistant Program charged certified nursing assistant Rick Mason (NC10018432) with unprofessional conduct. He allegedly exposed his genitals to a patient.
In June 2009 the Nursing Assistant Program charged Ina L. Qawasmeh (NA00169184) with unprofessional conduct. She was convicted of assault — domestic violence.
Skagit County
In June 2009 the Registered Counselor Program granted the application of Tiffany Shireen Barley (RC60056414) and placed her registration on probation for three years. She was convicted of disorderly conduct, forgery, and possession of stolen property. She must comply with terms and conditions set against her registration.
Snohomish County
In June 2009 the Veterinary Board of Governors granted the application of animal technician Jill M. Lewis (AT60014155) and placed her license on probation for three years. She created an account at her place of employment without permission. The account was coded with no late fee, no billing fee, and no statement. She transferred a patient’s balance of $480 to the new account. She must comply with terms and conditions set against her license.
Spokane County
In June 2009 the Nursing Assistant Program charged Brandon M. Affre (NA00200412) with unprofessional conduct. He allegedly took a wedding ring from a patient and sold it at a pawn shop. He was convicted of malicious mischief.
In June 2009 the Registered Counselor Program granted the application of Deborah Olin (RC60039227) and placed her registration on probation for three years. She was convicted of unlawful possession of a controlled substance on more than one occasion, and possessing stolen property.
In June 2009 the Medical Commission charged physician Nathan P. Stime (MD00010797) with unprofessional conduct. He allegedly practiced below the standard of care while treating patients.
Yakima County
In June 2009 the Nursing Assistant Program reinstated the certification of certified nursing assistant Gary L. Davis (NC10047920) and placed it on probation for two years. He must comply with terms and conditions set against his certification.
In June 2009 the Licensed Mental Health Counselor Program entered into an agreed order with Mark E. Heeringa (LH00008002). He entered into a client referral contract with another mental health provider. He must comply with terms and conditions set against his license.
In June 2009 the Licensed Mental Health Counselor Program entered into an agreed order with Pia P. Hull (LH00007865). She entered into a client referral contract with another mental health provider. She must comply with terms and conditions set against her license.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
