Step 5: Get Your User Account
Child Profile will send a welcome letter to your school
district superintendent once we get a signed
Child Profile Information Sharing Agreement
(PDF, 58KB). Your superintendent or designee
will notify school principals and nurses within the school
district that access to the Immunization Registry has been
initiated.
User names and passwords
will be created centrally by Immunization
Registry staff. Your school nurse will be your contact. A
signed
Confidentiality Agreement (PDF, 16KB) is required to get
a user account.
The school nurse will gather
signed agreements from each staff person assigned to
work with the Immunization Registry. These forms can be kept
on site or in the district office.
Using the
Web Account Information Worksheet
(PDF, 24KB), school nurses will fax
their name and user account information along with that of
other assigned staff. The address and fax number is on the
worksheet.
User names and temporary
passwords will be sent back to the school nurse for
distribution.
Returning users
Check with your lead nurse
if you forget your user name. The lead nurse submits your
name and e-mail address to the Immunization Registry.
Forgot your password?
- Go to the
Immunization Registry home page.
- Click on
Login.
- Enter your user name
and click “Forgot Password.” The Immunization Registry
automatically e-mails you a temporary password.
- Enter the temporary
password.
- On the “Change
Password” screen, enter your temporary password, then
enter a new password twice. Passwords must be at least
six characters and include one number.
- Click "Submit."
The Immunization Registry
reminds you to change your password every 90 days.
Once training is done and
you have a user name and password, go to
www.childprofile.wa.gov, click on
Login, and enter your user name and password. You are
ready to search!
