State disciplines health care providers

OLYMPIA ¾ The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.

The department’s Health Systems Quality Assurance Office works with boards, commissions, and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency website. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In July 2010 the Chemical Dependency Professional Program charged Norma I. Mendoza (CP00000511) with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her license.

In July 2010 the Chemical Dependency Professional Program granted the application of chemical dependency professional trainee Simon S. Tedrow (CO60164436) and placed his certification on probation until April 2012. In February 2009 his credential to practice as a registered counselor was placed on probation. He must comply with terms and conditions set against his certification.

In July 2010 the Radiologic Technologist Program granted the application of Lesley Theresa Winston (RT60128340) and placed conditions against her certification. In 2003 she accepted a position as a nuclear medicine technologist but didn’t hold a radiologic technologist credential. She must comply with terms and conditions set against her certification.

Clark County

In July 2010 the Registered Counselor Program terminated the order of probation against Angela M. Walborn (RC60036502).

Jefferson County

In July 2010 the Office of Emergency Medical Services and Trauma Systems Program granted the application of emergency medical technician Jeffrey Bruce Fairbanks (ES60142479) and placed his license on probation for two years. He was convicted of solicitation to unlawfully possess a narcotic drug. He must comply with terms and conditions set against his license.

In July 2010 the Registered Counselor Program terminated the order of probation against Clarence William Wetherald, Sr. (RC60032063).

King County

In July 2010 the Registered Counselor Program terminated the order of probation against Meagan Marie Bowser (RC60001991).

In July 2010 the Medical Commission released physician Deborah S. Golob (MD00044606) from terms and conditions set against her license. Dr. Golob’s license is unrestricted in Washington.

In July 2010 the Nursing Assistant Program entered into an agreed order with certified nursing assistant Charles L. Njuguna (NA60075995, NC60059781). He was convicted of driving under the influence, and disorderly conduct — domestic violence. He must comply with terms and conditions set against his registration and certification.

Kitsap County

In July 2010 the Nursing Assistant Program charged Lucas James Bennet (NA60019420) with unprofessional conduct. He allegedly removed a fentanyl patch from a patient for his own personal use, and diverted fentanyl from another patient for his own personal use.

Kittitas County

In July 2010 the Secretary of Health charged licensed practical nurse Debi D. Guse (LP00050370) with unprofessional conduct. She allegedly began a personal relationship with a patient’s husband, lived with the husband after the patient died, and accepted money from the husband.

In July 2010 the Chiropractic Commission charged chiropractor Richard F. Renzetti (CH00001276) with unprofessional conduct. He allegedly continued to provide and bill patients for chiropractic treatments for several years while his credential was expired.

Pierce County

In July 2010 the Massage Program entered into an agreed order with Aisha N. Viera (MA60022044). She was convicted of assault of a child. She must comply with terms and conditions set against her license.

San Juan County

In July 2010 the Medical Commission charged physician Robert A. Wilson (MD00032057) with unprofessional conduct. He allegedly entered into a sexual relationship with a patient, gave the patient medication samples without documenting it in her patient records, and prescribed medications to the patient when he knew the medications were intended for her pet.

Snohomish County

In July 2010 the Health Care Assistant Program terminated the order of probation against Elisha A. Ellis (HC60016982).

In June 2010 the Dental Commission denied the application of dental assistant Angela D. Lambert (D160136930). She was convicted of driving under the influence.

Spokane County

In July 2010 the Health Care Assistant Program reinstated the license of Shannon B. Belisle (HC00154966) and placed her license on probation for three years. She must comply with terms and conditions set against her license.

In July 2010 the Examining Board of Psychology reinstated the credential of Edward L. Deatherage (PY00000714) and placed restrictions against his license.

In July 2010 the Nursing Assistant Program charged certified nursing assistant Kimberley A. Norman (NA00199503, NC60012288) with unprofessional conduct. She was convicted of seven counts of rendering criminal assistance.

Thurston County

In July 2010 the Nursing Assistant Program granted the reactivation application of certified nursing assistant Pabior C. Ajang (NC10085671) and placed conditions against his certification. He was convicted of driving under the influence on more than one occasion.

In July 2010 the Health Care Assistant Program charged Jolene R. Peters (HC00136657, HC00152657) with unprofessional conduct. She allegedly failed to comply with terms and conditions set against her license.

Walla Walla County

In July 2010 the Acupuncture Program reinstated the license of Alfred Laggner (AC00002980) and placed conditions against his license.

Whatcom County

In July 2010 the Nursing Assistant Program granted the applications of certified nursing assistant Trisha M. Killian-Flye (NA60137819, NC60137660) and placed conditions against her registration and certification. She was charged with under the influence of a controlled substance.

In July 2010 the Massage Program charged Cheryl Ann Saxer (MA00004881) with unprofessional conduct. She was convicted of malicious mischief, driving under the influence, theft, driving while license suspended or revoked, and assault.

Yakima County

In July 2010 the Unlicensed Practice Program issued a notice of intent to issue cease and desist order to Robyn Fiebelkorn. She allegedly practiced medicine by using prescriptive laser, and/or prescriptive intense pulse light devices on patients for the purposes of hair removal, and removal of a tattoo. The tattoo removal allegedly resulted in patient harm. She doesn’t have a license to practice medicine in the state of Washington.

In July 2010 the Unlicensed Practice Program issued a notice of intent to issue cease and desist order to Adrianne Phillips. She allegedly practiced medicine by using prescriptive laser, and/or prescriptive intense pulse light devices on patients for the purposes of hair removal. She doesn’t have a license to practice medicine in the state of Washington.

In July 2010 the Office of Emergency Medical Services and Trauma Systems Program charged emergency medical technician Chad Sainsbury (ES01174109) with unprofessional conduct. He was convicted of assault.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
