Order Free Immunization Materials

Free immunization materials may be ordered online, by e-mail, or by fax. Faxes will not be accepted unless you do not have Internet access. Ordering materials online is the easiest and fastest way to place your order. If you do not have Internet access, please contact Lonnie Malone at 360-236-3529 for an order form and information on how to place your order by fax.

• Order Materials Online:
  • Go to the Department of Printing Fulfillment Center General Store.
  • For steps on how to order online, view How To Order Materials (PDF, 36 KB).
• Order Materials by E-mail:
  • Send an e-mail to ImmuneMaterials@doh.wa.gov including:
    • The material(s) you want to order.
    • The quantity of each material.
    • Your name, address, and phone number.
