What is a collaborative?

A collaborative is a learning and quality improvement process where 15-40 healthcare teams work to measure and improve the quality of care delivered by their practices during a project period of a year or more.

Collaborative teams:

  • Test and measure practice innovations on a defined group of patients.
  • Share their experiences and learning among all participating teams.
  • Spread the resulting best practices throughout their systems of care.

The Washington Patient Centered Medical Home Collaborative, jointly sponsored by the Department of Health  and the Washington Academy of Family Physicians offer proven tools to:

  • Family medicine practices
  • Internal medicine practices
  • Pediatric practices
  • Geriatric practices


The clinics improve outcomes for their patient. Collaborative participants create a rich learning community to share information and effective ideas.
 

When did the Washington State Patient Centered Medical Home Collaborative begin?

Registration began March 2009, and continued through May 2009. Successful applicants were notified the first week of June. The first learning session will be held September 29 & 30, 2009. The collaborative will continue through 2011.

How many teams are enrolled in the Collaborative?

There are 33 teams currently registered.

How will success be measured?

The Washington State Patient Centered Medical Home Collaborative will be evaluated in five categories:
  • Patient experience
  • Provider satisfaction
  • Clinical Outcomes
  • Degree of medical home implementation
  • Cost and utilization
