State disciplines health care providers
OLYMPIA - The Washington State Department of Health has taken
disciplinary actions or withdrawn charges against health care providers in our
state.
The department’s Health Systems Quality Assurance Office works with boards,
commissions, and advisory committees to set licensing standards for more than 70
health care professions (e.g., medical doctors, nurses, counselors).
Information about health care providers is on the agency Web site. Click on
“Provider Credential Search” on the left hand side of the
Department of Health home page (www.doh.wa.gov).
The site includes information about a health care provider’s license status, the
expiration and renewal date of their credential, disciplinary actions and copies
of legal documents issued after July 1998. This information is also available by
calling 360-236-4700. Consumers who think a health care provider acted
unprofessionally are also encouraged to call and report their complaint.

In October 2009 the Health
Care Assistant Program charged Sylvia E. Woods (HC00142809) with unprofessional
conduct. She was convicted of perjury.
Clark County
In October 2009 the Nursing Assistant Program charged Bradley K. Devall
(NA00190633) with unprofessional conduct. He was convicted of assault.
Jefferson County
In October 2009 the Registered
Counselor Program granted the application of Kimberly Annjanette Carroll
(RC60101152) and placed her registration on probation for three years. She was
convicted of theft on more than one occasion, unlawful possession of a
controlled substance, attempted taking a motor vehicle without permission,
possession of stolen property, trafficking of stolen property on more than one
occasion, and driving under the influence. She must comply with terms and
conditions set against her registration.
King County
In October 2009 the Chemical Dependency Professional Program granted the
application of George F. Brummell (CP60104516) and placed his license on
probation for three years. He received a counselor registration in May 2009 with
an agreement to practice with conditions. He must comply with terms and
conditions set against his chemical dependency professional credential.
In September 2009 the Board of Osteopathic Medicine and Surgery reinstated the
license of Lawrence J. Greenblatt (OP00000791).
In October 2009 the
Nursing Assistant Program terminated the order of probation against certified
nursing assistant Margaret S. Kadzomba (NA00167158, NC60005605).
In
October 2009 the license of physician Geoffrey A. MacPherson (MD00011292) was
placed on probation for three years. He had sexual contact with two mothers of
his patients. He must comply with terms and conditions set against his license.
In October 2009 the Nursing Assistant Program denied the credential
reactivation request of certified nursing assistant Molly J. Marshall
(NC10069008). She was convicted of attempted theft and theft on more than one
occasion.
In October 2009 the Nursing Commission reinstated the license
of registered nurse Kelli K. Phillips (RN00136506) and placed her license on
probation for one year. She must comply with terms and conditions set against
her license.
In September 2009 the Counseling Program granted the
application of agency affiliated counselor Mindy Rae Rushing (CG60110998) and
placed her license on probation for one year. She was convicted of driving under
the influence, and must comply with terms and conditions set against her
license.
In October 2009 the Licensed Mental Health Counselor Program
charged David E. Weed (LH00008760) with unprofessional conduct. He allegedly
entered into a sexual relationship with a patient.
Pierce County
In September 2009 the Nursing Assistant Program denied
the application of Regina Carita Bryant (NA60069871). She surrendered her
credential to practice as a certified nurse aide in Alaska and failed to
disclose the surrender on her application.
In October 2009 the Chemical
Dependency Professional Program granted the application of Kimberly Elizabeth
Sparks (CP60097366) and placed conditions against her license. Her registration
to practice as a counselor was placed on probation. She must comply with terms
and conditions set against her license.
In October 2009 the Nursing
Assistant and Health Care Assistant Program charged certified nursing assistant
Beatriz S. Taday (NC10032206, HC00133565) with unprofessional conduct. She was
convicted of two counts of identity theft, and one count of theft.
In October
2009 the Nursing Assistant Program granted the application of certified nursing
assistant Ramsie L. Young (NC60104351) and placed conditions against her
certification. She was convicted of delivery of a controlled substance. She must
comply with terms and conditions set against her certification.
Skagit County
In October 2009 the Registered
Counselor Program granted the application of Diane L. Sheehan (RC60101026) and
placed her registration on probation for one year. She was convicted of driving
under the influence, and must comply with terms and conditions set against her
registration.
Snohomish County
In
September 2009 the Chiropractic Commission charged chiropractor Michael T.
Breneman (CH00001293) with unprofessional conduct. He was convicted of
conspiracy to distribute and possession with intent to distribute 1,000
kilograms or more of marijuana, and conspiracy to commit money laundering.
In September 2009 the Nursing Commission charged registered nurse Sharon J.
Stanley (RN00106813) with unprofessional conduct. She allegedly failed to comply
with previous terms and conditions set against her license.
Spokane County
In October 2009 the Board of
Pharmacy reprimanded Northwest Health System (CF00058494) and placed its
pharmacy license on probation for 60 months. Northwest Health System began a
program to distribute samples to patients. The board sent a letter to the firm
with a notification that it could no longer continue the program. The pharmacy
disregarded the letter and continued distributing. It must comply with terms and
conditions set against its license.
In September 2009 the Nursing
Assistant Program charged Bonnie L. Olsen (NA00092911) with unprofessional
conduct. She was convicted of theft, and five counts of Medicaid false
statement.
In September 2009 the Nursing Assistant Program denied the
application of certified nursing assistant Stacy S. Sale (NC60026054). Her
license to practice as a pharmacy technician was indefinitely suspended and she
failed to disclose the suspension on her application.
In October 2009 the
Nursing Assistant Program granted the application of Evelina Kay Vaughn
(NA60028154) and placed her registration on probation for two years. She was
convicted of theft, and must comply with terms and conditions set against her
registration.
In September 2009 the Nursing Commission released licensed
practical nurse Glory Yahrling (LP00045085) from terms and conditions set
against her license.
Thurston County
In
September 2009 the Dispensing Optician Program denied the application of Stacy
Halliday (DO60031925). She practiced as a dispensing optician without a license.
She’s also known as Stacy Christian.
In October 2009 the Nursing
Commission reinstated the license of registered nurse Elena M. Hunt (RN00133562)
and placed her license on probation for two years. She must comply with terms
and conditions set against her license.
Whatcom
County
In October 2009 the Office of Emergency Medical Services and
Trauma System Program released paramedic George M. St. James (ES00131260) from
terms and conditions set against his license.
In October 2009 the Nursing
Assistant Program terminated the order of probation against Amy Swenson
(NA00134972).
Yakima County
In September
2009 the Registered Counselor Program denied the application of Brian Lee
McCloud (RC60094511). He was convicted of attempted assault, and two counts of
assault.
Out of State
Missouri:
In October 2009 the Nursing Assistant Program reinstated the certification of
certified nursing assistant Latrasye C. Boldien (NC10040808).

Health care providers charged with unprofessional conduct have 20 days to
respond to the Department of Health in writing. The case then enters the
settlement process. If no disciplinary agreement can be reached, the case will
go to a hearing.
