OLYMPIA The departments Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on Provider Credential Search on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care providers license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In June 2009 the Nursing Assistant Program granted the application of Efrain P. Roman (NA60050463) and placed his registration on probation for two years. He was convicted of intimidating a public servant. He must comply with terms and conditions set against his registration.
Cowlitz County
In June 2009 the Registered Counselor and Chemical Dependency Professional Programs released Krista R. Heppe (RC00054943, CP60002760) from terms and conditions set against her registration and certification.
Franklin County
In June 2009 the Board of Optometry continued the restrictions against the license of Kelly J. Cochrane (OD00001536). He must comply with terms and conditions set against his license.
Grays Harbor County
In June 2009 the Massage Therapy Program terminated the order of probation against Jarrid D. Horn (MA00008352).
Island County
In June 2009 the Medical Commission entered into an agreed order with physician Wylie N. Vracin (MD00020146). He must comply with terms and conditions set against his license.
King County
In June 2009 the Medical Commission released physician William G. Couser (MD00020107) from terms and conditions set against his license. Dr. Cousers license is unrestricted.
In June 2009 the Naturopathy Program placed the license of Anne L. Davis (NT00000421) on probation for two years. She caused unreasonable risk of harm to a seven-year-old patient by over prescribing Vitamin A, providing expired supplements to the patient, failing to inform the parents of the risks of taking the vitamins and supplements, and failing to do an in-person follow-up with the patient. She must comply with terms and conditions set against her license.
In June 2009 the Health Care Assistant Program modified the agreed order against Tommy L. Grissette (HC00147976). He must comply with terms and conditions set against his license.
In May 2009 the Nursing Commission entered into an agreed order with registered nurse David House (RN00174958). His applications to become a registered nurse in California were denied, and his license to practice as a vocational nurse in California was revoked. He must comply with terms and conditions set against his license.
In June 2009 the Board of Pharmacy charged pharmacy technician Gail L. Kostelecky (VA00021871) with unprofessional conduct. She allegedly stole controlled substances from her place of employment and ingested the drugs without a doctors prescription during her workday.
In June 2009 the Nursing Assistant Program reinstated the registration of Nelson R. Lopecillo (NA00190366).
Kitsap County
In June 2009 the Medical Commission charged physician Julie A. McMillan (MD00041147) with unprofessional conduct. She allegedly practiced below the standard of care while treating two pregnant women.
In May 2009 the Registered Counselor Program granted the application of Anthony Lee Moore (RC60078746) and placed his registration on probation for three years. He was convicted of vehicle prowling, burglary, possession of a dangerous weapon on more than one occasion, forgery on more than one occasion, possession of a controlled substance, taking a motor vehicle without permission, driving while license suspended, assault domestic violence, and violation of a protection order. He must comply with terms and conditions set against his registration.
Mason County
In June 2009 the Registered Counselor Program released Sheryl L. Bragg (RC00054941) from terms and conditions set against her registration.
Pierce County
In June 2009 the Nursing Assistant Program charged Adam Hren (NA00169790) with unprofessional conduct. He allegedly failed to comply with terms and conditions set against his registration.
Snohomish County
In June 2009 the Nursing Assistant Program amended the statement of charges against Fatou M. Barrow (NA00145690). She was convicted of theft and allegedly provided falsified training certificates and documentation to her employer.
In June 2009 the Health Care Assistant Program charged Kathleen C. Egan (HC00151264) with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her license.
In June 2009 the Nursing Assistant Program charged certified nursing assistant Molly J. Marshall (NC10069008) with unprofessional conduct. She was convicted of attempted theft and theft on more than one occasion.
In June 2009 the Registered Counselor Program denied the application of Sean D. Vandommelen (RC60042101). He was convicted of escape, possession of heroin, forgery, theft, and possession of stolen property.
Spokane County
In June 2009 the Nursing Assistant Program charged certified nursing assistant Jessie N. Bailey (NC10092611) with unprofessional conduct. She allegedly used oxycontin without a proper prescription.
In June 2009 the Nursing Commission released registered nurse Marian Jean Fisher (RN00055830) from terms and conditions set against her license.
In June 2009 the Health Care Assistant Program charged Andrew L. Mourning (HC00154439) with unprofessional conduct. He was convicted of assault and two counts of assault domestic violence.
In June 2009 the Nursing Commission charged registered nurse Carlene V. Shopbell (RN00137007) with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her license.
In June 2009 the Nursing Commission terminated the order of probation against licensed practical nurse Kesne Winton (LP00045250).
California: In June 2009 the Massage Therapy Program reinstated the license of Thomas Luke Benson (MA00004400).
Idaho: In June 2009 the Medical Commission released physician Eugene M. Baldeck (MD00024177) from terms and conditions set against his license. Dr. Baldecks license is unrestricted.
Illinois: In June 2009 the Veterinary Board of Governors modified the agreed order and placed the license of Scott E. McDonald (VT00004351) on probation for two years. He must comply with terms and conditions set against his license.
Louisiana: In June 2009 the Nursing Commission charged registered nurse Mary G. Faucheaux (RN00152333) with unprofessional conduct. Her licenses to practice as a registered nurse in Louisiana and California were placed on probation.
New Hampshire: In June 2009 the Radiological Technologist Program released Stephen Michael Tatasciore (RT60034954) from terms and conditions set against his certification.
Oregon: In May 2009 the Registered Counselor Program denied the application of Joseph T. Pimentel (RC00010506). His registration to practice as a registered counselor was revoked in 1990 and he failed to disclose the revocation on his application.
Oregon: In June 2009 the Board of Pharmacy charged pharmacy technician Josh A. Waggoner (VA00062531) with unprofessional conduct. He was convicted of theft and possession of hydrocodone and allegedly failed to respond to department inquiries about the conviction.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
