State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency Web site. Click on "Provider Credential Search" on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In March 2010 the Nursing Assistant Program charged Diane E. McCormick (NA00180422) with unprofessional conduct. She was convicted of two counts of assault.

Clark County

In March 2010 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Sarah Nicole Denis (CO60126879) and placed her certification on probation for two years. Terms and conditions were placed against her registration to practice as a counselor. She must comply with terms and conditions set against her certification.

In March 2010 the Nursing Commission entered into an agreed order with licensed practical nurse Lana M. Leveque (LP00044588). She failed to comply with terms and conditions set against her license. She must comply with new terms and conditions set against her license. 

In April 2010 the Nursing Commission charged registered nurse Fairuza M. Stevenson (RN00139022) with unprofessional conduct. She allegedly believed medication prescribed by a physician was inappropriate and failed to administer the medication to the patient.

In March 2010 the Nursing Assistant Program granted the application of certified nursing assistant Julia Brookes Willich (NC60133300) and placed her certification on probation for two years. She was convicted of assault on more than one occasion, and disorderly conduct. She must comply with terms and conditions set against her certification.

Cowlitz County 

In March 2010 the Veterinary Board of Governors reinstated the license of veterinarian Jim C. Farrish (VT00001397).

Grays Harbor County

In March 2010 the Dental Commission released dentist Richard Alfred Pedersen (DE00005133) from terms and conditions set against his license in a 2005 order. He must continue to comply with terms and conditions set against his license in a 2008 agreed order.

Jefferson County

In March 2010 the Massage Program terminated the order of probation against Petra Murdoch (MA00019198).

King County

In March 2010 the Dental Commission terminated the order of probation against dentist Paul L. Alota (DE00007365). 

In March 2010 the Surgical Technologist Program terminated the order of probation against Devon N. Bauer (ST60027002).

In March 2010 the Health Care Assistant Program granted the application of Edrie Roy Caminong (HC60123496) and placed his license on probation for three years. He was convicted of assault on more than one occasion, possession of stolen property, possession of a controlled substance, and two counts of forgery. He must comply with terms and conditions set against his license.

In March 2010 the Dental Commission terminated the order of probation against dental assistant Trishia Michelle Henson (D160039082).

In March 2010 the Dental Commission released dentist Wonzel M. Mobley (DE00004032) from terms and conditions set against his license.

In March 2010 the Nursing Assistant Program charged Joyce W. Ngigi (NA60062245) with unprofessional conduct. She allegedly provided a false social security number on her application, and failed to respond to the Department of Health regarding the allegation.

In March 2010 the Veterinary Board of Governors charged veterinarian Keith D. Norton (VT00003742) with unprofessional conduct. Allegations include failing to maintain adequate patient records. The Department of Health also performed an inspection of Dr. Norton’s facility and allegedly found unsanitary conditions, and kenneling areas that didn’t provide the patients with comfort and safety.

In March 2010 the Dental Commission released dentist Douglas A. Seymour (DE00006613) from terms and conditions set against his license.

In February 2010 the Health Care Assistant Program denied the application of Diana Marie Ticer (HC60117016). She was convicted of two counts of drive by shooting.

Okanogan County 

In April 2010 the Nursing Assistant Program granted the application of Nicole R. Pargett (NA60058437) and placed her registration on probation for two years. She was convicted of possession of a controlled substance — methamphetamine, obstructing a law enforcement officer, assault on more than one occasion, and theft. She must comply with terms and conditions set against her registration.

Pierce County 

In March 2010 the Dental Commission terminated the order of probation against dentist Richard B. Ferguson, Jr. (DE00006345). He must continue to comply with terms and conditions set against him in a 2010 stipulation to informal disposition. 

In March 2010 the Nursing Assistant Program charged certified nursing assistant Shamon A. Williams (NC10066133) with unprofessional conduct. She allegedly failed to follow a patient’s care plan by attempting to transfer the patient without a Hoyer lift resulting in the patient falling. 

Skagit County

In March 2010 the Nursing Commission terminated the order of probation against registered nurse and advanced registered nurse practitioner Robyn Margot Choffel (RN00103955, AP30003913). 

Snohomish County

In March 2010 the Nursing Assistant Program charged Ebriama Jammeh (NA60036676) with unprofessional conduct. He was convicted of assault.

In March 2010 the Nursing Commission entered into an agreed order with registered nurse Sharon J. Stanley (RN00106813). She failed to comply with previous terms and conditions set against her license. She must comply with new terms and conditions set against her license.

Spokane County

In March 2010 the Nursing Assistant Program granted the applications of certified nursing assistant Miranda Ruth Wolfer (NA60104836, NC60128270) and placed her registration and certification on probation for two years. She entered into a diversion program for a charge of trafficking in stolen property. She must comply with terms and conditions set against her credentials.

Thurston County 

In March 2010 the Nursing Commission reinstated the license of licensed practical nurse Laura M. Dupont (LP00037834), also known as Laura M. Bozzo, and placed her license on probation. She must comply with terms and conditions set against her license. 

In April 2010 the Massage Program denied the application of Roger D. Maidens (MA60050032). He failed to properly drape patients, exposed their breasts and rubbed on or near their breasts and vagina. 

Out of State

Florida: In March 2010 the Medical Commission charged physician Kwame Asante-Nkansa (MD60019472) with unprofessional conduct. Allegedly his clinical privileges at Winn Army Community Hospital were revoked. He also allegedly failed to disclose this action on his application.

South Carolina: In February 2010 the Medical Commission denied the application of physician Ishmeal Major (MD60065634). He relinquished his license to practice medicine in Florida, and his application to practice medicine in North Carolina was denied.

Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
