The bathroom toilet seat cover was bolted only on one side allowing the seat cover to shift and slide.

The bathroom toilet seat cover was bolted only on one side allowing the seat cover to shift and slide. The toilet bowl cover was cracked and not the right size for the tank.  A gap appeared exposing the inside of the tank as the cover was too small.
