Yellow Fever Vaccination Center Authorization Process
If you wish to purchase and administer yellow fever vaccine, you must follow
the process outlined below to become an authorized provider.
- Contact the Washington State Immunization Program CHILD Profile by phone or
email to request the YFV Stamp. Be prepared to give your medical license
number and the year the license was issued when you make your request.
- The Yellow Fever Coordinator will respond to your request and ask for
additional demographic information.
- The Yellow Fever (YF) Coordinator will verify that your license is
up-to-date. Upon approval of your request, a YFV stamp will be sent along with a
YF Vaccine Information Statement (VIS) and a letter explaining your
responsibilities.
- A letter will also be sent to Sanofi Pasteur, the YF vaccine manufacturer,
to notify them a YFV stamp has been issued. Once the manufacturer receives the
letter, you will be able to purchase YF vaccine.
- Clinic contact information will be entered in the Centers for Disease
Control and Prevention’s (CDC)
Yellow Fever Vaccination Clinic Database. ** The public can use this
database to locate Yellow Fever Vaccination Clinics.
