How Can I Participate in the Child Profile Immunization Registry?
Steps to
register for participation in the Immunization Registry:
- Sign an information
sharing agreement.
- Complete the
Immunization Registry account application.
- Schedule training.
You can
also complete our
online
practice profile.
Contact the
CHILD Profile Help Desk
at 1-800-325-5599 to request forms or for more
information.
