State disciplines health care providers
OLYMPIA - The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

In September
2009 the Nursing Assistant Program charged Donald J. Zeller (NA00197560) with unprofessional conduct. He
was convicted of third degree child rape, one count of dealing in depictions of
minor engaged in sexually explicit conduct, and two counts of possession of
depictions of minor engaged in sexually explicit conduct.
Douglas County
In September 2009 the
Nursing Assistant Program charged John A. McAlexander
(NA00199566) with unprofessional conduct. He allegedly failed to respond to
department inquiries about an allegation he diverted medications from a patient.
Island County
In September
2009 the Massage Therapy Program terminated the order of probation against
Anastasia M. Brencick (MA00009346).
King County
In September 2009 the
Dental Commission released dentist Hilal Khan
Ahmad (DE00005778) from terms and conditions set against his license.
In September 2009 the Nursing Commission reinstated the license of licensed
practical nurse Elzbieta E. Archibald (LP00040513) and placed
her license on probation. She must comply with terms and conditions set against
her license.
In September 2009 the Dental Commission charged dentist Duane Jones
(DE00006522) with unprofessional conduct. The orthodontic treatment he provided
to a patient fell below the standard of care.
In September 2009 the
Health Care Assistant Program released Sally H. Lozano
(HC00141564) from terms and conditions set against her license.
In
September 2009 the Nursing Commission charged registered nurse Kristi D. Richardson
(RN00089469) with unprofessional conduct. She allegedly withdrew controlled
substances for patients, and failed to document administration or wastage of the
medications.
In September 2009 the Nursing Commission reinstated the
license of licensed practical nurse Tsitsi E. Smith (LP00030190).
Kitsap County
In September 2009 the Board
of Physical Therapy reinstated the license of physical therapist Kelly B. Campo (PT00006092).
In September 2009
the Nursing Commission charged registered nurse Suzette M. Zehrung
(RN00146791) with unprofessional conduct. She allegedly failed to document
administration or wastage of hydrocodone for three patients.
Lewis County
In September 2009 the Nursing
Commission granted the application of registered nurse Tera J. Pilon (RN60093289) and put conditions on her
license. She was convicted of reckless driving, and driving under the influence.
She must comply with terms and conditions on her license.
Lincoln County
In September 2009 the
Nursing Assistant Program charged certified nursing assistant Olivia D. Foley (NC10090844) with unprofessional conduct. She
allegedly took legend drugs for her own use and was convicted of unlawful
possession of a controlled substance other than marijuana, unlawful possession
of a legend drug, and theft.
Mason County
In September 2009 the Veterinary Board of Governors terminated the order of
probation against veterinary technician Jeananne Gallent (AT00000797).
Pierce County
In September 2009 the
Nursing Assistant Program charged certified nursing assistant Tiffany M.
Quimby (NC10093963) with unprofessional conduct. She was convicted of
identity theft, two counts of forgery, and two counts of theft.
Snohomish County
In September 2009 the
Health Care Assistant Program granted the application of Kelly Renee De La Cruz (HC60042579) and placed her license on
probation for two years. She was convicted of identity theft, forgery, and
theft. She must comply with terms and conditions set against her license.
In September 2009 the Nursing Commission reinstated the license of licensed
practical nurse
Mimi O. Etherington (LP00048872).
In August 2009 the Nursing
Assistant Program denied the application of Nola Harb (NA60072421). She was
convicted of possession of a controlled substance.
In September 2009 the
Nursing Assistant Program charged Chiedu C. Okeke (NA00190284) with
unprofessional conduct. She was convicted of attempted forgery, attempted
possession of stolen property, and theft.
Spokane County
In September 2009 the Nursing Assistant
Program denied the application of certified nursing assistant Brandon M. Affre (NC60001188) for certification as a nursing assistant.
His registration to practice as a nursing assistant was suspended.
In
September 2009 the Nursing Assistant Program denied the application of Kyle D. Aiken
(NA60073670). He was convicted of child molestation, and theft.
In
September 2009 the Board of Pharmacy terminated the order of probation against
pharmacy assistant Xiang Y. Jing (VB00059137).
In September 2009 the Board
of Pharmacy released pharmacy technician Tina K. Kelley (VA00020327) from
terms and conditions set against her license.
In September 2009 the
Nursing Commission charged registered nurse Larissa S. Mack
(RN00145423) with unprofessional conduct. She allegedly failed to comply with
terms and conditions set against her license.
In September 2009 the
Nursing Assistant Program charged Stacy S. Sale (NA00109667) with unprofessional conduct.
Her pharmacy technician license was indefinitely suspended, and she allegedly
failed to disclose the suspension on her nursing assistant reactivation
application.
Thurston County
In September 2009 the Nursing Assistant Program charged Chrystal D. Chipman (NA00063833) with unprofessional
conduct. She allegedly took a food stamp debit card from a patient for her own
use.
In September 2009 the Health Care Assistant Program granted the
application of Roberto Ramirez (HC60101255) and placed conditions on his
license. He was convicted of driving under the influence — alcohol. He must
comply with terms and conditions set against his license.
Walla Walla County
In September 2009
the Nursing Commission charged registered nurse Andrea L.
Dressler (RN00128337) with unprofessional conduct. She was convicted of
six counts of obtaining a controlled substance by fraud, three counts of
forgery, and possession of a controlled substance — hydrocodone.
Out of State
Arizona:
In September 2009 the Nursing Commission
reinstated the license of registered nurse Shane G. Roberts (RN00153880).
Note to Editors:
Health care providers charged with unprofessional conduct have 20 days to
respond to the Department of Health in writing. The case then enters the
settlement process. If no disciplinary agreement can be reached, the case will
go to a hearing.
