State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA - The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions, and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Asotin County
In July 2010 the Nursing Assistant Program suspended the credential of Sarah L. Broner (NAR.NA.00153640) for at least five years. Broner diverted about 20 tables of Hydrocodone and Hydromorphone from three patients. She was also convicted of three counts of attempting to obtain a controlled substance through fraud, and one count of possession of a controlled substance in 2009.

Benton County
In July 2010 the Medical Commission withdrew the March 2010 statement of charges against physician Sophia S. Chang (MD00036177).

King County
In July 2010 the Nursing Assistant Program suspended the credential of certified nursing assistant Faten Anwar (NAR.NA.00201685, NAC.NC.60013350). Anwar’s license to practice as a physician and surgeon in Washington was revoked in March 2010.
In July 2010 the Nursing Commission indefinitely suspended the license of licensed practical nurse Norma Jean Denbrook (LPN.LP.00038659). Denbrook failed to comply with a previous stipulation.

In July 2010 the Department of Health indefinitely suspended the credential of agency affiliated registered counselor Ebonee Heller (CAAR.CG.60121313). Heller failed to comply with terms of an earlier agreement.

In July 2010 the Department of Health indefinitely suspended the credential of massage practitioner LaTanya N. Moore (MASS.MA.60019056), who failed to comply with terms of an earlier agreement.

In July 2010 the Nursing Commission indefinitely suspended the license of registered nurse Erika I. Moorhouse (RN.RN.00118811). Moorhouse failed to adequately assess a patient after he fell; failed to notify the patient’s doctor, the director of nursing services, or the resident care manager of the fall; and failed to complete an incident report. The patient was later found to have broken his hip from the fall.

In June 2010 the Department of Health dismissed a statement of charges against health care assistant Amardeep Singh (HCA.HC.00150093).

Kitsap County
In June 2010 the Nursing Care Quality Assurance Commission indefinitely suspended the credential of licensed practical nurse Laurie McDougall (LPN.LP.00045509), who failed to comply with terms of an earlier order.

In June 2010 the Nursing Assistant Program entered into an agreed order with Veronica F. McInnis (NAR.NA.00196306) under which her registered nursing assistant credential is suspended for at least five years. McInnis was convicted in Bremerton Municipal Court of third-degree theft, a gross misdemeanor, and in Kitsap County Superior Court of second-degree theft, a class C felony.

Klickitat County
In July 2010 the Nursing Commission withdrew the October 2008 statement of charges against registered nurse Lynn E. Schneider (RN.RN.00175322).

Pierce County
In July 2010 the Radiologic Technologist Program suspended the credential of Tina L. Nystrom (RADT.RT.00007515). Nystrom was convicted of first degree assault with a deadly weapon/domestic violence in 2009 and the court found Nystrom had a mental defect caused by severe substance abuse addiction. She also failed to report the conviction to the department.

Snohomish County
In July 2010 the Chiropractic Commission granted the reactivation application and then immediately suspended the license of chiropractor Michael Breneman (CHIR.CH.00001293). If Breneman completes certain requirements, the suspension will be lifted and he will be placed on probation with terms and conditions. He was convicted in federal district court of conspiracy to distribute and possession with intent to distribute 1,000 kilograms or more of marijuana, and conspiracy to commit money laundering.

In July 2010 the Department of Health suspended the license of licensed practical nurse Levius Ian Davis (LPN.LP.00045901). Davis failed to comply with a child support order.

In June 2010 the Nursing Commission entered into an agreed order to suspend the license of registered nurse Stacy Lee Harmon (RN.RN.00117155). Harmon failed to comply with a previous stipulation.

In July 2010 the Department of Health suspended the credential of health care assistant Rebecca M. Sundsmo (HCA.HC.00151860). Sundsmo called in a prescription for herself using a false name and without physician authorization. She also used the physician’s DEA number without authorization. She also failed to respond to department inquiries about the incident.

Spokane County
In June 2010 the Nursing Assistant Program suspended the credential of certified nursing assistant Shyesta M. Kennedy (NAC.NC.10089743). Kennedy improperly and fraudulently used bank account information from a patient to pay personal bills and make other purchases totaling about $8,000.

In June 2010 the Nursing Assistant Program suspended the credential of certified nursing assistant Ashli E. Paladie (NAC.NC.10090219). Paladie was convicted of solicitation to commit possession of a controlled substance — heroin in 2009.

Thurston County
In July 2010 the Nursing Assistant Program indefinitely suspended the credential of Dean L. Jensen (NAR.NA.00198964). Jensen failed to comply with a previous stipulation.

In July 2010 the Health Care Assistant Program suspended the credential of Sally Castillo Yancey (HCA.HC.60016541). Yancey was charged with 10 counts of second degree identity theft in 2009. She entered into a drug court contract and the court entered order imposing sanctions for drug court program violations in 2009. She also admitted to a department investigator that she wrote prescriptions for narcotic drugs for herself for eight months while working as a health care assistant in 2007.Yakima County

In June of 2010 the Dental Quality Assurance Commission indefinitely suspended the credential of dental assistant Crystal G. Castaneda (DEAS.D1.60079847). Castaneda failed to comply with terms of an earlier agreement.

Out of State
Alaska: In July 2010 the Medical Commission summarily suspended the license of physician assistant-certified Jesse Thomas Fenger (PA10005297). Fenger’s license to practice as a physician assistant in Alaska was suspended for engaging in sexual misconduct; he remains prohibited from practicing in that state.

Arizona: In July 2010 the Medical Commission summarily suspended the license of physician Jefferson Lin (MD60020948). Lin’s application to practice medicine in North Carolina was denied because he failed to disclose that a complaint was made against him in Colorado while he was applying for a medical license in that state. The Colorado board issued a “stipulated” license, and then later withdrew the application at Lin’s request.

California: In June 2010 the Nursing Commission suspended the license of registered nurse Kevin J. McDonald (RN.RN.00174031). McDonald’s license to practice nursing in California was revoked due to allegations he diverted morphine for his own use and a conviction for child endangerment.

Michigan: In June 2010 the Unlicensed Practice Program ordered Karen Shaw to permanently cease and desist from any and all conduct constituting the practice of medicine, including but not limited to the application of any penetrating laser energy to the human body and the prescribing of medication to any person in the state of Washington unless Shaw first gets the appropriate credentials to do so. Shaw burned a patient while performing laser skin treatment.

Oregon: In July 2010 the Nursing Commission summarily suspended the license of licensed practical nurse Billie Faye Olson (LPN.LP.00042888). Olson’s license to practice nursing in Oregon was revoked; she’s prohibited from practicing in that state.

Oregon: In July 2010 the Nursing Commission indefinitely suspended the license of registered nurse Judy A. Woody (RN.RN.00132089). Woody surrendered her license to practice nursing in Oregon for failing to administer and document medications as ordered.
