Washington State Collaborative To Improve Health

The Washington State Collaborative to Improve Health helps health care teams improve clinical outcomes for their patients with chronic disease.

The Collaborative:

  • Is co-sponsored by the Washington State Department of Health and the University of Washington Child Health Institute.

  • Is a 13-month quality improvement program to help medical practices make changes to improve the care they provide to patients.

  • Takes a team approach to quality improvement, rather than focusing only on the doctor-patient relationship.

  • For 2008-2009 the Collaborative addresses these topics: hypertension management, diabetes, asthma, obesity prevention in children, and medical home for children with special healthcare needs.
