State Plan  

The Washington State Diabetes Plan (PDF 167KB), launched in March 2005, is a 20-page printed document containing 10 goals to be addressed over a five-year period. The plan outlines the relationship between the goals, specific activities, network organizations and the 10 Essential Public Health Services.
