The department’s Health Systems Quality Assurance Office works
with boards, commissions and advisory committees to set licensing standards for
more than 70 health care professions (e.g., medical doctors, nurses,
counselors).
Information about health care providers is on the agency’s
Web site. Click on “Provider Credential Search” on the left hand side of the
Department of Health home page (www.doh.wa.gov). The site includes
information about a health care provider’s license status, the expiration and
renewal date of their credential, disciplinary actions and copies of legal
documents issued after July 1998. This information is also available by calling
360-236-4700. Consumers who think a health care provider acted unprofessionally
are also encouraged to call and report their complaint.
In June of 2009 the Massage Program indefinitely suspended
the credential of licensed massage therapist Jason T. Lutz
(MASS.MA.00020848) after the Oregon State Board of Massage Therapists suspended
his credential to practice in that state.
In July of
2009 the Nursing Care Quality Assurance Commission summarily suspended the
credential of registered nurse Amy L. Ford (RN.RN.00132500) after the
Montana State Board of Nursing suspended her credential to practice in that
state. She cannot practice pending
a hearing.
In July of
2009 the Registered Counselor Program indefinitely suspended the credential of
Phoebe M. Olivera, also known as Phoebe Marie Muro
(RC.RC.00059530). She failed to comply with terms of an earlier order.
In June of 2009 the Naturopathy Program corrected a
modification order with naturopathic physician Mark D. Groven (NATU.NT.00000649)
in which a suspension of his credential may be stayed after 90 days if he
complies with certain conditions.
In July of 2009 the Registered Counselor Program entered
into an agreed order with Christopher Bruce Evans (RC.RC.00040982) in
which his credential is indefinitely suspended. Evans had sex with a client.
In July of
2009 the Registered Counselor Program indefinitely suspended the credential of
Vickie T. Anderson (RC.RC.00056803). She failed to comply with terms of
an earlier order.
In July of 2009 the Department of Health withdrew a
statement of charges against respiratory care practitioner Patricia D.
Martinell (RESP.LR.00001510).
In June of
2009 the Board of Pharmacy entered into an agreed order with pharmacy assistant
and pharmacy technician Sabra L. Sin (PHAA.VB.00066172, PTEC.VA.60017222)
in which Sin’s credential is revoked for at least 10 years. Sin stole controlled
substances for personal use while filling prescriptions.
In July of
2009 the Registered Counselor Program indefinitely suspended the credential of
Randall S. Johnston (RC.RC.00059982) because he failed to comply with
terms of an earlier agreement to practice with conditions.
In July of
2009 the Health Care Assistant Program entered into an agreed order with
Kathleen C. Egan (HCA.HC.00151264) in which her credential is suspended for
at least two years. Egan failed to comply with terms of an earlier order.
In July of
2009 the Registered Counselor Program indefinitely suspended the credential of
Greg R. Eickmier (RC.RC.60045897) because he failed to comply with terms
of an earlier agreement.
In June of
2009 the Registered Counselor Program modified an agreed order with Stacey E.
Barrus (RC.RC.60066016) and indefinitely suspended her credential.
In July of 2009 the Health Care Assistant Program entered into an agreed order
with Richard A. Cokusis (HC00156295) in which his credential is
indefinitely suspended. Cokusis failed to comply with terms and conditions
against his license.
