Report Suspected WIC Fraud and Abuse

If you think someone is abusing the WIC Program, you can let WIC know.

Some people abuse WIC by:

  • Selling their WIC infant formula.
  • Lying to get on WIC.
  • Cheating in the store.
  • Trading WIC checks for things other than WIC foods.

How to contact WIC about fraud and abuse

You can tell WIC in four different ways:

  1. Call us at 1-800-841-1410 extension 3620. You’ll be connected to the Customer Service Representative. This WIC staff person is available Monday through Friday, from 8 a.m. to 5 p.m. You can leave a message at any time. You do not have to give your name.
  2. Email the WIC Fraud Reporting Form (81.79 KB) to wicfraud@doh.wa.gov
  3. Fill out the form and mail it to:

    PO Box 47886
    Olympia WA 98504
  4. Fax the form to 360-236-3777.
