State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency’s Web site. Click on "Provider Credential Search" on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In July 2009 the Nursing Commission summarily suspended the credential of registered nurse Charles B. Miller (RN.RN.00134940). Miller’s license to practice nursing in Oregon was revoked. He remains prohibited from practicing there. In August 2009 the Nursing Commission indefinitely suspended Miller’s license to practice nursing in Washington.
In June 2009 the Nursing Assistant Program permanently revoked the credential of certified nursing assistant James R. Pugh (NAC.NC.10042994). Pugh was convicted of second degree manslaughter relating to the death of a four-year-old boy.
In July 2009 the Psychology Board accepted the voluntary credential surrender of Thomas M. Stallone (PSYC.PY.0001974). Stallone had sex with a client. He also had a romantic relationship with a different client after he stopped treating her. Stallone also had a client sign a release of medical records to another client for no therapeutic reason.
Grant County
In July 2009 the Nursing Commission indefinitely suspended the license of registered nurse Latanya J. Lucas (RN00149289). Lucas failed to comply with previous terms and conditions set against her license.
In July 2009 the Health Care Assistant and Nursing Assistant Programs indefinitely suspended the credentials of Tequia F. Perkins (HCA.HC.00142935, NAC.NC.10062982). Perkins was convicted of third degree theft. She accepted and cashed checks totaling $3,900 from a resident at the facility where she worked. Perkins was terminated for the incident.
Grays Harbor County
In July 2009 the Medical Commission revoked the license of physician Antoine Johnson (MD00039048). Johnson failed to meet the standard of care in treating and prescribing medications for more than two dozen patients. This put the patients at risk of serious harm.
In July 2009 the Nursing Assistant Program indefinitely suspended the registration of Marcy L. Thatcher (NAR.N.00183940). Thatcher was convicted of third degree assault.
King County
In July 2009 the Pharmacy Board revoked the pharmacy firm credential of AMS Home Care USA, Inc. doing business as 65 Plus; Rani Gill, President; and Harj Gill, CEO. After the 1997 closure of their pharmacy, the Gills left partial bottles of controlled substances, drugs requiring refrigeration, over the counter medications, their Drug Enforcement Agency (DEA) license, blank DEA order forms, and blank prescription pads on the premises.
In July 2009 the Pharmacy Board withdrew the statement of charges against King Pharmacy (PHAR.CF.00058681).
In July 2009 the Massage Practitioner Program indefinitely suspended the credential of Nicholas Schmidt (MA00023623). Schmidt failed to comply with previous terms and conditions set against his credential.
Pierce County
In July 2009 the Nursing Assistant Program indefinitely suspended the credential of Adam Hren (NAR.NA.00169790). Hren failed to comply with previous terms and conditions set against his credential.
In July 2009 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Megan N. King (NAC.NC.10093115). King accepted money from a resident at the facility where she worked. She was to use the money to buy something for the resident, but failed to buy the product or return the money. King also failed to respond to department investigators about the incident.
In July 2009 the Registered Counselor and Chemical Dependency Professional Programs indefinitely suspended the credentials of David Charles Larsen (RC.RC.00021390, CDP.CP.00000530). Larsen purchased a Doctor of Psychology degree from St. Regis University, an online "diploma mill". He misrepresented his education and training on a resume he submitted for a counseling position.
In July 2009 the Medical Commission denied the request of a full license reinstatement for physician Lance A. May (MD00038397). May is permitted to practice in an approved adult male correctional setting. The commission also permanently restricted May’s license. He cannot treat minors and can never petition the commission to modify this restriction.
In July 2009 the Nursing Assistant Program permanently revoked the credential of certified nursing assistant Brandy M. Pittman (NAC.NC.10075906). Pittman was convicted of third degree assault and two federal counts of possession with intent to distribute cocaine base. Pittman also deposited and cashed checks belonging to a resident at the nursing home where she worked. These checks totaled $4,000 and were not authorized or written by the resident. Pittman failed to respond to a department investigator about the incident.
In July 2009 the Nursing Assistant Program indefinitely suspended the credential of Ina L. Qawasmeh (NAR.NA.00169184). Qawasmeh was convicted of third degree assault.
Snohomish County
In July 2009 the Health Care Assistant Program indefinitely suspended the credential of Michael D. Buckner (HC00149179, HC00155176). Buckner inappropriately touched a patient’s breasts and made an inappropriate comment to the patient. He also called the patient on her personal cell asking to get together at her home for a massage. The patient feared for her safety because she never provided contact information to Buckner, only to the hospital.
In July 2009 the Nursing Assistant Program indefinitely suspended the credential of Fatou M. Barrow (NAR.NA.00145690). Barrow was convicted of third degree theft. Barrow also provided falsified certificates and documentation to her health care employer.
Spokane County
In July 2009 the Nursing Assistant Program indefinitely suspended the credential of Heather Y. Colwell (NAR.NA.00144215). Colwell never replied to a department investigator who was looking into a complaint about medication theft at the facility where Colwell worked.
In June 2009 the Registered Counselor and Chemical Dependency Professional Programs suspended the credentials of Ronnie B. Fowler (RC00032953, CP00003067). Fowler had a sexual relationship with a client.
In June 2009 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Joshua C. Hooper (NAC.NC.10065365). Hooper failed to comply with previous terms and conditions set against his credential.
Walla Walla County
In July 2009 the Nursing Assistant Program permanently revoked the credential of Tyrone A. Davis (NAR.NA.00188662). Davis was convicted of second degree murder, second degree assault, and first degree unlawful possession of a firearm. He’s also known as Quincy V. Hawkins.
Yakima County
In August 2009 the Registered Counselor Program indefinitely suspended the credential of Barbara J. Allen (RC.RC.00051196). While serving as a vulnerable client’s care provider, bookkeeper, and power of attorney, Allen used the client’s money for her personal use without permission. This conduct occurred over a three-year period. Allen also opened up a credit card account in the client’s name without permission and used it for personal use. She was convicted of second degree identity theft and forgery relating to this conduct.
Idaho: In July 2009 the Nursing Commission summarily suspended the license of licensed practical nurse Daniel L. Austin (LPN.LP.00052814). Austin’s license to practice nursing in Idaho was summarily suspended and he remains prohibited from practicing there.
Louisiana: In July 2009 the Nursing Commission indefinitely suspended the license of registered nurse Mary G. Faucheaux (RN.RN.00152333). Faucheaux’s licenses to practice nursing in Louisiana and in California are on probation.
Oregon: In August 2009 the Nursing Commission summarily suspended the license of registered nurse Kimberly T.D. Joers (RN.RN.00177473). Joers voluntarily surrendered her license to practice nursing in Oregon. She remains prohibited from practicing there.
Oregon: In July 2009 the Pharmacy Board revoked the credential of pharmacy technician Josh A. Waggoner (PTEC.VA.00062531). Waggoner was convicted of possession of hydrocodone and third degree theft. He failed to respond to department investigators about the incident.
