State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA
The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s website. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County 

In November 2010 the Board of Osteopathic Medicine and Surgery suspended the license of osteopathic physician and surgeon Hosain Beckmohamadi (OP00001241). Beckmohamadi was convicted of fourth degree assault with sexual motivation in 2009 for conduct toward a patient at the clinic where he worked. He was also convicted of third degree assault in 2009.

Clark County

In December 2010 the Secretary of Health suspended the license of licensed practical nurse Matthew A. Long (LP00059133). Long taped his cell phone underneath a sink in a private restroom at a medical facility where he worked, and filmed a coworker using the restroom. Both employees and patients used the restroom. Long pleaded guilty to one count of voyeurism relating to the conduct.

Franklin County 

In November 2010 the Nursing Commission indefinitely suspended the license of licensed practical nurse Helen Henrikson (LP00044172). Henrikson failed to comply with a previous order.

Grant County 

In December 2010 the Nursing Assistant Program indefinitely suspended the credential of Leanne R. Mueller (NA60001873). Mueller failed to comply with a previous order. 

Island County

In December 2010 Nursing Assistant Program suspended the credential of Kelley F. Bugg (NA00112187). Bugg used a resident’s debit card for a purchase without permission. Bugg also withdrew money from the resident’s bank account. She pleaded guilty to third degree theft.

King County 

In December 2010 the Pharmacy Board indefinitely suspended the credential of pharmacy technician Sara M. Gagliardo (VA00067549). Gagliardo failed to comply with a previous agreed order.

In December 2010 the Nursing Commission indefinitely suspended the license of licensed practical nurse Kenneth W. Kuenz (LP00038805). Kuenz failed to comply with a previous order.

In November 2010 the Medical Commission accepted the license surrender of physician Geoffrey A. MacPherson (MD00011292). MacPherson kissed a patient’s mother in a passionate manner, touched her inappropriately over and under her clothing, and also pursued a personal relationship with her after she asked him to stop contacting her. He also kissed another patient’s mother in a sexual manner. MacPherson lied to a department investigator regarding his conduct. He agreed to not resume the practice of medicine in Washington including any temporary, emergency, or volunteer practice.

In December 2010 the Marriage and Family Therapist Program entered into an agreed order to suspend the credential of James A. Rapson (LF00001324). Rapson had sexual contact with a patient after ending care. While treating the patient, Rapson used physical contact including handholding and hugging. He also failed to keep proper treatment records during their sessions. 

In December 2010 the Nursing Assistant Program suspended the credential of certified nursing assistant Nythanya K. Sa (NC10072752). While a live-in caregiver for two patients, Sa allowed her children to live in the patients’ homes without their permission. 

In December 2010 the Nursing Assistant Program suspended the credential of Eric D. Silver (NA00198269). Silver left an Alzheimer’s patient alone when the patient’s care plan required full-time care.

In December 2010 the Nursing Commission indefinitely suspended the license of registered nurse Ora Pearl Smith-Gutter (RN00079416). Smith-Gutter failed to comply with a previous order.

Mason County

In December 2010 the Nursing Commission revoked the license of licensed practical nurse Emilio Torrella (LP00051104). In 2009 Torrella was convicted of one count of conspiracy to commit wire fraud, one count of wire fraud, and one count of aggravated identity theft in federal court.

Pend Oreille County

In October 2010 the Secretary of Health ordered Janelle L. Belton-Leslie to permanently cease and desist in any and all conduct constituting the practice of registered nursing in Washington unless she is properly licensed or otherwise meets an exemption. Belton-Leslie must also pay a $10,000 fine. She represented herself as a registered nurse and provided another person’s credential number to get a job at a medical facility. She performed duties of a registered nurse for multiple patients without being properly licensed.

Pierce County

In December 2010 the Pharmacy Board suspended the credential of pharmacy assistant Carrie L. Nelson (VB60070067). Nelson stole 80 tablets of Oxycontin from the pharmacy where she worked. She distributed them to friends and also took some herself. 

Snohomish County 

In December 2010 the Nursing Commission indefinitely suspended the license of registered nurse Kathleen Gilham (RN00096708). Gilham is unable to practice with reasonable skill and safety. In 2007 she reported suicidal thoughts and mentioned that she had homicidal feelings towards her patients and that she sometimes was inclined to overmedicate them to cope.

Spokane County

In December 2010 the Chiropractic Commission entered into an agreed order to indefinitely suspend the license of chiropractor Willis S. Muncey (CH00002393). Muncey engaged in sexual contact with a patient on numerous occasions, including in his chiropractic office.

In December 2010 the Massage Program entered into an agreed order to suspend the credential of Anthony D. Pfeiffer (MA00007225). Pfeiffer massaged a client’s breast and exposed buttock during a massage session. He was convicted of indecent liberties for this conduct in 2010

In December 2010 the Medical Commission withdrew the statement of charges against physician Edward J. Reisman (MD00024932).

In November 2010 the Pharmacy Board suspended the credential of pharmacy technician Thao Ropp (VA00021759). Ropp was convicted of third degree theft. Ropp is prohibited by the court from contacting or providing care to vulnerable adults for 24 months, and is also barred from participating in Medicare, Medicaid, and all federal health programs.

In November 2010 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant James L. Sale (NA00192552, NC60017246). Sale became angry with a patient, used inappropriate language toward the patient, and failed to completely provide the patient care.

Thurston County

In November 2010 the Pharmacy Board modified a prior order to accept the license surrender of pharmacist John W. Morse (PH00016723).

In December 2010 the Health Care Assistant Program suspended the credentials of Jolene R. Peters (HC00136657, HC00152657). Peters failed to comply with a previous stipulation.

In November 2010 the Nursing Assistant Program suspended the credential of Sharin M. Sesin-Lucca (NA00130961). Sesin-Lucca was convicted of third degree assault and driving under the influence twice in 2010. 

Yakima County

In October 2010 the Unlicensed Practice Program ordered Robyn Fiebelkorn to permanently cease and desist in any and all conduct constituting the practice of medicine in Washington unless she is properly licensed or otherwise meets an exemption. Fiebelkorn must pay a $1,000 fine. She practiced medicine by using prescriptive laser, and/or prescriptive intense pulse light devices on patients for the purposes of photo-facials. She also used a prescriptive intense pulse light device in an attempt remove a tattoo. The tattoo removal resulted in patient harm. She doesn’t have a license to practice medicine in Washington. 

Out of State

California: In December 2010 the Medical Commission vacated the October 2010 summary suspension and withdrew the statement of charges without prejudice against physician Ahmed G. Rashed (MD00039390).

Georgia: In December 2010 the Medical Commission indefinitely suspended the license of physician Ernest J. Steinhilber, III (MD00039123). Steinhilber surrendered his license to practice medicine in Alaska and he remains prohibited from practicing in that state. His license to practice medicine in New York was revoked; he remains prohibited from practicing in that state. He also surrendered his license to practice medicine in Kentucky, Connecticut, Massachusetts, and Pennsylvania.
