Washington State Yellow Fever Provider

Authorization Process

If you want to buy and give yellow fever vaccine, follow this process to become an authorized provider:

  1. Fill out the Yellow Fever Vaccine Provider application. Contact Wendy Bowman or call 1-866-397-0337 if you have questions.
     
  2. The Yellow Fever Coordinator will respond to your request and ask for more information.

  3. The Yellow Fever Coordinator will verify that your license is up to date. After your request is approved, a yellow fever vaccination package, including the official stamp, will be sent to you.

  4. A letter will also be sent to sanofi pasteur, the yellow fever vaccine manufacturer, to let them know a yellow fever vaccination stamp has been issued.

  5. Clinic contact information will be entered into the Centers for Disease Control and Prevention’s Yellow Fever Vaccination Clinic Database. The public can use this database to locate Yellow Fever Vaccination Clinics.
