State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA - The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Adams County

In December of 2009 the Board of Pharmacy indefinitely suspended the pharmacy assistant and pharmacy technician credentials of Rosa G. Sandoval (PHAA.VB. 00064081, PTEC.VA.60065206). Sandoval stole hydrocodone pills from the pharmacy where she worked and illegally possessed hydrocodone- acetaminophen tablets.

Benton County

In November of 2009 the Department of Health withdrew a statement of charges against the licensed social worker credential of Melinda Johnson (SWI.LW.00005353).

Chelan County

In December of 2009 the Department of Health indefinitely suspended the registered nursing assistant credential of John A. McAlexander (NAR.NA.00199566). McAlexander failed to respond to an inquiry about allegations that he diverted medication from a patient.

Clark County

In December of 2009 the Department of Health indefinitely suspended the health care assistant credential of Shirley E. Combs (HCA.HC.00146247) because she failed to comply with terms of an earlier order.

In December of 2009 the Board of Pharmacy indefinitely suspended the pharmacy assistant credential of Kenneth C. Fahey (PHAA.VB.00071055). Fahey diverted Darvocet-N, a controlled substance, from his workplace and was caught delivering some of it to a high school student on school premises. He was convicted of delivery of a controlled substance

King County

In October of 2009 the Medical Quality Assurance Commission entered into an agreed order with physician Elizabeth M. Bennett (MD00033971) under which a summary suspension order is terminated and restrictions on her ability to practice are put in place for at least five years. Bennett has a history of substance abuse, treatment, monitoring and relapse dating to 1997.

In November of 2009 the Examining Board of Psychology dismissed charges against psychologist Kim E. Kendall (PY00001379) and exonerated her.

In December of 2009 the Dental Quality Assurance Commission withdrew a statement of charges against dentist Douglas A. Seymour (DENT.DE.00006613).

Kitsap County

In November of 2009 the Massage Practitioner Program indefinitely suspended the massage practitioner credential of Chance C. Shurwin (MASS.MA.60043057), who failed to comply with terms of an earlier agreement. 

In December of 2009 the Nursing Care Quality Assurance Commission suspended the licensed practical nursing credential of Shaundell L. Wascher (LPN.LP.00057794), who failed to comply with terms of an earlier stipulation.

Pierce County

In November of 2009 the Department of Health indefinitely suspended the X-ray technician credential of Heather R. Weilert (XRAY.XT.00005491), who failed to comply with terms of an earlier stipulation.

In November of 2009 the Registered Counselor Program indefinitely suspended the registered counselor credential of James L. McClure (RC.RC.00004338), who failed to comply with terms of an earlier order.

In December of 2009 the Department of Health suspended for at least seven years the certified nursing assistant credential of Tiffany M. Quimby (NAC.NC.10093963). She was convicted in Pierce County Superior Court of one count of first-degree identity theft, a Class B felony; two counts of forgery, a Class C felony; and two counts of second-degree theft, a Class C felony.

In December of 2009 the Nursing Care Quality Assurance Commission suspended for at least five years the registered nursing credential of Kent R. Dillard (RN.RN.00131114). Dillard was convicted in Pierce County Superior Court of first-degree assault, a Class A felony.

In December of 2009 the Board of Pharmacy entered into an agreed order with pharmacist Jennifer M. Gray (PHRM.PH.00019102) under which her credential is suspended for at least a year. Gray stole and used controlled substances and legend drugs from the pharmacy where she worked, and continued to work as a pharmacist while addicted to and abusing drugs.

Snohomish County

In November of 2009 the Board of Pharmacy indefinitely suspended the pharmacy technician credential of Xoa T. Oaks (PTEC.VA.00039372), who admitted forging prescriptions to obtain drugs for her own use.

Spokane County

In November of 2009 the Department of Health withdrew a statement of charges against the nursing assistant credential of Raymond P. Juidici (NAC.NC.10061462).

In November of 2009 the Department of Health suspended for at least three years the nursing assistant registration of Bonnie L. Olsen (NAR.NA.00092911). Olson was convicted in Spokane County Superior Court of one count of first-degree theft, a Class B felony, and five counts of Medicaid false statements, class C felonies.

Thurston County

In November of 2009 the Department of Health indefinitely suspended the registered counselor credential of Maria Elaine Reynolds (RC.RC.60048121), who failed to comply with terms of an earlier agreement.

Walla Walla County

In November of 2009 the Department of Health withdrew a statement of charges against the health care assistant credential of Aimee R. Lewis (HCA.HC.00155608).

Whatcom County

In November of 2009 the Nursing Care Quality Assurance Commission indefinitely suspended the registered nursing credential of Pamela Thornton (RN.RN.00165307) after the Texas Board of Nurse Examiners revoked her credentials to practice in that state.

In December of 2009 the Chiropractic Quality Assurance Commission entered into an agreed order with chiropractor Harry Charles Woodfield III (CHIR.CH.00034341) under which his credential is indefinitely suspended. On his Washington license application, Woodfield answered questions inaccurately and failed to disclose his North Carolina pharmacist license had been suspended because of drug diversion.

Yakima County

In December of 2009 the Department of Health suspended for at least two years the certified nursing assistant credential of Kathy J. Case (NAC.NC.10060442). Case took $40 from a patient at a long-term care facility where she worked.

Out of State

 Alaska: In October of 2009 the Nursing Care Quality Assurance Commission indefinitely suspended the registered nursing credential of Phyllis A. Inman (RN.RN.00106959) after the North Carolina Board of Nursing revoked her privilege to practice in that state. 

Oregon: In October of 2009 the Department of Health entered into an agreed order on modification under which the registered nursing assistant credential of Sean P. Walsh (NAR.NA.60070797) is indefinitely suspended.

In December of 2009 the Nursing Care Quality Assurance Commission summarily suspended the registered nursing credential of Fawn M. Hasty (RN.RN.00146664) after the Oregon State Board of Nursing revoked her credential to practice in that state.
