State disciplines health care providers

OLYMPIA - The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In December 2009 the Nursing Assistant Program reinstated the registration of Sabrie L. Tramble (NA00197331) and placed her registration on probation for two years. She must comply with terms and conditions set against her registration.

Columbia County

In December 2009 the Emergency Medical Services and Trauma Systems Program granted the application of emergency medical technician Yancy T. Yost (ES60119282) and placed his credential on probation for one year. He was convicted of negligent driving on more than one occasion, possession of drug paraphernalia, harassment, assault, and possession with intent to manufacture or deliver — marijuana. He must comply with terms and conditions set against his certification.

Grant County 

In January 2010 the Nursing Assistant Program granted the applications of certified nursing assistant Jennifer R. Owens (NA60097975, NC60102017) and placed conditions against her registration and certification. She was convicted of possession of methamphetamine, and theft. She must comply with the terms and conditions set against her registration and certification

In December 2009 the Health Care Assistant Program granted the application of Julia Antoinette Stephens (HC60118473) and placed her license on probation for two years. She was convicted of violation of the uniform controlled substance act, conspiracy to commit manufacturing of methamphetamine. She must comply with terms and conditions set against her license.

Grays Harbor County

In December 2009 the Medical Commission charged physician Jeffrey Wong (MD00041288) with unprofessional conduct. He allegedly had aberrant and hostile behavior towards hospital administration, and showed poor conduct and behavior towards staff, and certain patient care situations at the hospital.

King County

In December 2009 the Acupuncturist Program charged Kunhye P. Golec (AC00002648) with unprofessional conduct. She allegedly entered into a sexual relationship with a patient, and charged a patient’s insurance for services not rendered.

In December 2009 the Registered Counselor Program reinstated the registration of Natalie N. Gomez (RC00052638) and placed her registration on probation for three years. She must comply with terms and conditions set against her license.

In December 2009 the Nursing Assistant Program denied the application of Crystal Dawn Moniz (NA60094695). She was convicted of unlawful possession of a controlled substance.

In December 2009 the Registered Counselor Program terminated the order of probation against Antony L. Xczar (RC00059839). He is also known as Antony L. Carr.

Pierce County

In December 2009 the Emergency Medical Services and Trauma Systems Program granted the application of emergency medical technician Sarah Anne Baumgardner (ES60117451) and placed her certification on probation for two years. She entered into an agreed order of continuance without findings for charges on two counts of assault, and one count of malicious mischief. She must comply with terms and conditions set against her license.

In December 2009 the Nursing Assistant Program charged Monica D. Boggs (NA00155145) with unprofessional conduct. She was convicted of shoplifting on more than one occasion.

In December 2009 the Nursing Commission entered into an agreed order with licensed practical nurse Sheila C. Cruse (LP00032550). She failed to administer, document administration, or document wastage of oxycodone on multiple occasions. She must comply with terms and conditions set against her license. 

In December 2009 the Nursing Assistant Program placed the certification of certified nursing assistant Ashleigh M. Futrell (NC10099613) on probation for two years. She transferred a patient by herself when the patient required a two-person transfer. This resulted in bruising to the patient’s arm. She must comply with terms and conditions set against her certification. 

In December 2009 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Gary R. Probst (CO60107571) and placed his certification on probation for three years. He was convicted of theft and must comply with terms and conditions set against his certification. 

In January 2010 the Dental Commission granted the application of dental assistant Renee M. Purcell (D160121702) and placed conditions against her registration. She was convicted of driving under the influence, and failed to disclose the conviction on her application. She must comply with terms and conditions set against her registration.

Snohomish County

In January 2010 the Nursing Assistant Program granted the application of certified nursing assistant Shantel A. Jenks (NC60054905) and placed conditions against her certification. She was convicted of assault, and minor in possession of alcohol. She must comply with terms and conditions set against her certification. 

Spokane County

In December 2009 the Nursing Assistant Program charged Brianna Justice (NA00187876) with unprofessional conduct. She allegedly stole more than $400 from a patient by using their debit card to withdraw cash without permission. She was charged with theft, and identity theft.

Thurston County

In December 2009 the Registered Counselor Program reinstated the registration of Michael E. Dassow (RC00055441).

In December 2009 the Nursing Assistant Program granted the application of Haviela S. Patino (NA60120450) and placed her registration on probation for one year. She was convicted of assault and must comply with terms and conditions set against her registration.

Whatcom County 

In December 2009 the Registered Counselor and Chemical Dependency Professional Programs terminated the order of probation against Margaret M. Clark (RC00026541, CP00003531).

Yakima County

In December 2009 the Licensed Mental Health Counselor Program released mental health counselor Pia P. Hull (LH00007865) from terms and conditions set against her license.

In December 2009 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Russell Gene Taylor (CO60116194) and placed his certification on probation for two years. He was convicted of driving under the influence on more than one occasion. He must comply with terms and conditions set against his certification.

Out of State

Oregon: In December 2009 the Chemical Dependency Professional Program granted the application of chemical dependency professional Renee Kayhan Ulrich (CP60116496) and placed conditions against her license. She was convicted of two counts of supplying contraband, MIVP, possession of a controlled substance —schedule 1, false information to police, and prostitution. She must comply with terms and conditions set against her license.

Texas: In December 2009 the Nursing Commission reinstated the license of registered nurse Joseph E. Temko (RN00073948) and placed his license on probation. He must comply with terms and conditions set against his license.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
