When did the Washington State Patient Centered Medical Home Collaborative begin?Registration began March 2009, and continued through May 2009. Successful applicants were notified the first week of June. The first learning session will be held September 29 & 30, 2009. The collaborative will continue through 2011.How many teams are enrolled in the Collaborative?There are 33 teams currently registered. How will success be measured?
