State disciplines health care providers
OLYMPIA - The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In September 2009 the Dental Commission released dentist Robert B. Smith (DE00007099) from terms and conditions set against his license.
Clark County
In September 2009 the Dental Commission granted the application of dental assistant Kortney S. Rund (D160054962) and placed conditions against her license. She was convicted of false verification for welfare, theft — welfare fraud, theft, and forgery on more than one occasion. She must comply with terms and conditions set against her license.
In September 2009 the Dental Commission entered into an agreed order with dentist Steven J. Sorensen (DE00004256). While placing temporary crowns on a patient, he provided substandard treatment because there were open margins on the crowns, and one of the tooth’s post and core contained residual caries and wasn’t cemented. He placed resin crowns instead of the requested permanent porcelain-fused-to-metal crowns, and failed to indicate if he received patient consent to make the change. He must comply with terms and conditions set against his license.
In September 2009 the Dental Commission reinstated the license of dentist Paul F. Genung
(DE00003370) and placed his license on probation for two years. His practice is restricted and he must comply with terms and conditions set against his license.
In September 2009 the Dental Commission denied the application of dental assistant Rebecca J. Ish (D160050817). She was convicted of theft on more than one occasion, and filing a false claim or proof.
In September 2009 the Nursing Commission released registered nurse Karen L. Jenkins (RN00086074) from terms and conditions set against her license.
In September 2009 the Dental Commission charged dentist Robert M. Keenan (DE00003579) with unprofessional conduct. The care he provided was allegedly below the standard while treating a patient for substantial periodontal bone loss, and allowed an unlicensed oral surgeon to perform dental surgery.
In September 2009 the Licensed Mental Health Counselor Program released Gary R. Sarozek (LH00007235) from terms and conditions set against his license.
In September 2009 the Medical Commission granted the application of physician Shine Sun (MD60077186) and placed conditions against his license. He was convicted of petty theft on two occasions, and failed to disclose the convictions on his application. He must comply with terms and conditions set against his license.
Kitsap County
In September 2009 the Registered Counselor Program granted the application of Krissy Merrion Murphy (RC60101526) and placed her registration on probation for two years. She was convicted of driving under the influence, rendering criminal assistance, and theft. She must comply with terms and conditions set against her registration.
Mason County
In September 2009 the Dental Commission granted the application of dental assistant Melanie D. Enger (D160050041) and placed her license on probation for 18 months. She was convicted of driving under the influence, entered into deferred prosecution on another count of driving under the influence, and admitted to using marijuana. She must comply with terms and conditions set against her license.
Pierce County
In September 2009 the Nursing Assistant Program denied the application of Susan C. Jansen (NA60063410). She was convicted of shoplifting.
In September 2009 the Massage Therapy Program granted the application of Lisa Ann Kingsley (MA60101493) and placed her license on probation for three years. She entered into a deferred prosecution for driving under the influence. She must comply with terms and conditions set against her license.
In September 2009 the Chemical Dependency Professional Program granted the application of Leah S. Niccolocci (CP60097351) and placed her license on probation for two years. Her license to practice as a registered counselor was placed on probation. She must comply with terms and conditions set against her license.
In September 2009 the Nursing Assistant Program denied the application of Lorilyn M. Schrader (NA60098015). Her license to practice as a licensed practical nurse was indefinitely suspended and she failed to disclose the suspension on her application.
Snohomish County
In September 2009 the Dental Commission entered into an agreed order with dentist Russell A. Clark (DE00006917). The care he provided was below the standard while treating a patient, and extracting the patient’s tooth. He’s restricted from performing extractions and must comply with terms and conditions set against his license.
In September 2009 the Nursing Commission terminated the order of probation against licensed practical nurse Herco C. Ordona (LP00049311).
Spokane County
In September 2009 the Board of Pharmacy placed the license of pharmacy technician Jill M. Horstman (VA00048105) on probation for five years. She tested positive for barbiturates during an employer requested urinalysis. She must comply with terms and conditions set against her license.
In August 2009 the Nursing Commission denied the application of licensed practical nurse Heather Ann Jackson (LP60080594). The Idaho State Board of Nursing denied her application for a practical nurse license, and she failed to disclose the denial on her application.
In September 2009 the Nursing Assistant Program granted the application of Patricia Jo Terry (NA60093861) and placed her registration on probation for one year. Her license to practice as a registered nurse was suspended. She must comply with terms and conditions set against her registration.
In September 2009 the Veterinary Board of Governors denied the application of veterinary technician Robin Lynn Wieck (AT00002679). She misrepresented herself as a licensed veterinary technician, and provided a false document to gain employment while her application was pending with the department. She can’t reapply for 60 months.Thurston County
In September 2009 the Nursing Assistant Program granted the application of David Patrick Bolin (NA60080683) and placed conditions against his registration. He was convicted of malicious mischief, and violation of a protection order. He must comply with terms and conditions set against his registration.
In September 2009 the Registered Counselor and Chemical Dependency Professional Programs reprimanded Karen J. Manners (RC00036529, CP00004600). Her license to practice as a registered nurse was indefinitely suspended.
In September 2009 the Board of Physical Therapy denied the application of physical therapy assistant Bret A. Streeter (P160033462). His license to practice as a physical therapy assistant in Oregon was revoked. He can’t reapply for three years.
Out of State
California: In August 2009 the Medical Commission denied the application of physician Carlos Warter (MD60056461). The Florida Board of Medicine denied his application to practice as a physician, and he failed to disclose the denial on his application.
Oregon: In September 2009 the Board of Physical Therapy granted the application of physical therapy assistant Robert V. Lyons (P160088358) and placed his license on probation for one year. He was convicted of theft, and must comply with terms and conditions set against his license.
Oregon: In September 2009 the Nursing Commission terminated the order of probation against registered nurse Teresa M. O’Leary (RN00057658).
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
