Washington State Collaborative: A proven model for health-care improvement

The Washington State Collaborative to Improve Health, sponsored by the Washington State Department of Health, offers proven tools for pediatric and family medical practices  to improve outcomes for their patients with chronic diseases.

A collaborative is a learning process where 15-40 medical teams work to improve the quality of care delivered by their practices.
